Paget’s disease is a well-known disorder of unclear etiology that results in deformity and enlargement of single or multiple bones due to abnormal osteoclastic-osteoblastic remodeling of bone. Osteoclastoma, also known as giant cell tumor, is a rare complication of Paget’s disease that usually occurs in the skull or facial bones. This is a report of a 35-year-old woman with recurrent osteoclastoma of the sino-nasal tract, a complication of Paget’s disease, for which she underwent 4operations. Background and Objectives: Recurrent respiratory papillomatosis (RRP) is difficult to treat because of its tendency to recur and spread throughout the aerodigestive tract. We aimed to estimate the effect of intralesional injections of cidofovir in patients with RRP. Materials and Method: Within the period from January 2003 to July 2007, 13 patients aged 2 to 61 years were treated with intralesional injections of cidofovir combined with surgical excision of RRP. Cidofovir was injected intralesionaly at a concentration of 5 mg/cc after complete removal of the papilloma with $CO_2$ laser or microdebrider. We evaluated the effect of intralesional cidofovir therapy by comparing pre-treatment mean interval of recurrence with post-treatment interval of recurrence. Results: Of 13 patients, two patients showed complete response during follow up period and four patients showed partial response. Seven patients did not respond to cidofovir at all. Mean pre-treatment mean interval of recurrence was 9 months and mean post-treatment interval of recurrence was 13.1 months (p=0.039). There was a statistical significance between the injected dose of cidofovir and post-treatment interval of recurrence (p=0.009). There were no local or systemic side effects caused by cidofovir. Conculsion : Intralesional injection of cidofovir seems to have a potential of a safe and effective adjuvant therapy of RRP. There was a positive correlation between the injected dose of cidofovir and patient clinical outcomes so that administration of higher doses and more frequency of injections should be needed to reduce recurrence. Further study regarding injection therapy regimen for RRP is required.
Linear viscoelasticity of covalent adaptable network (CAN) polymers comprising β-amino esters

We present an experimental investigation of the linear viscoelasticity for a series of covalent adaptable network (CAN) polymers of β-amino esters possessing tertiary amines at the β position of ester linkages. CAN polymers were synthesized by aza- Michael addition from di-acrylate monomers with or without β-hydroxyl groups using a tri-amine cross-linker. The prepared CAN polymers exhibited dissociative-type bond exchanges by aza-Michael reaction. The additional inclusion of β-hydroxyl group endowed them with associative-type bond exchanges by transesterification. Time–temperature superposition (TTS) was used to construct pseudo-master curves of storage, loss, and stress-relaxation moduli over wide timescales. The results showed that without transesterification, the slow kinetics of aza-Michael reaction considerably retarded terminal relaxation. The introduction of transesterification accelerated terminal relaxation rates but did not modify the overall broadness of terminal relaxation modes. Horizontal shift factors displayed Williams–Landel–Ferry (WLF) dependence below 120 °C but Arrhenius dependence above 120 °C. The former was due to slow segmental dynamics, whereas the latter reflected the characteristic of exchange reaction kinetics. In addition, we also compared and discussed two definitions of topology freezing transition temperature, an important concept for associative-type CANs. Conclusively, the topology freezing transition temperature obtained from the transition of shift factors (WLF → Arrhenius) was a more practical definition for the potential processing and applications of CAN polymers.

Background and Objectives:Antrochoanal polyp (ACP) is a benign maxillary sinus polyp that originates from the mucosa of the maxillary sinus, passes through the sinus ostium, and extends into the choana. Surgical methods employed in the treatment of antrochoanal polyp are simple avulsion, Caldwell-Luc operation, osteoplastic maxillary sinus operation, and endonasal endoscopic removal. In this study, we evaluated the clinical presentation of ACP and compared its surgical outcomes. Materials and Methods:We retrospectively reviewed 111 patients who were treated for ACP between January 1995 and May 2008, and analyzed clinical features, radiologic findings, and surgical results. Results:The study group consisted of 111 patients (65 males, 46 females) with a mean age of 20.0 years (range, 4 to 70). Sixty-one cases were accompanied by sinusitis (20 with unilateral sinusitis, 41 with bilateral sinusitis). Endoscopic sinus surgery (ESS) was used to treat 95 cases, and the external approaches of Caldwell-Luc operation or osteoplastic maxillary sinus operation were applied in 12 and four cases, respectively. Among those who received ESS, ACP recurred in 16 cases, whereas none of the patients who received the external approach showed recurrence. Conclusion:Endoscopic sinus surgery is a good alternative method for the treatment of ACP. If the antral part of ACP cannot be removed completely, an external approach such as Caldwell-Luc operation or osteoplastic maxillary sinus operation for pediatric patients may be considered to prevent recurrence.
Nonlinear material functions under medium amplitude oscillatory shear (MAOS) flow

Dynamic oscillatory shear flow has been widely used to investigate viscoelastic material functions. In particular, small amplitude oscillatory shear (SAOS) tests have become the canonical method for characterizing the linear viscoelastic properties of complex fluids based on strong theoretical background and plenty of experimental results. Recently, there has been increasing interest in the use of large amplitude oscillatory shear (LAOS) tests for the characterization of complex fluids. However, it is difficult to define material functions in LAOS regime due to an infinite number of higher harmonic contributions. For this reason, many recent studies have focused on intrinsic nonlinearities obtained in medium amplitude oscillatory shear (MAOS) regime, which is a subdivision of the full LAOS regime. In this study, we reviewed recent experimental and theoretical results of nonlinear material functions in the MAOS regime, which contain four MAOS moduli (two first-harmonic moduli and two third-harmonic moduli) from Fourier and power series of shear stress, and a nonlinear material function Q0 and its elastic and viscous parts from Fourier-transform rheology (FT rheology). Furthermore, to identify linear-to-nonlinear transitions in stress response of model polystyrene (PS) solutions, we presented Pipkin diagrams in frequency ranges from the rubbery plateau region to the terminal region.

This paper present a new approach for optimal dispatch for energy and frequency control reserve considering contingency in the competitive electricity market. Ancillary service plays a key role in maintaining operation security of the power systems in a competitive electricity in power system. This paper proposes a methodology for optimal dispatch for frequency control reserve that satisfy all constraints such as equality and in-equality constraints for all possible contingencies of power system. The methodology are accomplished with the equality and in-equality constraints based on AC load flow. To verify the effectiveness of the proposed method, the numerical study has been performed for 5-bus sample system.
