Immunofluorescence is the mainstay among immunohistochemical techniques used in identifying the immunopathogenesis of renal disease. Enzyme-conjugated antibodies helpful in electron microscopy have note yet found wide use in light microscopy. Immunological renal iniury may follw, first , if antibodies react with the renal basement membranes, either glomerular or tubular (GBM, TBM), and second , if antibodies combine with nonglomerular antigens in the circulation to form nephritogenic immune complexes that lodge in the glomerular filter or renal interstitium. With immunofluorescence, one can then identify these antibody deposite which initiate renal injury. Since anti-GBM or anti-TBM, antibodies react all along the GBM or TBM, they appear as smooth linear deposits of immunoglobulin(Ig). Immune complexes depositing at random in the renal vasculature, predominant1 y the glomeruli , produce a granular, irregular accumulation of Ig. Either type of antibody reaction in the kidney can lead to activation of mediators of immunological injury, such as complement, which can also be characterized by immunofluorescence. Satisfactory immunofluorescence study requires adequate tissue sections, optimal fluorescent-antibody reagents, adequate fluorescence microscopic equipment, and a good deal of experience in interpreting the findings. Portions of this part are directed toward each of these pointes, and a detailed description of tissue processing, fixation, and staining is included.

Reticulin fibres are fine branching fibers which are difficult to see in H and E Preparations and need special stains to be visualized. The fibres are normally seen to best advantage in lymphoid tissue-e and liver. The demonstration of reticulin fibres can be of assistance in diagnosis in certain instances; myel-osclersis in bone biopsies, where there is an excess of reticulin fibres; also in differentiating poorly differentiated carcinoma showing a poorl-y marked reticulin pattern compared the reticu-losarcomawhere there is likely to be a profuse reticulin network. To show these fibres one may carry out a trichr-ome-type technique where they stain blue or green; the PAS technique where they positive or, more popularly, one may take advantage of their argyrophilia. There are many silver techni-ques for reticulin in common use, the more impor-tant of which will be described. Whilst I prefer to use the Gordon and Sweets’technique, all the methods described in this chapter are capable of yielding good results given the necessary experti-se.

Objectives: Genetic polymorphisms of Monoamine oxidase A (MAO-A) have been investigated in affective disorders. The results were controversial and even contradictory. In order to evaluate the genetic polymorphisms of MAO-A in Korean major depressive disorder, I investigated the genotype distribution and allele frequencies of MAO-A in the patients group, and compared them with those of healthy control group. Methods: Using polymerase chain reaction and electrophoresis, MAO-A genotypes were identified in 42 major depressive disorders and 100 healthy controls. Results: When genotypes of MAO-A were classified as g/g, g/t, t/t, there were no statistical differences in the genotype distribution and allele frequencies between two groups. Conclusion: These results suggest that genetic polymorphisms of MAO-A seem to be unrelated to pathogenesis of major depressive disorder.
항우울제와 몽유병:Mirtazapine에 의한 증례 1례

몽유병은 수면수반증으로 분류되는데, 항우울제와 기타 향정신약물에 의해 이차적으로 유발될 수 있다는 보고들이 있었다. 그러나, NaSSA(noradrenergic and specific serotonergic antidepressant)계열인 mirtazapine에 의해 유발된 몽유병에 대한 보고가 없었다. 저자들은 우울증 환자에서 mirtazapine 사용에 의한 몽유병 1예를 경험하였기에 관련 문헌들을 고찰하고 보고하는 바이다. Somnambulism is classified as a parasomnia and has been reported with the use of antidepressants and other psychotropics. However, to our knowledge, there have been no reports of somnambulism associated with the use of mirtazapine (a noradrenergic and specific serotonergic antidepressant;NaSSA). We experienced a case of the mirtazapine (30 mg/day)- induced somnambulism in a 65 year-old woman with major depressive disorder who has never been diagnosed as panic disorder, somnambulism, other parasomnias, neurological disorders (including epilepsy), and other medical diseases. The sleepwalking symptoms disappeared after the antidepressant was replaced with paroxetine (20 mg/day).

Two kinds of silver impregnation methods are most widely used in routine histopathological laboratory. that is frozen section which requires careful handling, and processing and produces inconstant result and uncomfortable procedure, in addition the section is easily or frequently detached from the slide due to high alkalinity of silver particles. This may be overcome by carrying out steps of the procedure . by floating the paraffin section on the solutions. The procedure shows simple and no detachment from the slide, also it can get a clear background by deparaffinization.

The principle of the technique is that the component mucins are hydrolyzed by sulfuric acid, hydrochloric acid and glacial acetic acid. Thus the sputum viscosity is lowered, and enables concentration by centrifugation. It is a reliable method, and can be recommended. normally the sputum is treated with the HCI, CH3COOH and H2S04 overnight and 3 hours by shaking. We were able to observe excel1ent morphology of cells examined with 6% hydrochloric acid among three kinds of acid.

Historically, the first-class infectious disease led to SARS in 2002, swine flu in 2009, MERS in 2015, and COVID-19 in 2019. This study aims to design and arrange an essential unit of a modular screening clinic for continuous use in the event of a future infectious disease outbreak, particularly focusing on laying the foundation to quickly respond to disasters. The research process involved specifying essential units based on previous studies that derived the essential and recommended facilities for screening clinics. The required unit module of the modular screening clinic was set as A, B, and C, and the combination was reviewed. As a result of the study, A+B(3)+C type was found to be the most appropriate in terms of function and movement. The results also indicated that a module unit with a size of 1800 × 3,000 mm was the most suitable for module assembly and transportation. A plan, elevation, and cross-sectional view were then designed by connecting the derived units into five combinations. The essential elements of the space consisted of a reception room, a treatment room, a medical staff space, a locker room, and a waste disposal room. The third, serial, parallel, and mixed layout scenarios were prepared in anticipation of site utilization by a public health center.
