      • 만성육아종성염증에서의 T-림프구, B-림프구 및 자연살세포의 분포에 관한 비교 연구

        Chronic granulomatous inflammation, such as tuberculosis, leprosy, parasitic infestation foreign body granuloma, is charecterized by the formation of granuloma. Especially, leprosy shows much difference of immune reaction and prognosis according to subtypes. Tuberculoid leprosy shows adequate immune reaction and good prognosis with localized lesion, but lepromatous leprosy shows immune nonresponsiveness and poor prognosis with frequent dissemination. Borderline leprosy is intermediate type beteween tuberculoid and lepromatous leprosy. Numerous studies have been performed to understand the cell mediated immune response on leprosy and tuberculosis, but the immune mechanism of natural killer cell on granulomatous inflammation is not understood well. Therefore, to understand distribution of T-lymphocyte, B-lymphocyte, and natural killer cell on granulomatous inflammation, the author collected 32 cases, such as tuberculosis 12 cases; leprosy 9 cases, cysticercosis 5 cases, and foreign body granuloma 6 cases, of formalin fixed, paraffin embedded tissue obtained by biopsy, performed immune histologic reaction by avidin-biotin complex method using MT1, LN2, and anti-Leu7. Results obtained are as follows : 1. In tuberculosis, there were numerous MT1 positive cells. In contrast, relatively few LN2 positive cells wese observed. 2. In the inner and outer areas of granuloma, MT1 positive cells were more abundant in tuberculoid and borderline leprosy than in lepromatous leprosy. LN2 positive cells were also demonstrated. 3. In cysticercosis, LN2 positive cells, rather than MT1 positive cells, were distributed in the periphery of granuloma. In the foreign body granuloma, many MT1 positive cells, but not LN2 positive cells were found. 4. Anti-Leu7 positive cell was observed in all cases of tuberculoid and borderline leprosy and not in all cases of tuberculosis. 5. Cysticercosis and foreign body granuloms showed infiltration of anti-Leu7 positive cells in a few cases. The above results suggest that, in tuberculoid and borderline leprosy, natural killer cell is involved in the defense mechanism.

        교정구에 의한 음향도플러유향유속계의 평균 체적후방산란강도 검토

        ADCPs have been widely used to estimate the dynamic characteristics and biomass of sound scattering layers (SSLs), and swimming speed of fish schools for analyzing SSLs spatial distribution and/or various behavior patterns. This result showed that the verification of the mean volume backscattering strength (MVBS or averaged SV, dB) acquired by the ADCP would be necessary for a quantitative analysis on the spatial distribution and the biomass estimation of the SSLs or fish school when ADCP is used for estimating their biomass. In addition, the calibrated sphere method was used to verify values of each MVBS obtained from 4 beams of ADCP (153.6 kHz) on the base of 3 frequencies (38, 120, 200 kHz) of Scientific echo sounder``s split beam system. Then, the measured SV values were compared and analyzed in its Target Strength (TS, dB) values estimated by a theoretical acoustic scattering model.

        Congenital Thoracic Ectopic Kidney associated with Diaphragmatic Hernia in a 15-month-old Boy

        선천성 흉강내 신장은 이소성 신장의 가장 드문 형태로 알려져 있다. 흔히 증상이 없어 우연히 흉부 방사선 촬영을 통해 발견 되며, 주로 성인 시기와 산전 진단에서 진단 받는 경우가 많은 것으로 알려져 있다. 선천성 흉강내 신장은 다른 동반기형이 없는 무증상의 경우 수술적 치료가 필요하지 않으며, 횡격막 탈장이 동반된 경우 출생 후 수일 이내에 교정 수술이 필요 하다. 이에 저자들은 분만력, 과거력상 특이 소견이 없었던 15개월 남아로 간헐적인 심한 보챔을 주소로 내원하였고 흉부 방사선 촬영에서 종격동 종괴를 보여 시행한 초음파와 스캔에서 신장 위치의 이상을 확인하였고, MRI를 통해 횡격막 탈장을 확인한 예를 경험하였기에 보고하는 바이다. Congenital thoracic ectopic kidney is a very rare developmental disorder and the rarest type of ectopic kidney. This condition is usually asymptomatic and detected incidentally on routine chest radiography. Most cases of thoracic ectopic kidney develop in adulthood and during the neonatal period, and congenital thoracic ectopic kidney rarely develops in children. Most patients are asymptomatic, and the treatment depends on the diagnosis. Herein, we report a rare case of ectopic thoracic kidney associated with a diaphragmatic hernia in a 15-month-old male infant, who presented with periodic severe irritability. The thoracic ectopic kidney was detected as a mass in the right base of the chest on routine chest radiography.

        Nowadays, many manufacturing companies design, engineer, and produce their products through a globally distributed supply chain. In the automotive industry, globalization of the supply chain is expanding and becoming more complicated under increasing global competition and demand for reduction in production costs. Under such circumstances, automotive parts manufacturers are making efforts to implement smart manufacturing (SM) system that ensures the production of products with reasonable prices and high quality, while meeting the needs of various customers. To implement SM, a production line must be smart based on data from manufacturing sites through the application and convergence of technologies such as cyber-physical system(CPS) and digital twin. With the application of such technologies, it is possible to achieve faster and more accurate decision-making and more efficient utilization of manufacturing resources. In addition, a CPS-based integrated platform is needed to link and integrate data with the distributed manufacturing environment composed of heterogeneous facilities. By utilizing this, it is possible to implement and operate a production system that can flexibly respond to various external changes. In this paper, we propose a methodology for design, implementation and application of CPS platform-based digital twin that meets global standards for SM of global automotive parts manufacturers.

