내독소에 의한 내피세포와 Albumin 및 Fibronectin간의 결합력의 변화

목적 : 내독소에 노출된 후의 내피세포와 혈장 단백의 결합력의 변화를 규명하기 위해 본 연구를 시행하였다. 재료 및 방법 : Sprague-Dawley계 흰쥐에 8㎎/㎏의 내독소를 투여한 후 알부민과 fibronectin에 교질금을 부착한 결합체를 폐동맥을 통해 관류한 후 전자현미경으로 관찰을 시행하였다. 결과 및 결론 : 알부민과 fibronectin은 내피세포하 간질 조직에 축적되었으며 대조군에 비해 내독소를 투여한 군에서 그 양이 상당히 감소되었다. 그리고 내피세포의 혈관강쪽 세포막에는 국소적으로 관찰되었다. 내독소를 투여한 군의 내피세포는 부종과 불규칙한 세포질 돌기가 형성되었다. 내독소에 의해 알부민과 fibronectin의 내피세포와의 결합이 감소하며 이는 내독소에 의한 내피세포의 알부민 및 fibronectin에 대한 수용체의 상해로 알부민 및 fibronectin의 내피세포를 통한 수송의 감소가 일어난 것으로 해석된다. The author studied the transendothelial transport of albumin and fibronectin in endotoxin-induced lung injury. The lungs of Sprague-Dawley rats were examined by electron microscopy at 3 minutes after perfusion of colloidal gold labelled albumin and fibronectin respectively at 6 hours after intraperitoneal injection of 8㎎/㎏ of E. coli endotoxin. The results obtained were summarized as follows ; Both albumin and fibronectin were accumulated in the subendothelium. The amounts of subendothelial albumin and fibronectin were decreased compared to control group. Endothelial swelling and irregular cytoplasmic processes of endothelium were observed in alveolar capillaries after endotoxin injection. These results suggest that decrease of transendothelial transport of albumin and fibronectin is caused by endotoxic injury of albumin and fibronectin receptors of the endothelium.

Anorectal malformations comprise a spectrum of disease and the majority of patients have one or more abnormalities that affect other systems. In evaluating a newborn with anorectal malformation, the decision regarding the need for a colostomy and detection and management of any life threatening associated anomalies are thetwo most important considerations. Perineal inspection provides the clue to the surgical approach in about 80-90 % of male and 90 % of female newborn baby. The remaining patients who do not show any clinical evidence need radiologic evaluation to decide whether a colostomy should be performed. In most cases the decision to make a colostomy should not be made until the baby is 20 to 24 hours old and evaluation to rule out the presence of associated anomalies completed. A divided colostomy at the junction of the descending and sigmoid colon is recommended for anorectal malformations.
