뇌하수체 종양의 방사선 수술 시 영상 융합 프로토콜이 실제 PTV와 OAR 선량에 미치는 영향

뇌하수체 종양의 치료 계획 시 다양한 영상 융합 프로토콜이 실제 PTV (planning target volume) coverage 및 OAR (organ at risk) 보호에 미치는 선량학적 영향을 확인하고자 하였다. 선량 체적 히스토그램(DVH, dose volume histogram)을 이용하여 각 프로토콜 별 실제 PTV의 coverage 및 그 변화 그리고 시각경로(optic pathway) 및 뇌간(brainstem)의 최대 흡수 선량과 임계 선량을 초과한 체적을 획득하였고 축상면(axial plane)과 관상면(coronal) 영상을 함께 사용하여 S-I (superior-inferior) 방향의 불확정도를 보정한 영상 융합 프로토콜이 축상면 만을 이용한 경우보다 시각경로 및 뇌간의 최대 흡수 선량과 임계 선량을 초과한 체적 모두 감소된 수치를 보임을 확인 하였다. 본 case에 한해 축상면 그리고 관상면 영상을 함께 사용하는 것이 OARs 보호에 가중치를 둔 방사선 수술 치료 계획 수립에 도움을 줄 수 있다. The purpose of this study is to verify the dosimetric effect on real PTV (planning target volume) coverage and safety of OARs (organs at risk) at various image fusion protocol-based radiosurgery plan for pituitary adenomas. Real PTV coverage and its variation was acquired and maximum dose and the volume absorbing above threshold dose were also measured for verifying the safety of optic pathway and brainstem. The protocol that can reduce superior-inferior uncertainty by using both axial and coronal MR (magnetic resonance) image sets shows relatively lower values than that of case using only axial image sets. As a result, the image fusion protocol with both axial and coronal image sets can be beneficial to generate OAR-weighted radiosurgery plan.

자기공명영상기반 겔 선량측정법을 이용한 3차원적 목표 중심점 점검기술

전반적 시스템 점검(overall system test)을 위해 일반적으로 필름을 이용한 hidden-target test가 시행되어 왔으나 2차원적 측정기로 3차원적인 방사선 영역(radiation field)을 구(sphere)라 가정하고 목표 중심점을 찾는 것이 내재적 분석 오차를 야기시킨다. 본 연구에서는 겔 선량계를 이용하여 3차원적 목표 중심점 오차를 확인하고 이 기술을 소개하고자 한다. 실제 환자의 두경부를 모사할 수 있으며 10개의 겔 선량계를 내부에 삽입할 수 있는 팬텀을 제작하였고 원하는 시기에 합성과 자유로운 용기 선택이 가능한 $BANGkit^{TM}$을 겔 측정기로서 사용하였다. 필름을 이용하는 분석방법이 야기시키는 내재적 분석 오차를 정량적으로 확인 하기 위하여 2개의 방사선 영역은 타원(ellipse) 나머지 8개는 구 형태로 설정하였다. 방사선 수술 전용 선형가속기 기반의 치료기인 노발리스를 이용하여 모의 치료를 3번 반복 수행하였고 $BrainSCAN^{TM}$의 Image fusion, Drawing, Windowing setting 등의 내장 기능을 이용하여 자기공명영상 상의 방사선 영역을 추출하고 기하학적 중심 점을 측정하였다. 본 연구의 목표 중심점 분석 결과는 10개의 방사선 영역에 대해 $0.77{\pm}0.15mm$의 목표 중심점 오차가 발생하였고 각 AP (anterior-posterior), LAT (lateral), VERT (vertical) 방향으로 $0.54{\pm}0.23mm$, $0.37{\pm}0.08mm$, $0.33{\pm}0.10mm$의 방향별 목표 중심점 오차를 갖는다. 10개의 방사선영역 모두에서 목표 중심점 오차는 1 mm 이내이므로 방사선 수술을 시행하기에 적합한 치료 절차와 치료 장비를 갖추고 있음을 간접적으로 보여주었고 자기공명영상기반 겔선량측정법은 실제 방사선 영역의 체적을 이용 함으로서 겹쳐진 필름을 사용하는 기존 목표 중심점 점검기술의 한계를 보완하였다. 결과적으로, 겔 선량측정법을 이용한 3차원적 목표 중심점 점검기술은 전반적 시스템 점검을 위한 하나의 기술이 될 수 있다. For overall system test, hidden-target test have been used using film which leads to inherent analysis error. The purpose of our study is to quantify this error and to propose gel dosimeter based verification technique for 3-dimensional target point error. The phantom was made for simulation of human head and this has ability to equip 10 gel-dosimeter. $BANGkit^{TM}$ which we are able to manufacture whenever it is needed as well as to easily change the container with different shapes was used as a gel dosimeter. The 10 targets were divided into two groups based on shapes of areas with a planned 50% isodose line. All treatment and analysis was performed three times using Novalis and $BrainSCAN^{TM}$. The target point error is $0.77{\pm}0.15mm$ for 10 targets and directional target point error in each direction is $0.54{\pm}0.23mm$, $0.37{\pm}0.08mm$, $0.33{\pm}0.10mm$ in AP (anterior-posterior), LAT (lateral), and VERT (vertical) direction, respectively. The result of less than 1 mm shows that the treatment was performed through each precise step in treatment procedure. In conclusion, the 3-dimensional target point verification technique can be one of the techniques for overall system test.