        Scale-up of stirred media mills for fly ash grinding

        In this study, fly ash was ground for use as a CO2 foam stabilizer. The investigated set of mills comprised 1 L and 10 L batch-type wet grinding stirred media mills. The employed grinding media was zirconia (ZrO2) beads with a density of 6 g/cc and mean diameter of 1.0 mm. Grinding experiments were conducted under different rotational speeds of the rotor. The investigated rotational speeds varied from 360 to 800 rpm for each type of mill. Based on the experimental results, a scale-up procedure related to the net specific grinding energy was developed, taking mill dimension into consideration. With the developed procedure, the D50 particle size could be estimated on the basis of operating conditions such as the rate of revolution and grinding time. Finally, this study investigated a scale-up procedure for stirred media mills, and the useful application of the function to predict mean particle size.

        인지 증진을 위한 보드게임 개발 및 효과성 검증

        목적: 본 연구에서는 노인을 대상으로 보드게임을 직접 개발하여 보드게임이 인지기능 중 기억력과 우울감 감소에 미치는 효과를 알아보고자 한다. 연구방법: 본 연구는 2020년 10월 까지 기초조사와 자료 분석을 통해 보드게임을 개발하여 제주시의 65세 이상의 노인 2명을 대상으로 실시하였고, 단일대상연구 설계 중 AB 디자인을 통해 기초선 3회와 중재 10회, 총 13회를 실시하였으며, 평가는 전반적인 인지기능과 우울감을 알아보기 위해 사전ㆍ사후에 MoCA-K와 GDS-K를 사용하였다. 연구결과: 작업치료 교수 및 임상가의 자문을 구해 ADL 요소 40가지를 선택하고 보드게임의 규칙을 만들었다. 개발한 보드게임을 시행한 결과, 대상자 A, B 모두 기초선 3회기의 평균 ±2 표준편차 밴드보다 중재기의 결과값이 전반적으로 높게 나타났다. 그리고 MoCA-K의 결과값이 중재 전보다 중재 후에 점수가 증가하였고 GDS-K의 결과값은 점수가 하락하여 인지 기능의 증가와 우울감이 감소하는 경향을 보였다. 결론: 보드게임을 통해 기억력의 긍정적인 변화가 나타났다. 또한 인지기능과 우울감의 감소에도 영향을 미치는 것을 확인했다. 하지만 대상자의 수가 적어 일반화하기 어렵고 보드게임의 방법 및 구성품의 수정이 필요하다. 추후 연구를 통해 문제점들을 보완한 보드게임이 계속 개발되어 노인의 인지기능 및 삶의 질 향상에 기여할 수 있기를 기대한다. Objective : The purpose of this study is to develop a board game for the elderly to find out the effects of board games on the reduction of memory and depression among cognitive functions. Methods : This study developed a board game through basic survey and data analysis until October 2020, targeting two elderly people aged 65 years or older in Jeju-si. After a total of 13 times, MoCA-K and GDS-K were used before and after evaluation to feel overall cognitive function and depression. Results : Through consultation with occupational therapy professors and clinicians, 40 ADL elements were selected and the rules of the board game were created. In both subjects A and B, the results of the intervention period were higher than the mean ±2 standard deviation band of the 3 baseline sessions after they played the developed board game. In addition, the score of MoCA-K increased after the intervention compared to before the intervention, and the score of GDS-K decreased, leading to an increase in cognitive function and decrease in depression. Conclusion : Through the board game, a positive change in memory appeared. It was also confirmed that it has an effect on cognitive function and reduction of depression. However, it is difficult to generalize due to the small number of targets, and the method and components of the board game need to be modified. It is hoped that through further study, board games that complement the problems will continue to be developed and contribute to the improvement of cognitive function and quality of life of the elderly.

        Targeted exome sequencing resolves allelic and the genetic heterogeneity in the genetic diagnosis of nephronophthisis-related ciliopathy

        Nephronophthisis-related ciliopathy (NPHP-RC) is a common genetic cause of end-stage renal failure during childhood and adolescence and exhibits an autosomal recessive pattern of inheritance. Genetic diagnosis is quite limited owing to genetic heterogeneity in NPHP-RC. We designed a novel approach involving the step-wise screening of Sanger sequencing and targeted exome sequencing for the genetic diagnosis of 55 patients with NPHP-RC. First, five NPHP-RC genes were analyzed by Sanger sequencing in phenotypically classified patients. Known pathogenic mutations were identified in 12 patients (21.8%); homozygous deletions of NPHP1 in 4 juvenile nephronophthisis patients, IQCB1/NPHP5 mutations in 3 Senior–Løken syndrome patients, a CEP290/NPHP6 mutation in 1 Joubert syndrome patient, and TMEM67/MKS3 mutations in 4 Joubert syndrome patients with liver involvement. In the remaining undiagnosed patients, we applied targeted exome sequencing of 34 ciliopathyrelated genes to detect known pathogenic mutations in 7 (16.3%) of 43 patients. Another 18 likely damaging heterozygous variants were identified in 13 NPHP-RC genes in 18 patients. In this study, we report a variety of pathogenic and candidate mutations identified in 55 patients with NPHP-RC in Korea using a step-wise application of two genetic tests. These results support the clinical utility of targeted exome sequencing to resolve the issue of allelic and genetic heterogeneity in NPHP-RC.
