        The need for reconstruction of large bone, soft tissue defect of mandible has greater emphasis due to development of industry, traumatic accident and increase of tumor. The mandibular reconstruction had greatly progressed through the first and the second World Wars. The Fibular free flap by using microscope was reported in 1970 and many maxillofacial reconstructive surgeons had used. In 1988, Dr. Hidalgo first reported mandibular reconstruction by using fibular free flap. Mandibular reconstruction by using fibular free flap has several advantages. First, it provides up to 25 cm of bone, enough to reconstruct any length of mandible defect. Second, a skin island, based on a septocutaneous blood supply, is available in a size large enough to simultaneously reconstruct internal and external soft tissue defect. Third, The fibular donor site morbidity is low, fourth, it provides a esthetic effect of mandible line. And finally bone viability is good. The Fibular osteocutaneous free flap was performed after COMMANDO operation due to squamous cell cancer in oral cavity (15 cases). Therefore we report out successful operation of the mandible reconstruction by using fibular osteocutaneous free flap.

      • 두경부 암환자에서 방사선 괴사에 의한 연부조직 결손의 근피판술을 이용한 재건

        Radionecrosis of head and neck area following ablative caner surgery seems to be a formidable challenge for both reconstructive surgeon and patients. It always accompanies the secondary infection, and makes progressive necrosis of soft tissue, bone, and muscle. Finally it develops exposure of vital structure like major vessel, brain, and aerodigestive tracts. The purpose of study is to introduce the treatment strategy for severe radionecrosis in head and neck patients, and to evaluate the usefulness of flap in these complicated wounds. 6 patients had the severe radionecrotic wounds related to head and neck cancer underwent wide resection and reconstruction with myocutaneous flap from May, 1994 to April, 1997. Their ages ranged from 26 to 72. They were 4 male and 2 female patients. Reconstruction sites were 1 head and 5 neck. Used flap for reconstruction were 4 pectoralis major myocutaneous flap and 2 scapular myocutaneous flap. 6 patients who had severe radionecrotic wounds in head and neck underwent extensive debridement of necrotic tissue and immediate reconstruction with well vascularized tissue. All patients were treated successfully. Wound was healed uneventfully to cover the vital structure, and infection was subsided completely so that patients preserved their forms and functions of head and neck. This strategy for these complicated cases seems to be very effective for the successful treatment. Appropriate flap selection is mandatory for reconstruction of head and neck.         Doppler Flowmeter를 이용한 허혈지수와 Wagner 임상 분류를 이용한 만성 허혈성 족부괴저의 임상적 분석

        It is currently necessary to manage chronic ischemic foot gangrene because the rate of hospitalization and operations is increasing as a result of diet change and the growth of an aging population. Chronic ischemic foot gangrene is caused by Buerger`s disease, atherosclerosis and particularly, diabetes. In this study, we used the Wagner ischemic index with Doppler flowmeter and Wagner classification on 62 patients over 8 years from 1991 to 1998 as an index for treatment and prognosis. We measured the systolic pressure to determine the ischemic index, and according to Wagner the blood flow was inadequate in cases of diabetic foot gangrene and nondiabetic foot gangrene when it was below 0.45 and 0.35, respectively. According to the observation of clinical symptoms, we divided the cases by Wagner classification. Based on the above data, we performed conservative treatment, skin graft, local flap, superficial sural arterial island flap and amputation. We then followed up the patients and observed the treatment results. We concluded that the Wagner ischemic index with Doppler flowmeter and Wagner classification was useful in determining, selecting, and predicting the rate of survival or death as a result of amputation and graft when a surgical operation is performed.

        한국산 초피의 어독성분에 관한 연구

        한국산 초피나무의 물고기에 대한 독성력를 확인하기 위하여 물질를 용매추출, silica gel column chromatography와 재결정을 통하여 분리 정제하였다. 추출물의 물고기에 대한 독성실험 결과, methanol 추출물에서 가장 강한 독성이 나타났으며, 그 다음으로는 물추출물, 초피의 건조분말 그리고 chloroform추출물 순서로 독성을 보였다. 이 추출물의 구조를 기기분석을 통해 해석한 결과 L-asarinin 임을 확인하였다. To study an ichthyotoxicity of Zanthoxylum piperitum DC (Chopi) against fish (Misgurnus anguillicaudatus), the bark of Chopi was extracted with water, methanol and chloroform. The ichthyotoxicities of each extract were found in order of MeOH extract, H₂O extract, dried-powder of Chopi and CHCI₃ extract. The active compound was isolated from the MeOH extract by silica gel column chromatography, HPLC and recrystalization, successively, and it was identified to be L-asarinin by the comparison of UV, GC-Mass and lC-Mass data.
