        All the process, acquiry, demineralization, cleansing etc, of demineralized allogeneic bone must be conducted under a germ free stable condition without bacteria and/or viruses contamination. But this sterilization process is very difficult. Even though the bone is acquired under a germ free state, we must have confidence of disinfection of all the solutions that come in contact with tissue and blood. To make germ free allogeneic bone materials, many kind of antibacterial agents have been introduced, for example, Merthiolate, formaldehyde, glutaraldehyde, alcohol, formalin, and acetone. Recently ethylene oxide gas sterilization or radiation sterilization is being frequently used. The biological and biochemical response of the graft material differs with the type and concentration of the disinfectant, and various toxic reactions have been reported due to the graft material itself and the substance released by the disinfectant. The author conducted the Millipore filter test, which is a toxic test on fibroblast according to the effect on activity of succinate dehydrogenase, to confirm the cytotoxicity of sterilization with irradiation and ethylene oxide gas to the demineralized allogeneic bone powder. The results were as follows: 1. Around the copper disk, positive control group, 10㎜ diameter discoloration could be seen 2. As same as the negative control group, the silicone disk showed no discoloration. 3. The demineralized allogeneic bone which was sterilized with ethylene oxide gas or irradiation showed no cytotoxicity. 4. From this results, it is suggested that treatment with ethylene oxide gas and irradiation should be effective to sterilized the demineralized allogeneic bone. The results were as followings ; 1. Male:Female ratio was 17:23 and the 32 patients(80%) were twenties. 2. 26 patients(65%) complained not only esthetic problems but also functional problems, only estehtic problem(10, 25%), and only functional problem(4, 10%) were found. 3. Mandibular prognathism was found most frequently(25, 39%) and facial asymmetry(13, 20%), angle hypertrophy(11, 17%) were found also. 4. Sagittal split ramus osteotomy was done most frequently(27, 35%), and LeFort I osteotomy(13, 17%), Angle reduction(12, 16%), Genioplasty (11, 15%) were done also. 5. Cases of nonspecific etiologic factor(22, 56%), cases of inherited tendency(12, 30%), cases of congenital anomaly(3, 7.5%), and cases of trauma(3, 7.5%) were found. 6. As analysis of operation site, cases of only maxillary surgery(2, 5%), cases of only mandibular surgery(23, 57.5%), and cases of simultaneous two jaw surgery (15, 37.5%) were found.

        멸균방법에 따른 탈회동종골의 세포독성 여부에 관한 실험적 연구

        Procurement, cutting, cleansing, freezing, freeze-drying, and demineralization of the allogeneic bone must be made under the germ-free stable condition without bacterial and/or viral contamination. Even thought the bone is procured under the germ free condition, we must have confidence on disinfection of all the solutions that come in contact with tissue during the whole procedure. Lots of antibacterial agents have been introduced for chemical sterilization. Recently ethylene oxide gas sterilization or radiation sterilization is frequently selected as a secendary sterilization procedure. The biological and biochemical response of the graft material differs with the type and concentration of the sterilizing agents, and various toxic reactions have been reported due to the graft material itself and the substance released by the chemicals. The authors conducted the Millipore filter test to observe the toxic effect on L929 fibroblasts according to the effect on activity of succinate dehydrogenase, during the secondary sterilization of the demineralized allogeneic bone powder with irradiation or ethylene oxide gas. The result were as follows: 1. Around the copper disk, positive control group, 10mm diameter discoloration was observed. 2. As same as the negative control group, the disk showed no discoloration. 3. The demineralized allogeneic bone which was sterilized with ethylene oxide gas or irradiation showed no cytotoxicity. 4. From this results, it is suggested that treatment with ethylene oxide gas or irradiation should be effective to sterilize the deminineralized allogeneic bone.

      • 성견에서 악관절원판 절제가 악관절에 미치는 영향에 관한 실험적 연구

        The purpose of this study was to observe changes of TMJ after unilateral diskectomy. 7 dogs of 12kg weight ( control : 1, experimental : 6 ) were used. Left side of experimental group was diskectomized. The experimental animals were sacrified on 2, 12, and 22 weeks. Bilateral excised TMJ were fixed in 10% neutral formalin, decalcified with 8% formic acid, and 4-8um paraffin sections were stained with Hematoxylin-Eosin staining and Masson's Trichrome staining. The specimens were examined with light microscope. The results were as follows 1. Hyperplasia of articular zone and proliferating zone increased but hypertropic zone and erosive zone decreased in the non-surgical site. These features were assumed to mean adaptations of subchondral bone by change from chondral bone to bone. 2. Articular zone and hypertropic zone were increased but proliferating zone was decreased in surgical site of 22 weeks. It was assumed that stimulations in mandibular condyle accelerate shift from proliferating zone to hypertropic zone. Erosive zone was decreased a little. 3. Remodelling of subchondral bone was seen until to 22weeks of surgical site, but barely seen in non-surgical site. 4. There were more active bone remodelling at the inner part of trabecules in both them surgical and non-surgical site.
