        Glutaric Aciduria Type 1 1례

        저자들은 경련 발작후 퇴행성 발달 지연과 무도증 양상의 운동을 보이는 13개월 여아를 두부 자기공명 검사와 소변 유기산 검사를 이용하여 국내 최초로 glutaric aciduria type 1 1례로 진단하였기에 보고하는 바이다. Glutaric aciduria(type 1) is characterized clinically by progressive dystonia and dyskinesia in childhood, pathologically by degeneration of caudate and putamen, biochemically by tissue deficiency of glutaryl-CoA dehydrogenase(GCDH), and is transmitted as an autosomal recessive traits. Mutations of the GCDH gene on chromosome 19 have been implicated in the causation of glutaric aciduria(type 1). Macrocephaly in infancy and crossing of percentiles for head circumference are real clues to early diagnosis. Acute neuroregression of dystonia following an initial phase of normal or almost normal development is a common mode of presentation, at times preceded by seizures. We experienced a case of glutaric aciduria(type 1) in a 13-month old girl. She was admitted due to development delay and choreoasthetoid movememt that developed after generalized tonic-clonic type seizures. She was diagnosed as having glutaric aciduria(type 1) based on brain MRI and urine organic acid analysis finding.

      • 연부조직 결손을 동반한 감염성 경골 불유합 및 골결손의 치료(유리피판술과 동시에 시행한 Ilizarov기구를 이용한 골연장술의 유용성)

        Purpose: The purpose of this study was to evaluate the effectiveness of internal transport using Ilizarov apparatus with free flap surgery for infected tibial nonunion. Materials and Methods: We reviewed 8 patients of infected tibial nonunion treated with internal transport using Ilizarov apparatus and free flap surgery. Seven of eight patients were available for at least 1 year follow-up. All patients were male. The mean age at the time of the surgery was All fractures were Gustilo's type III B open fracture. The mean length of the bone defect was 8.5 cm. All used flaps for covering the soft tissue defect were free rectus abdominis muscle flap. We evaluated bone and functional results with use of the Paley and Catagni's classification. And we classified the complication with use of the Paley's classification. Results: Acceptable length and solid union of bone was achieved in all cases. The mean size of the bone length was 7.2 cm. The mean healing index was 69.5 days/cm. All but one case needed bone graft at docking site. All flaps were survived. There was no recurrence of infection. According to Paley and Catagni's classification, all cases showed excellent or good results. Complications were pin tract infection in 3 cases, persistent pain in 2 cases and limitation of joint motion in 2 cases. Conclusion: Simultaneous free-tissue transfer and Ilizarov distraction osteogenesis was thought to be an attractive treatment modality for infected nonunion of the tibia.

        Stearic acid로 표면 코팅된 탄산칼슘의 폴리프로필렌에서의 분산

        Calcium carbonate ($CaCO_3$) with 15~40 nm average diameter was compounded with polypropylene (PP) in a melted state to form composite materials. To enhance the dispersibility in PP, the surface of the $CaCO_3$ was coated with hydrophobic stearic acid (SA). The SA-coated $CaCO_3$ (SA-$CaCO_3$) showed hydrophobic nature, which resulted in rapid precipitation in water. When melt-compounded with PP, SA-$CaCO_3$ was uniformly distributed throughout the PP matrix, while the uncoated $CaCO_3$ formed aggregated structures in the PP. At up to 5 wt% of SA-$CaCO_3$ content, the composites showed well-dispersed structure, although some level of aggregation was observed in the composites with 10 wt% of SA-$CaCO_3$. The dispersibility improved with a repeated process of compounding. The melting and crystallization temperatures of PP increased with incorporation of $CaCO_3$. The enthalpy of the transitions also increased and the full-width of half maximum of the crystallization peak decreased. However, SA-coating did not produce additional effects on the crystallization behavior of PP. These results showed that the $CaCO_3$ acted as crystal nuclei.

        The life expectancy of HIV-infected subjects has improved dramatically since the introduction of highly-active antiretroviral therapy (HAART). Considering that patients with HIV infection are living longer, treatment plans should include those for the accompanying co-morbidities such as cardiovascular diseases, metabolic syndrome, malignancy, etc. This review discusses the impact of HAART on the epidemiology of survival and co-morbidities (malignancy and cardiovascular diseases) among HIV-infected subjects.

