Ventricular fibrillation and cardiac arrest rarely occur after local application of epinephrine. Local and superficial application of epinephrine is a common hemostatic method during bronchoscopy, especially after biopsies. Although high plasma levels following endobronchial application of epinephrine have been observed in previous animal studies, there is no report of ventricular fibrillation after a usual dose of endobronchial application of epinephrine during bronchoscopy. We present a case of endobronchial epinephrine-induced ventricular fibrillation and cardiac arrest in a 31-year-old man with no previous history of cardiac disease.
폐암 말기 환자에서 발생한 Phialemonium dimorphosporum과 Myrothecium Species에 의한 심재성 피부 진균증 1예

Phialemonium and Myrothecium species are rare agents of human deep mycosis infection and those have been increasingly documented as emerging agents of opportunistic infection in immunocompromised patients. We discribed a 69-year-old Korean male, previously diagnosed as lung cancer, presented with 2×2 cm sized normal skin to erythematous nodules on the both lower legs for 10 years. Biopsy specimen showed brownish septated hyphae in the dermis and subcutaneous tissue from the left lower leg and colorless spores from the right leg (H&E). Each identifications were confirmed by polymerase chain reaction of ITS2 using universal fungal primers as Phialemonium dimorphosporum on the left lower leg and Myrothecium species on the right lower leg. The patient was treated with surgical excision and fluconazole.
Two lepidopteran pests and damage on the cones of Abies koreana (Pinaceae) in Jeju Island, Korea

In the present study, we report two lepidopteran pests and their damage on the cones of Abies koreana E. H. Wilson from Mt. Halla, Jeju, South Korea: Cydia kamijoi Oku and Dioryctria abietella (Denis & Schiffermüller). The former is new to Korea, and the latter is well known as an insect pest on cones of various coniferous trees. Larvae of these species bore into the immature cones of the host tree. Damaged cones can be easily distinguished by reddish brown frass piled around the holes that were made by the moths, and the cones that are severely damaged become crooked and eventually are folded in half. The average damage rate on the cones was 75.3 ± 2.34% from the survey sites in 2014, but the insect damage could not be found again from the sites as the host did not bear any cones in 2015. Descriptions and images of C. kamijoi and D. abietella are provided along with a list of host species and distribution.

Pseudo-Meigs syndrome is accompanied with pleural effusion, ascites and a benign or malignant tumor of ovary, tubes, uterus, round ligament or colon. We reported a case of hypoxia in an ovarian cancer patient with moderate ascites after head-down tilt position for central venous catheterization under general anesthesia. Massive pleural effusion was detected on portable chest X-ray, which was not observed in a preoperative radiologic test. The patient had no respiratory symptoms and breath sound was normal in both lungs prior to surgery. The pleural effusion was resolved by a chest tube insertion.
