        군에서 발생한 안외상의 통계적 고찰

        Purpose: We investigated the clinical characteristics of ocular trauma in the military for prevention and treatment application. Methods: We retrospectively surveyed epidemiologic characteristics by investigating the medical records of 790 patients who were hospitalized in the Armed Forces Capital Hospital from January 1, 2001 to December 31, 2010 and investigated the prognostic factors that influenced visual outcome. Results: Among the 790 patients with ocular trauma, 22.9% of the patients had an open injury and 77.1% had a closed injury. The most common cause of injury was sports-related ocular trauma (39%) and fatigue duty-related trauma (23.4%). The following 8 risk factors were considered poor prognostic factors: open injury, involved posterior segment, operation, initial visual acuity of 0.1 or less, corneal laceration, hyphema, RD, and intraocular foreign body. There was a significant correlation between the probability of poor visual outcome and the number of risk factors (correlation coefficient = -0.468, p < 0.0001). Conclusions: In the present study, the cause, characteristics and prognostic factor of military personnel’s ocular injury were determined. The results can be useful in the prevention and management of ocular injury in the military.

        동일한 최소량의 하사근후전술에 따른 상사근마비의 치료효과

        Purpose: To investigate the effect of vertical deviation change, head tilt and diplopia relief, we performed a minimal amount of inferior oblique recession regardless of the vertical deviation angle and the degree of inferior oblique overaction (IOOA) in superior oblique palsy patients. Methods: We retrospectively analyzed 20 cases of unilateral congenital or acquired superior oblique palsy, which were treated with inferior oblique muscle recession from May 2005, to August 2007. We compared vertical deviation change, head tilt and diplopia relief following surgery. Improvement of head tilt was determined by patient satisfaction measurements. The inferior oblique muscle was attached 4 mm posterior and 2 mm temporal to the lateral border of the inferior rectus muscle insertion in all patients. Results: The average angle of vertical deviation prior to surgery was 12.4 prism diopters (PD), and the total average correction in the angle of vertical deviation after surgery was 9.6PD. After surgery, head tilt improved in 13 of 16 eyes(81%), and of the four remaining eyes, vertical diplopia was absent. Conclusions: Although vertical deviation remained after surgery, if patients have enough vertical fusional amplitude, they should be able to overcome the diplopia and the head tilt. Judging from the indication of inferior oblique recession in diplopia and cosmetic problem in superior oblique palsy, a minimal amount of inferior oblique recession is thought to be an effective treatment of unilateral superioroblique palsy with no significant IOOA.

        당뇨황반부종에서 다국소 망막전위도검사와 치료 후 시력과의 상관관계

        Purpose: To evaluate the prognostic value of multifocal electroretinography (mfERG) after a decrease of retinal thickness in eyes with diabetic macular edema (DME). Methods: Sixty patients (60 eyes) underwent intravitreal injection of triamcinolone acetonide or bevacizumab for diabetic macular edema. Eyes were divided into three groups, according to foveal thickness : Group A (<500 μm, n=20 eyes), Group B (500 μm≦ <800 μm, n=20 eyes), and Group C (≧800 μm, n=20 eyes). Best-corrected visual acuity (BCVA) and mfERG responses from the most central seven hexagons were analyzed both before and after treatment. Results: The pre-injection P1 amplitudes of mfERG correlated significantly with end-point BCVA in Groups A and B (p<0.05), but did not correlate with end-point BCVA (p>0.05) in Group C. Conclusions: The level of P1 amplitudes of mfERG may be a useful tool to predict visual outcomes after a decrease of retinal thickness in patients with diabetic macular edema.         안와골절 후 상악동으로의 안구 탈출 1예

        `Purpose: We report a case of a 38-year-old man who suffered a blowout fracture of the orbital wall with an intact eyeball entrapped within the maxillary sinus after trauma. Case summary: The 38-year-old man was admitted to the emergency room after sustaining a work-related trauma. His chief complaints were loss of vision and bleeding from the left periorbital area. He had no light perception and no eyeball was found in the orbit. Facial CT revealed that the intact eyeball was entrapped within the maxillary sinus. The condition of the optic nerve was difficult to ascertain. Ten hours after post-trauma, reduction surgery was done with a graft from the iliac bone. Ruptured extraocular muscles were not primarily sutured. After four months, vitrectomy was performed on the left eye. The eyeball was repositioned in its place. He had no light perception. Extraocular motility improved at the last follow-up examination. Conclusions: We report the dislocation of the eyeball globe into the maxillary sinus after a blowout fracture. Visual acuity showed no light perception as a result of central retinal artery occlusion and optic nerve injury. We were able to obtain a good aesthetic and functional result after the operation.`