        다이나믹 테이핑과 키네시오 테이핑 적용에 따른 발바닥 굽힘근의 지구력과 피로도에 미치는 효과 비교

        Background: The purpose of this study is to investigate the effect of plantar flexor on muscle fatigue and endurance when two different sport tapes are applied. Design: Cross-sectional design. Methods: Fifty-one healthy adults were randomly assigned to dynamic taping group (n=17), kinesio taping group (n=17), and control group (n=17). The dynamic taping group used the dynamic tape. The kinesio tape group used the kinesio tape, and the control group doesn't used tape. Every group performed heel rise test and sEMG for every during pre-test and post-test. To evaluate plantar flexor endurance, the number of times was measured during the heel rise test. Results: The number of heel rises increased significantly in the post-test in dynamic taping group compared to the pre-test, and In comparison between groups, the number of heel rises significantly increased in dynamic taping group compared to kinesio taping group and control group. PEF value was significantly increased in the post-test compared to the pre-test in dynamic taping group, and there was a significant difference between the three groups according to the taping application. Conclusion: The results of this study confirmed that dynamic taping was effective in musclular fatigue and endurance on plantar flexor in healthy adults. Based on these results, it is suggested that the application of dynamic tape can be suggested as one of the intervention methods for muscle endurance and muscle fatigue.

      • 유리 조직 이식술을 이용한 당뇨병성 족부 궤양의 치료

        Diabetic foot ulcer is a serious complication which result from long-standing diabetes. Especially, severe infected diabetic foot ulcer results in unwanted lower extremity amputation. The diabetic patient is considered the relative contraindication for microsurgery because of the severe peripheral vascular disease. Recently, microvascular free tissue transfer technique applied to diabetic foot ulcer. It is well known that free tissue transfer provides immediate soft tissue coverage and control of infection. So it is possible that preservation of the lower extremity through free tissue transfer. A retrospective study of diabetic patients who had infected foot ulcer from 1999 to 2000 with foot defects reconstructed with free tissue transfer were reviewed. Thirteen patients were studied with mean follow-up of 12.7 months. There were two deaths during follow-up period. There were two failures after free flap surgery. All eleven survived patients were ambulatory. There was no recurrence of ulcer. No patient need amputation above the ankle joint. We have found that free tissue transfer for infected diabetic foot ulcer is very effective surgical technique. Careful patient selection and regular follow-up is important.

      • 역행성 요측 전완부 피판을 이용한 수지 절단부 재건

        Purpose: We analyzed the result of the radial forearm island flap for the reconstruction of amputated fingers. Materials and Methods: From March 2001 to February 2004, we assessed two patients who could not be able to receive replantation and six patients who had necrosis of the fingers after replantation. There were six men and two women. The mean age was 42 years. The average duration of follow-up was 26months. Results: All flaps were survived. Two patients needed bone graft. Two patients needed curettage due to osteomyelitis. All patients needed fat reduction procedure. No patients needed amputation of reconstructed digit due to complication. Conclusion: Radial forearm island flap seems useful operation procedure for severely amputated digit or necrotized digit after replantation.

        요소분해효소에 의한 탄산칼슘 침전을 통한 지반 개량 평가

        This study presents the experimental results of $CaCO_3$ formation in sand by the Enzyme Induced Carbonate Precipitation (EICP) method. Concentration of $CaCO_3$ with elapsed reaction time is calibrated by standardized procedure by measuring $CO_2$ pressure, and it increases with time towards asymptotic value. Jumunjin sand saturated with EICP solution shows that both shear wave velocity and electrical conductivity sharply increase as the reaction starts to approach to the constant values after 50 hours of reaction time. Urease concentration of 0.5 g/L exhibits 224% higher final shear wave velocity than that of 0.1 g/L. The nucleation models hint that carbonate tends to precipitate not only at grain contacts but also at grain surfaces. Regardless of urease concentration, electrical conductivity and shear wave velocity follow the unique path. The scanning electron microscopic images and X-ray computed tomographic images validate the spatial configuration of produced $CaCO_3$ in soils. 본 연구에서는 사질토에서의 EICP에 의한 탄산칼슘 침전량을 정량적으로 평가하였다. 생성된 탄산칼슘은 염산과의 반응에 수반되는 이산화탄소 기체 압력 증분을 통해 간접적으로 측정하였으며, 이는 반응이 진행됨에 따라 특정 값으로 수렴하는 경향을 보였다. EICP 용액으로 포화된 주문진표준사의 전단파 속도 및 전기전도도값은 측정된 탄산칼슘량의 수렴시간보다 선행하여 일정한 값에 도달함을 확인하였다. 결정화 모델은 탄산칼슘이 흙 입자간 접촉점과 입자표면에서 생성됨을 나타내며, 이를 통해 최종 전단파 속도 및 최종 전기전도도에 도달하는 시간과 탄산칼슘 생성량의 수렴시간 간의 불일치가 설명 가능함을 보였다. 또한, 용액 농도 0.5g/L를 이용한 최종 전단파 속도는 0.1g/L의 것보다 224% 높은 효율을 나타내었다. 더불어 효소의 농도와 무관하게 전기전도도와 전단파 속도의 상관관계가 있음을 확인하였으며 주사전자현미경과 X-ray CT 이미지 분석을 통해 생성된 탄산칼슘의 공간적 분포를 확인하였다.
