Integra®를 이용한 화상 반흔구축 재건에 있어 Vacuum-Assisted Closure (VAC)의 유용성

Purpose: Negative pressure therapy has been used in various conditions to promote wound healing. It has also been used to secure a skin graft by improving microcirculation and improving tight adhesion between the graft and the recipient bed. To reduce post burn scar contracture and improve aesthetical result, many types of dermal substitutes have been invented and used widely. The goal of this study is to evaluate usefulness of the VAC(Kinetic concepts Inc., San Antonio, TX) in improving the take rate and time to incorporation of Integra® in reconstruction of burn scar contracture. Methods: A retrospective study was performed from October, 2006 to December, 2008. The VAC was utilized for 11 patients. The patient’s ages ranged from 5 to 27 with an average of 19.7 years. The surface area ranged from 24 to 1,600cm2 with an average of 785㎠. The burn scars were excised deep into normal subcutaneous tissue to achieve complete release of the scar, Integra® was sutured in place with skin staple and Steri-strip®. Then slit incisions were made on silicone sheet only with No.11 blade for effective drainage. The VAC was used as a bolster dressing over Integra®. Negative-Pressure ranging from 100 to 125mm Hg was applied to black polyurethane foam sponge trimmed to the appropriate wound size. An occlusive seal over the black polyurethane foam sponge was maintained by a combination of the occlusive dressing, OP-site®. The VAC dressing changes were performed every 3 or 4 days until adequate incorporation was obtained. The neodermis appeared slightly yellow to orange color. When the Integra® deemed clinically incorporated, The VAC was removed and take was estimated with visual inspection. Very thin STSG(0.006-0.008 inches) was performed after silicone sheet removal. Result: The mean time for clinically assessed incorporation of Integra® was 10.00 days(range 9-12). The mean dressing change was 3.5 times until take was obtained. In All patients, Integra® had successful incorporation in tissue without serious complications. Conclusion: Integra® in combination with Vacuum- Assisted Closure(VAC) may be incorporated earlier than conventional dressing method.

To learn the differences between the structure-activity relationship and molecular vibration-activity relationship in the ligand-receptor interaction of the histamine receptor, 47 ligands of the histamine receptor were analyzed by structural similarity and molecular vibrational frequency patterns. The radial tree that was produced by clustering analysis of molecular vibrational frequency patterns shows its potential for the functional classification of histamine receptor ligands.

Intentional carbon monoxide (CO) poisoning has become rapidly increased occurrence, also if the surviving patient is burnt and unconscious, the patient’s burn wound is likely to be severely deep and to need flap surgery. A 44-year-old fe-male patient suffered from a deep contact burn to the left posterior lower leg during intentional CO poisoning. The patient underwent escharotomy and treatment with a latissimus dorsi muscle neuro-vascularized free flap and scalp skin grafts. While harvesting 0.008 inches (300 cm2) of the scalp skin, some 25 cm2 of the skin was accidentally harvested as a thick split-thickness skin graft (SSG) from the right occipital scalp. Interfollicular epidermal and dermal regeneration was achieved at eleven days postoperatively at the thick SSG donor site. The patient’s deep burn wound was reconstructed by two staged surgeries. The patient was able to walk with the help of a supportive shoe one year postoperatively.
System-Wide Expression and Function of Olfactory Receptors in Mammals

Olfactory receptors (ORs) in mammals are generally considered to function as chemosensors in the olfactory organs of animals. They are membrane proteins that traverse the cytoplasmic membrane seven times and work generally by coupling to heterotrimeric G protein. The OR is a G protein‒coupled receptor that binds the guanine nucleotide-binding Gαolf subunit and the Gβγ dimer to recognize a wide spectrum of organic compounds in accordance with its cognate ligand. Mammalian ORs were originally identified from the olfactory epithelium of rat. However, it has been recently reported that the expression of ORs is not limited to the olfactory organ. In recent decades, they have been found to be expressed in diverse organs or tissues and even tumors in mammals. In this review, the expression and expected function of olfactory receptors that exist throughout an organism’s system are discussed.

Flap surgery may be required in cases of severe contact burns. The authors report a patient with an exposed knee joint. A 47-year-old man experienced a wide and deep contact burn on his right knee from a stove while under the influence of alcohol. The patient underwent escharectomy, medial and lateral gastrocnemius muscle flap surgery, pedicled anterolateral thigh flap surgery with a meshed scalp split-thickness skin graft, and secondary antegrade peroneal fasciocutaneous flap surgery with a thigh splitthickness skin graft. The patient was able to walk with a hinged brace at 2 months after surgery.

Reconstruction of the cervical esophagus has been attempted with wide varieties of techniques by the lead and neck surgeon. Recent advances in microvascular surgery have been resulte to be accomplished safe free jejunal transfer as a method of primary reconstruction of the cervical esophagus. Author present a 47 year old male patient suffering from recurrent laryngeal epidermoid carcinoma which is reconstructed with a free jejunal transfer for the cervical esophagus and a pectoralis major myocutaneous flap for the coverage of skin defect after radical excision including skin and cervical esophagus. The postoperative course of patient is uneventful except the wound infection and tolerable early oral diet.