        ($Interferon-{\gamma}$)가 마우스 조골세포의 생물학적 활성에 미치는 영향에 관한 연구

        Interferon(IFN) is a sort of glycoproteins that are produced by activated lymphocyte, monocyte and fibroblast. IFN has anti-viral effects, immuno-defensive mechanism and regulating properties to the several kinds of cells that includes affect on the bone formation and resorption. The effect of IFN on the osteoclast & other tissue cells has been studied in a number of researchers with the limited reports on the osteoblast. The purpose of this study was to evaluate the effects of IFN on the osteoblastic function. The MC3T3/El cell(Mouse osteoblast) was incubated in ${\alpha}-minimum$ essential medium containing 10% FBS. To detect the cytotoxic effect of $IFN-{\gamma}$ on osteoblast, the cells were cultured in 96-well plate to which $IFN-{\gamma}$ of various concentrations were added for 2 days. After staining with trypan blue, total cells and living cells were counted under microscope. To determine the activity of alkaline phosphataset(ALP), various concentrations of $IFN-{\gamma}$ were treated to culture medium, and biochemical assay was performed. $IFN-{\gamma}$ and $IFN-{\gamma}$ plus cycloheximide were added to culture medium separately and then ALP activity were determined. To detect the effect of the $IFN-{\gamma}$ on the bone formation of osteoblast, long-term culture was performed, and calcified nodule formation were observed using von Kossa's staining. After the addition of $IFN-{\gamma}$ with various concentrations to the medium, no cytotoxic effect of $IFN-{\gamma}$ was detected at any concentration. The significant increase in ALP activity of osteoblast were found the concentration of $IFN-{\gamma}$ 500-2500U/ml and the culture time of 24-48 hours respectively. The enhancement of ALP activity by $IFN-{\gamma}$ of osteoblast was decreased significantly by the treatment of cycloheximide. After long-term culture of osteoblast, the nodule formation was found to be increased in number and density by the addition of 500 U/ml $IFN-{\gamma}$. These results suggest that $IFN-{\gamma}$ was affected on the bone formation of osteoblast. Forthemore this kind of study or $IFN-{\gamma}$ to osteoblast will be held continuously.

        통신용 증폭기의 ESD 고장분석과 대책

        Low-noise amplifier(LNA) is a component that amplifies the signal while lowering the noise figure of high-frequency signal. LNA holds a very important position in RF system so that it is widely used for telecommunication. Electro static discharge(ESD) is the most common cause of malfunction for low-powered components, such as Large Scale Integration and IC type LNA is weak in ESD. This thesis studies static effect of communication LNA. It analyzes ESD effect, which occurs within LNA circuit, and describes testing standard and methods. In order to find out LNA"s susceptiblity to electro static, two well-recognized communication IC type LNA models were selected to be tested. Then static-induced malfunction was carefully analyzed and it suggests architectural problem and improvement from the LNA"s ESD point of view.

      • 치은 섬유아세포 배양시 합성되는 단백질에 관한 연구

        This experiment was performed to study the effects of fibroblast growth factor(FGF) and fetal bovine serum(FBS) on the protein synthesis and proliferation of human gingival fibroblasts. Gingival tissues obtained from healthy human were diced into 1 ㎣ and cultured in α-MEM containing 50% FBS for 7~10 days. After treatment with trypsin-DETA, the collected gingival fibroblasts were cultured in α-MEM with 10% FBS until confluency was reached. After 24 hour incubation in serum-free media, control groups were cultured in fresh serum-free media, while experimental groups were cultured in serum-free media containing various concentrations of FGF, 2% FBS or both. Cell proliferation was determined by [^3H]-thymidine incorporation rate into DNA, and proteins produced by cultured gingival fibroblasts were deter mined by SDS-PAGE of conditioned media and cell lysate. The obtained results were as follows. 1. In case of incubation of the cultured gingival fibroblast in the serum-free media containing FGF(10ng/ml), it was shown the tendency of cell proliferation as comparison with the control group, but statistically not significant(table 1). 2. In the event of incubation of the cultured gingival fibroblast in the serum free media containing FGF(30ng/ml) and FGF(30ng/ml) plus Fetal bovine serum. The 1st case was statistically not significant, the 2nd case was significantly higher than the control group(table 2). 3. In case that the cultured gingival fibroblast was cultivated in the serum free media containing FGF 50 ng/ml, the cell growth rate was three times as much as the control group. A case of the combined incorporation of FGF 50 ng/ml and 2% FBS, the cell proliferatiion effect was significantly higher than the control group(table 3). 4. The total protein amount about the effects of serum free media containing FGF(50 ng/ml) and FBS was slightly increased but statistically not significant(table 4). 5. After the conditioned media and cell extracts was carried out electrophores, several protein bands were identified by silver stain. The molecular weight of procollagen and fibronectin was showed positive band for the serum free media containing FGF 50 ng/ml(Fig. 2).
