목장을 하기위해 10여년을 준비한 끝에 1.3톤 규모의 목장을 만들었다. 목장시설은 우선 2층 거실에서 목장상황을 한눈에 내려다 볼 수 있게 거실창문을 크게 하였고 안방에 누워서도 볼 수 있게 작은 창문도 만들어 놓았다. 우사는 사료급여조를 포함하여 120평 정도로 적어 규제를 약하게 하였고 우사의 양옆으로 150평씩의 비가림시설(장수비닐)을 하여 운동장으로 이용하고 있다.

소아에서 흔한 신장 질환에서 Lp(a)의 양상과 영향을 미치는 인자에 대한 평가

목적 : Lipoprotein(a)는 LDL 콜레스테롤, 심근 경색의 가족력 다음으로 동맥경화의 독립적인 위험 인자로 보고되고 있다. 그러나 아직까지 Lp(a)의 농도를 조절하는 유전적, 대사적 요인은 잘 알려지지 않았다. 최근 연구에서는 신장 질환에 있어서 신증후군이나 말기신부전 환자의 경우 Lp(a) 농도가 상승하는 것으로 알려지고 있다. 이에 본 연구에서는 신증후군 이외의 신장 질환에서 Lp(a)의 농도를 측정함으로써 Lp(a)의 변화유무를 확인하고 그 의의를 알아보며 Lp(a)의 농도에 영향을 미치는 요인을 알아보고자 한다. 방법 : 1999년 3월부터 2003년 2월까지 중앙대학교 용산병원 소아 신장 클리닉에 내원하거나 입원한 환아들 중 75명(원인 불명의 혈뇨 34명, 고칼슘뇨증을 동반한 혈뇨 10명, IgA 신염 8명, 연쇄상구균 감염 후 급성 사구체 신염이 이환된 환자 8명, Hench-Schonlein 자반증 신염 환자 3명, 요로 감염 환자 7명, 기립성 단백뇨 환자 5명)을 대상으로 하였고 대조군은 신장 질환이나 간질환이 없는 외래환자 20명으로 하였다. 대상환자들은 혈청에서 Lp(a), BUN, 크레아티닌, 알부민, 단백질을, 일반 소변 검사 중 현미경적 검사를 통해 적혈구와 백혈구 수를, 24시간 소변검사를 통해 크레아티닌 청소율과 칼슘을 분석하였다. Lp(a)는 Enzyme-linked immunosorbent assay(Bio-RAD 450, Japen)을 이용하여 측정하였다. 결과 : 소아 신장 질환 가운데 기립성 단백뇨군(P=0.031)이 통계적으로 의미있게 높았으며 기타 질환들에서는 Lp(a)값이 통계적으로 의미가 없었으며 조사된 여러 가지 인자 중 단백뇨(P=0.000, r=0.440), 혈장 단백질(P=0.004), 알부민(P=0.007)이 Lp(a)값과 통계적으로 의미있는 상관 관계를 보였다. 결론 : 소아에서 흔한 신장 질환 환아에서 Lp(a)에 영향을 받는 환아는 연구된 집단군에서 기립성 단백뇨군에서만 확인할 수 있었으며 이는 Lp(a) 농도에 영향을 미치는 것이 신장이라기 보다는 단백질 감소에 따른 또는 알부민 감소에 따른 것으로 생각된다. 이후에 더 많은 환자들을 대상으로 조사하여 통계적 오류를 줄여야 하며 기립성 환아를 추적 관찰하여 Lp(a) 상승에 따른 신장 기능에 대한 영향을 평가해 보아야 한다. Purpose : Lipoprotein(a) is a genetically determined risk factor for atherosclerotic vascular disease and is elevated in patients with renal disease. Especially the patients with nephrotic syndrome exhibit excessively high Lp(a) plasma concentrations. Also the patients with end-stage renal disease have elevated Lp(a) levels. But the mechanism underlying this elevation is unclear. Thus, in this study, by measuring the level of serum Lp(a) in common renal diseases in children, we hoped to see whether there would be a change in Lp(a) in renal diseases other than nephrotic syndrome. Then, we figured out its implications, and looked for the factors that affect the Lp(a) concentrations. Methods : A total of 75 patients(34 patients with hematuria of unknown etiology, 10 with hematuria and hypercalciuria, 8 with IgA nephropathy, 8 with poststreptococcal glomerulone phritis, 3 with $Henoch-Sch\"{o}nlein$ nephritis, 7 with urinary tract infection, and 5 with or- thostatic proteinuria) were studied. The control group included 20 patients without renal and liver disease. Serum Lp(a), total protein, and albumin levels, 24-hour urine protein and calcium excretions, creatinine clearance and the number of RBCs and WBCs in the urinary sediment were evaluated. Data analysis was peformed using the Student t-test and a P-value less than 0.05 was considered to be statistically significant. Results : LP(a) was not correlated with 24-hour urine calcium and creatinine. Lp(a) level had a positive correlation with proteinuria and negative correlation with serum albumin and serum protein. Among the common renal diseases in children, Lp(a) was elevated only in orthostatic proteinuria (P<0.05). Conclusion : Lp(a) is correlated with proteinuria, serum protein, and serum albumin, but not with any kind of specific renal disease. Afterward, Lp(a) needs to be assessed in patients with orthostatic proteinuria and its possible role as a prognostic factor could be confirmed.
