        난청 환자에서 이명 주파수와 어음명료도의 연관성

        Background and Objectives Patients with hearing loss and tinnitus experience difficultyin engaging in daily conversations. However, only few studies have examined how tinnitus af-fects individual speech discrimination to comprehend speech. This study aimed to analyze thecorrelation between tinnitus frequency and speech discrimination in patients with hearingloss and tinnitus. Subjects and Method A total of 275 ears with hearing impairment were retrospectively an-alyzed via audiometry and tinnitogram. The ears were divided into three groups depending onthe frequency of their tinnitus. Average pure tone audiometry (PTA), hearing threshold of tin-nitus frequency, speech discrimination test (SDT), degree of discrepancy between tinnitusfrequency and worst PTA frequency were collected and compared among the three groups. Results No significant difference was observed in PTA in the three patient groups. Hearingthreshold of tinnitus frequency was the highest at 78.27 dB in the high-frequency group andthe lowest at 45.14 dB in the low-frequency group. SDT was significantly lower (53.69%) inthe low-frequency group. The correlation between tinnitus loudness and SDT was the stron-gest in the low-frequency group. The degree of discrepancy between tinnitus frequency andworst PTA frequency was also significantly observed in the low-frequency group. Conclusion Patients who are severely affected by tinnitus are found to have significant dis-crepancy between tinnitus frequency and worst PTA frequency, indicating decreased SDT.

        췌장의 낭성종양과 낭성암종;방사선학적 감별진단

        By far the most common cystic lesion of the pancreas is the inflammatory pseudocyst. Among the true cysts the mucinous neoplasm is unique because of its proliferative natura and the potential for carcinomatous degeneration. Hence establishment of the diagnosis prior to surgical exploration is of great importance. The imaging characteristics of ultrasonography and computed tomography are particularly useful in the preoperative differentiation of mucinous cystic neoplasms from pseudocysts of the pancreas. This report descrives the ultrasonographic and computed tomographic findings of eight mucinous neoplasms(three cystadenoma five cystadenocarinoma) and twenty four pseudo-cysts. Mucinous cystadenomas are well-defined large multilocular cystic masses containing irregu-lar thick speta solid components and locally thickened wall. Sonographic and computed tomographic findings of cystadenocarcinomas are similar to those of cystadenoma but cys-tadenocarcinomas frequently show daughter cysts along the primary cystic wall locally inva-sion and distant metastasis. Pseudocysts are unilocular cystic masses which walls are uniform in width contanining many debris within the cysts and are often located at extrapancreatic areas.

        신손상의 초음파검사 및 전산화단층촬영 소견상 비교분석

        To evaluate the accuracy of sonographic diagnosis in renal trauma we compared the sonog-raphic findings with the computed tomographic findings in 14 patients performed sonography and computed tomography simultaneously. The common findings of renal injury on computed tomography and sonography were periren-al hematoma intrarenal hematoma and parenchymal tear. Of these patients 10cases were managed successfully with conservative therapy 2 cases underwent surgery and 2 cases were performed by external drainage. Computed tomography is certainly the most effective method in regard to evaluation of the exact extent of parenchymal and /or vascular damage to the kidney. But sonography provides on excellent inxpensive diagnostic tool for initial diagnostic study of patients with renal trauma especially for the detection of hematoma. Sonography is the best screening method in patients with minimal renal trauma and computed tomography must be performed in the cases with abnormal sonographic findings.

        상부 경추 경막내 지방종의 전산화단층촬영 및 자기공명촬영

        경막내 지방종은 모든 척추종양의 1% 미만을 점하는 드문 질환이다. 이것은 척추 어느곳에서도 발생 할수 있고 주로 경흉추와 흉추 부위에 호발한다. 그러나 상부 경추에서 발생된 것은 매우 드문 것으로 알려져 있다. 저자들은 상부 경추에서 발생한 경막내 지방종 2례의 CT 및 MR 소견을 문헌 고찰과 함께 보고하는바이다. Intradural spinal lipoma occurs in less than 1% of all spinal cord tumors. It has been described at every level of the spinal canal, although its most common location is the cervicothoracic and thoracic region. However, lipoma located in the high cervical region is very unusual. We described two cases, a teenager and an adult, with progressive neurologic dificit from such a lipomatous tumor, which were evaluated by CT scanning and MR imaging.

