Background:The purpose of this study was to determine the tibial fixation strength provided by different intraosseous soft tissue graft lengths within the tibial tunnel. Methods: Porcine tibial bones and digital flexor tendons were used for testing. Bone mineral densities of proximal tibial medial condyles were measured, and two-strand tendon bundles of 8 mm diameter were used. An intraosseous graft length of 2 cm was used in group 1 (n = 10), and a graft length of 4 cm was used in group 2 (n = 10). Tunnels were 4 cm in length and 8 mm in diameter. Tibial fixation was performed using a suture tied around a screw post with a washer and an additionally inserted 7 × 20 mm bioabsorbable screw. After applying preconditioning loading of 10 cycles, 1,000 cycles between 70–220 N were applied at a frequency of 1 Hz. Graft slippage and total graft movement were recorded. Ultimate tensile strength was measured by pull-out testing at an Instron crosshead speed of 1,000 mm/min. Results: No significant intergroup difference was found for total graft movement after cyclic loading (slippage in group 1, 1.2 mm and group 2, 1.2 mm, respectively, p = 0.917; and total graft movement in group 1, 3.3 mm and group 2, 2.7 mm, respectively, p = 0.199). However, mean ultimate tensile strength in group 2 was significantly higher than that in group 1 (group 1, 649.9 N; group 2, 938 N; p = 0.008). Conclusions: In a porcine model, ultimate tensile strength was greater for a 4 cm long intraosseous flexor tendon in the tibial tunnel. However, no intergroup difference in graft slippage or total graft movement was observed. The results show that a 2 cm intraosseous graft length in the tibial tunnel is safe and has sufficient strength (> 450 N) for adequate rehabilitation after anterior cruciate ligament reconstruction.

소성 이론은 아직 모든 금속성형 문제에 정확한 해를 줄 정도로 충분히 진보되어 있는 상태는 아니지만, 그럼에도 불구하고 실제 공정의 단순화, 가공물의 이상화를 통해 문제를 해석함으로써 가공하중, 금속유동등에 관한 인자들의 영향을 예측하고, 또 원하는 결과를 얻기 위한 적절한 가공조건을 선택할 수 있도록 근사해를 제공하고 있어 실제 공정이 성립하기 전에 행하는 실험적 시도의 량을 충분히 줄일 수 있다. 따라서 여기서는 우선 금속성형에 관련된 수학적 소성 이 론의 기초이론과 소성변형의 해석방법을 소개 하고자 한다.

A new kinematically admissible velocity field for a generalized three-dimensional flow is introduced, in which the flow is bounded by an analytic die-profile function. Then, by applying the upper-bound method th the velocity field, the flow patterns as the upper-bound method are obtained. Extrusion of elliptic sections from round billets is chosen as a computational example. Computation and experiments are carried out for work-hardening material such as aluminum alloy 2024. In order to visualize the plastic flow, the grid marking technique is employed. The theoretical predictions both in extrusion load and deformed pattern are in good agreement with the experimental data. 본 논문에서는 터어빈과 콤프레셔 블레이드의 정밀예비현상 및 캠 부품의 제 조에 사용되는 원형소재로부터의 타원형재의 압출에 대한 일반적인 해석방법을 제안할 것이다. 해석결과는 실제 가공경화재인 알루미늄합금(Al2024)의 압출실험을 수행하 여 타당성을 검토하도록 할 것이며 기존의 해석 및 실험결과와도 비교 논의하도록 하 겠다.
형상충전기법과 세분화된 유동장 재생성기법을 이용한 자유표면을 가진 비압축성 점성유동의 수치적 모사

In this paper, two new techniques, the pattern filling and the refined flow field regeneration, based on the finite element method and Eulerian mesh advancement approach have been developed to analyze incompressible viscous flow with free surfaces. The gorerning equation for flow analysis is Navier-Stokes equation including inertia and gravity effects. The penalty and Newton-Raphson methods are used effectively for finite element formulation. The flow front surface and the volume inflow rate are calculated using the pattern filling technique to select an adequate pattern among five filling patterns at each quadrilateral control volume. By the refined flow field regeneration technique, the new flow field which renders better prediction in flow surface shape is generated and the velocity field at the flow front part is calculated more exactly. Using the new thchniques to be developed, the dam-breaking problem has been analyzed to predict flow phenomenon of fluid and the predicted front positions versus time have been compared with the reported experimental result.

We analyzed Twenty eight cases of femoral head fracture associated with posterior hip dislocation managed in Kyungpook National University Hospital between 1984 march and 1993 June. Classification was made by Pipkin and we added impaction fracture as another classification by post reduction CT finding. The results were as follows 1. Most common type of fracture was impaction of femoral head by CT, Which is not included by pipkin classification. 2. Excellent, or Good result according to Epstein criteria, 7 of 8 cases in Pipkin type [, 1 of 3 cases in type II, 0 of 4 cases in type 0, 1 of 4 cases in type N, 6 of 9 cases in impaction fracture. 3. There were no significant difference between the results of operative treatment and conservative treatment. 4. Post reduction CT was useful to determine the method of the further treatment. 5. Complications were avascular necrosis in 5 cases, posttrumatic arthritis in 3 cases, sciatic nerve paresis in 2 cases, heterotropic ossification in 1 case.
슬관절의 관절경적 시술 후 발생한 폐 색전증 - 2례 보고 -

심부 정맥 혈전증(deep vein thrombosis)과 폐색전증(pulmonary embolism)은 정형외과적 수술 후 발생할 수 있는 심각하고 치명적인 합병증이다. 대부분의 시술은 인공관절 치환술등 절개가 큰 시술 후에 발생한다. 반면 슬관절의 관절경적 시술 같은 최소 침습적 수술의 경우, 그 위험도 및 예방적 조치에 대해 명확하게 밝혀지지 않고 있다. 슬관절의 관절경적 시술 후 발생한 폐색전증의 국외보고는 다소 있으나 국내보고는 아직 없어 2례에 대하여 보고하고자 한다. Deep vein thrombosis and pulmonary embolism are serious fatal complication after orthopedic surgery. Most of them occur after large operation such as total knee/hip arthroplasty. Some reports about deep vein thrombosis after simple arthroscopic procedure were on the Western literatures, but there was no report on the Korean literature. So we report two cases of pulmonary embolism after knee arthroscopic procedures.