벌크터미널 서비스품질 분석 및 이용주체별 차이에 관한 연구: 부산항 벌크터미널을 중심으로

해상운송화물이 꾸준하게 증가하면서 화물을 처리하는 터미널에 대한 관심도 증가하고 있다. 그러나 터미널과 관련된 대부분의 국 내연구는 컨테이너터미널에 국한되어 있다. 본 연구는 벌크터미널의 서비스품질 관점에서 수행하였다. 벌크터미널 서비스 이용자를 크게 선사 와 화주로 구분하여 제공받는 서비스에 대한 차이를 비교분석하였다. 또한 본 연구에서 제안한 속성 가운데 유형성은 선석 수나 수심, 장비 등 의 변수들이 반영되어 설명되기 때문에 형성지표로 분석하는 것이 적합하다고 판단하였다. 따라서 일반적인 구조방정식모델에서 활용되는 반 영적 측정모델뿐만 아니라 형성적 측정모델을 결합한 분석을 시도하였다. 전체 측정모델의 분석결과 공감성과 성과·만족도 간의 관계를 제외한 모든 항목이 유의한 것으로 나타났다. 이용주체별 차이 분석 결과는 화주가 선주보다 신뢰·반응성이 만족·성과에 미치는 영향에 대해 더 중요 하게 평가하고 있는 것으로 나타났다. 이러한 연구결과는 향후 벌크터미널의 경쟁력 확대를 위한 전략적 자료로 활용될 수 있을 것이라 기대된다. While marine transport freight is steadily increasing, people are more interested in terminals handling cargo. But the domestic researches on terminals were almost on container terminal. This study attempted to study the service quality for bulk terminals. The users of bulk terminal services were divided into shipowners and shippers to compare their differences by using bulk terminal service provided. During the attributions proposed in this study, Tangibles were descripted by the numbers of berth or depth, facilities, and so on. So it was suitable to consider the tangibles by formative indicators. And also this study has tried using “reflective model” and “formative model” to make a measurement model for comprehensive analyses. The results showed that all of the items were significant except the relation between empathy and performance·satisfaction in the overall measurement model analyses. User groups difference analyses results have showed that the trust·responsiveness impact on performance·satisfaction was more importantly assessed by shippers than by shipowners. It was expected to use the results of this study as a good strategic resource to help bulk terminal administrators make various expansion plans to improve the bulk terminal’s competitiveness in the future