        상용 DBMS에서의 플래시 SSD와 하드디스크를 이용한 다중 버전 기반 동시성 제어 기법 성능평가

        Multi-version concurrency control scheme is used in many commercial DBMS stores undo data inprivate undo area. When undo data is needed, DBMS reads it from undo area in random-read manner.However, if a data is concurrently updated by many transactions, reading the data spends too much timebecause the number of undo block to be read increase. Also, since many undo data to be written to thedata storage in a short period of time, it can cause deadlock. Unlike magnetic disks, FlashSSD has nomechanically moving parts. Therefore, it can provide fast and uniform random access speed and fastappend-only writing speed in 8 Kbyte. In this paper, we evaluate the performance of multi-versionconcurrency control scheme in commercial DBMS using FlashSSD and harddisk. Thus we show thatundo block read speed using FlashSSD is 15 times faster compared to using harddisk. We also say thatblock write performance is improved indirectly by using FlashSSD. 많은 상용 DBMS 에서 사용하는 다중 버전 기반 동시성 제어 기법은 데이터의 언두 정보를 전용 공간에 저장해 두고 필요 시 랜덤 블록읽기를 통해읽어 오는 구조를 선택하고 있다. 그러나 다수의 트랜잭션이 실시간으로 동일한 데이터를 업데이트 할 경우 랜덤하게 읽어야 할 언두블록이 많아지기 때문에 많은시간이 소모된다. 또한 언두 정보 쓰기 측면에서도 짧은 시간에 많은 데이터를 써야 하기 때문에 병목현상을유발할 수 있다. 플래시 SSD 는 하드디스크와 달리기계적인 파트가 존재하지 않기때문에 저장된 모든 블록에 대해 빠른 랜덤 읽기가 가능하며 8K 씩 순차 채워 쓰기 에서도 빠른 속도를 보장한다. 본 논문에서는 플래시 SSD 를 다중 버전 기반 동시성 기법에 적용했을 경우의 성능 평가를 통해 언두블록 읽기에 대해 하드디스크에 비해 1 5배 이상의 높은 성능을 보임을 증명하고 언두 블록 쓰기에 대해서는 간접적으로 성능향상을 이룰 수 있다는 것을 기술한다.

        Percutaneous drainage of liver abscess is an effective treatment, but the success rate is low for liver abscess that has septa and is multilocular. We experienced one case of successful treatment of multilocular liver abscess using administration of Urokinase into an abscess cavity during the course of percutaneous drainage.

        낙동정맥 내 OECM 적용 가능 지역 발굴을 위한 마을 특성과 서식지 질 비교

        The Jeongmaeks are Korea’s unique forest space recognition system that diverged from the Baekdudaegan. The Jeongmaeks are easily exposed to pressure because it is adjacent to the living area. Among them, Nakdong-Jeongmaek has high biodiversity, but damage is accelerating. According to the Convention on Biological Diversity (CBD) in 2022, the target is to expand the area of terrestrial and marine protected areas to 30% of national territory by 2030. As of September 2023, the area of terrestrial protected areas in South Korea is only 16.97% of the country's territory. This is due in part to the high proportion of private forests in the region, which makes it difficult to establish protected areas. Therefore, there is a need to establish Other Effective Area-based Conservation Measures (OECMs), which pursue complex and effective conservation that considers multiple values, as an alternative to protected areas. This study aims to identify areas suitable for OECM and to provide opinions on the establishment of appropriate management plans for each value using SOM and InVEST Habitat Quality model. This study evaluated the habitat quality of 206 villages located within 1km of the Nakdong-Jeongmaek and compared the characteristics of villages classified by SOM. As a result, the habitat quality was 0.867 for Tourism village (ClusterⅣ), 0.838 for Conservation village (ClusterⅥ), 0.835 for Mixed village (ClusterⅠ), 0.796 for Production (ClusterⅤ), 0.731 for Rural village (ClusterⅢ) and 0.625 for Urban village (ClusterⅡ). When the distribution was identified through statistical analysis, the Kruskal-Wallis test showed that the distributions were not identical, with a p-value of 1.53e-08. Dunn‘s test showed a difference between Tourism, Conservation and Rural, Urban village. However, Mixed village was overestimated due to the lack of villages and the small area included in the study area. Moreover, Conservation village was somewhat under-evaluated in the analysis due to the use of a single weight for protected areas. It is necessary to perform additional reinforcement of the value evaluation of Jeongmaeks by conducting Forest Resource Survey and the National Natural Environment Survey. Therefore, we believe that sufficient validity for the establishment of OECMs in the Nakdong-Jeongmaek can be provided by addressing these limitations and conducting additional research.
