        Effects of nitroglycerin (NTG), adenosine triphosphate(ATP) and sodium ni troprusside(SNP) on the hemodynamic and blood gas analysis changes during epinephrine(EPI) infusion in air embolized cats were investigated. Under the general anesthesia with pentobarbital and vecuronium, cats were given 2-2.5 ml/㎏ of air via internal jugular vein and were resuscitated with EPI bolus injection. Thirty two cats were divided into 4 groups of 8 each. Each 4 groups, were administered saline(group 1), NTG(group 2), ATP(group 3) and SNP(group 4) with EPI continuous infusion, respectively The air shadow in the heart of cats was visualized by C-arm image intensifier throught the study and all three vasodilators augmented the removal of air from the heart on C-arm image. All three vasodilators therapy groups produced statistically significant decrease in rate pressure product(p<0.05) but only NTG therapy group decreased heart rate(p<005). Central venous pressure were decreased in all three vasodilators therapy groups but only NTG therapy group was statistically significant(P<0.05) Only NTG therapy group increased both arterial(p<0.05) and venous oxgenation. These results indicate that NTG therapy group produced more prominant hemodynamic and blood oxygenation improvement than the other three groups It is concluded that NTG with vasopressor therapy could be recommended in venous air embolism.

        제왕절개술시 흡입마취제에 대한 임상적 비교 연구

        The maternal and neonatal effects of 50% O_2 -50% N_2O alone and 50% O_2-5O% N_2O combined with 0.5% halothane or 1% enflurane were studied in 37 parturients undergoing general anesthesia for cesarean section, All patients received thiopental and succinrlcholine for induction and were intubated and ventilated with a tidal volume of 10ml/kg and a rate of 10 breaths/min. until the delivery. Two of 11(18%) patients given O_2-N_2O alone had recall none in those patients receiving a potent inhalation agent had any recall. Blood loss was similar in the two groups. There were no statistically significant differences between groups in induction-todeliverr and uterine incision·to·delivery intervals, the frequencies of Apgar score$lt;7 at and 5 min., maternal and fetal blood gas tensions, and acid-base balance. It is concluded that analgesic concentrations of halothane and enflurane can be safely added to 50% O_2-5O% N_2O to prevent maternal awareness during general anesthesia for cesarean section while maintaining normal maternal and neonatal conditions.

        Neostigmine Methylsulfate 와 Atropine Sulfate 의 혼합율 및 투여방법이 심박수에 미치는 영향

        It is common to administrate neostigmiae methylsulfate and atropiae aulfate to counteract the effect of nondepolarising neuromuscular blocker. In case of cardiovascular disease, the effect of bradycardia or tachycardia resultiag from the administration of these drugs may be harmful to the patient. The purpose of this study is to investigate the effects of combinatioa and method of administration of neostigmine methylsulfate and atropine sulfate on heart rate. One hundred and two patients in both sexes were devided into three groups(A, B, C), and each group was devided into three subgroups (AI, AII, AIII, BI, BII, BIII, CI, CII, CIII). In group A, neostigmine methylsulfate and atropine aulfate were mixed and administered intravenously within fifteen seconds. In group B, atropine eulfate was administered thirty seconds after the administration of neostigmine methylsulfate. In group C, the two drugs were mixed and administered over a period of five minutes. In subgroup I the ratio of neostigmine methyliulfate to atropine sulfate was 2: 1(0. 04 mg/kg: 0.02 mg/kg) in subgroup 3 the ratio was 3: 2(0.03 mg/kg: 0. 02 mg/kg), and in subgroup Ⅲ the ratio was 1: 1(0. 02 mg/kg: 0. 02 mg/kg). The heart rate was counted just before, 0. 5 minute, 1 miaute, l. 5 minutes, 2 minutes, 3 minutes, 5 minutes, 10 minutes, 15 minutes, 20 minutes, 25 minutes, 30 minutes after the administration of the drugs. The results were as follows: 1) Initial increase in heart rate was significant in lager atropine ratio. 2) The later decrease in heart rate waa less in larger atropine ratio. 3) The maximal increases in heart rate in groap A and B appeared thirty seconds apart, but there was little difference betweea them. 4) The change ia heart rate was the least in group C.

        Myasthenia Gravis is a chronic disease of disputed etiology, possibly an auto-immune reaction to the moter end-plate, characterized by exacerbations and remissions, a rare disease entity in Korea. Myasthenia Gravis has offered many anesthetic problems because it affects respiratory muscles occasionally and bronchial secretion from preoperative anticholinesterase therapy. The chief concern is to ensure adequate respiration both during and after operation. During the year 1976-1982, we have experienced the anesthetic management of 8 patients with myasthenia gravis and thymectomy. From our experiences, we conclude that respiratory care and disuse of relaxants is the key to successful management.

        One-Pot Conversion of Trimethylsilyl Ethers into Urethanes Using Chlorosulfonyl Isocyanate: Application to the Synthesis of a Novel Neuromodulator Carisbamate

        This paper reports a novel synthetic method for the preparation of various urethanes and the application to the synthesis of carisbamate. The reaction of primary (2a, 2e and 2f) or secondary (2g-2i) trimethylsilyl ethers with chlorosulfonyl isocyanate afforded the corresponding urethanes in good yields without affecting the olefin moiety. However, in the case of secondary benzylic trimethylsilyl ether 2j, the corresponding urethane 3j was obtained in low yield. From the difference in reactivity between the primary and secondary benzylic trimethylsilyl ethers, the one-pot synthesis of carisbamate 1 from bis-trimethylsilyl ether 2l was achieved.

        전신마취에 있어서 유지수액에 따른 혈당 및 전해질의 변화

        Eighty Patients in the state of ASA classification I , II were investigated. Blood samples were collected before, just after, 30 minutes after and 60 minutes after induction of anesthesia. Glucose ana electrolyte changes in relation to maintenance fluids in balanced and halothane anesthesia were as fellows: 1) In both anesthetic techniques bloo4 sugar level stowed increasing tendency according to duration of anesthesia. 2) There were no specific changes in serum electrolytes related to type of anethesia. 3) The administration of D/S ana H/D showed a little increase in Na+ and Cl- level compared to the administration of just DsW. 4) The administration of H/D stowed less increase in K+ level than the administration of D_6W or D/S. 5) Just after induction of anesthesia the K+ level increased a little and decreased gradually thereafter.