증례 : 신장 ; 리팜핀 유발성 ANCA 음성 반월상 사구체신염

특이 병력이 없는 56세 남자가 폐결핵 치료 6주 후 전신부종과 신손상으로 왔고 신장 조직 검사를 통해 면역음성반월상 사구체신염으로 진단하였다. 폐결핵을 동반한 상태이므로 세포독성 약물은 사용하지 않았으며 약물 중단과 고용량 메칠프레드니솔론 치료를 통해 신기능 회복을 유도하였고, 리팜핀을 제외한 항결핵 약물로서 안정적으로 폐결핵을 치료할 수 있었다. A 56-year-old male with pulmonary tuberculosis was admitted to our hospital for evaluation of generalized edema. He began antituberculosis treatment with rifampin, isoniazid, ethambutol, and pyrazinamide. He experienced abnormal increments in weight and serum creatinine after 6 weeks. All serological findings, including anti-neutrophil cytoplasmic antibodies (ANCA), were negative. Rifampin was stopped because it might have caused the increase in creatinine. Renal biopsy was consistent with pauci-immune crescentic glomerulonephritis (CrGN). His renal function was improved by high-dose steroid treatment. Rifampin-induced, ANCA-negative pauci-immune CrGN is very rare; most cases of rifampin-induced acute renal failure are due to acute tubulointerstitial nephritis. We present here a case of rifampin-induced CrGN and pulmonary tuberculosis successfully treated with high-dose steroids and antituberculosis medications, excluding rifampin. 이 논문은 梁漱溟의 大人意識을 통하여 오늘날 우리가 추구해야할 ‘바람직한 인간상’ 이 무엇인가를 고찰한 것이다. 양수명의 사상은 陽明哲學을 중심으로 두고 있으면서 서양철학을 수용하는 태도를 견지하고 있다. 따라서 그의 인간관은 바로 이론과 실천이 합치된 지행합일의 인간상이 바탕이 되며, 이는 狂者의 모습으로 묘사할 수 있다. 양수명은 이러한 바람직한 인간상을 염두에 두면서 그들이 함께 실현할 수 있는 공동사회에 대한 일정한 대안을 갖고 있었다. 그것은 ‘新社會’ 라고 규정되며, 새로운 사회의 구현을 위한 실천방안은 향촌건설운동으로 나타난다. 그가 현실을 직시하고 적극적으로 대응하는 태도에서 우리는 바로 광자 곧 열의를 가진 실천인 그리고 참된 즐거움 곧 眞樂을 즐기는 大人의 모습을 찾을 수 있다. 그렇다면 대인의식의 내면적인 주체는 무엇인가? 양수명의 표현을 빌면 그것은 바로 直覺인데, 직각은 양지 또는 양심이라고도 할 수 있다. 보통사람들은 近道의 경지에 머물러 있지만 참된 인간 곧 대인은 완전한 직각을 지닌 경지에 있는 존재이다. 이러한 知行合一과 致良知가 투영된 양수명의 이상적인 인간관은 정보의 시대를 살아가는 삶 속에서 상실된 인간성을 회복하는데 한 지침이 될 것으로 기대한다. The purpose of this paper is to clarify the essence of the model man now and here under reviewing the Great Man(大人) spirit of Liang Shu-Ming(梁漱溟 1893~1988). Liang constructed his unique idea by filtering contemporary western ideas based on traditional Chinese Yang-Ming philosophy(陽明哲學). Basically, Yang-Ming philosophy emphasized on the conformity between knowing and doing(지행합일). Thus, Liang's model man idea came out of it. He coined his idea with the name of the Enthusiasm Man(狂者). He thought his New Society must be full of such model men. Actually, he, also one of his model men, tried to participate in the real world and establish new society by Home Town Reconstruction Movement(鄕村建設運動). Liang's Enthusiasm Man represents a man of doing with passion and a great man enjoying his life with Great Pleasure(眞樂). Such spirit is basically rooted in the insightful eye on the reality and active attitude toward the life itself. By the way, what is the substance of the Great Man spirit? Liang pointed out the Great Man spirit consisted of Intuition(直覺), which came from Yang-Ming's idea, primordial knowledge(良知) or conscience. Liang recognized ordinary people stayed at the stage of ordinary knowledge, while Great Man followed the imperatives of primordial knowledge by insightful intuition. In conclusion, Liang's model man spirit tells us the importance of recovering the natural human spirit at this Information Age. So, the pursuit of primordial knowledge by intuition will be recommended as a way of recovering the human nature and reinforcing desirable competencies our communities require at this moment.
