      • 임상전문간호사의 역할 인식에 관한 연구 : 임상전문간호사, 간호사, 의사를 중심으로 Focusing on the Clinical Nurse Specialist, Nurses and Doctors

        This study is a descriptive investigation research to provide information needed for establishing roles of Clinical Nurse Specialist by researching the recognition for their roles among doctors, nurses and Clinical Nurse Specialists. The research materials were collected from April 3, 2002 to September 23, 2002 and with cooperations of 17 Clinical Nurse Specialists, 41 nurses and 16 doctors, therefore total 74 medical practitioners who work for S hospital in Seoul. The collected data were analyzed by PC-SPSS Program(for real numbers, percentage, average and standard deviation), Kruskal-Wallis Test, Oneway ANOVA, Fridman Test and Twoway ANOVA. The final results are as follows in brief. 1) As distribution of all the role divisions was shown over 3.0 average of importance level and under 3.0 average of performance level, the recognition for importance level about Clinical Nurse Specialist' role in each division was high, but recognition for performance level was low among Clinical Nurse Specialists, nurses and doctors. 2) In the recognition for importance level about Clinical Nurse Specialist's role, nurse group's recognition for importance level about direct nursing was high, showing statistically significant difference(p<.000) 3) In the recognition for performance level about Clinical Nurse Specialist' roles, there was no role division showing significant difference statistically between groups. 4) The recognition for performance level among Clinical Nurse Specialist was shown in the order of counseling(2.80), education(2.74), administration/promotion of change(2.52), direct nursing(2.26) and research(2.20)(p<.000). The recognition for importance level was shown in the order of research(4.12), administration/promotion of change(4.08), counseling(4.05), education(3.94) and direct nursing(3.51), therefore this indicated significant difference statistically between groups(p<.000) 5) The recognition for performance level among nurses was shown in the order of counseling(2.65), education(2.53), direct nursing(2.50), administration/promotion of change(2.44) and research(2.92)(p<.000). The recognition for importance level was shown in the order of direct nursing(4.09), counseling(4.01), administration/promotion of change(4.00), research(3.89) and education(3.87), therefore this Indicated significant difference statistically between groups(p<.009). 6) The recognition for performance level among doctors was shown in the cider of counseling(2.78), direct nursing(2.60), administration/promotion of change(2.52), education(2.50) and research(2.36)(p<.150). The recognition for importance level was shown in the order of administration/promotion of change(3.74), counseling(3.66), education(3.65), research(3.63) and direct nursing(3.38), therefore it did not indicate any significant difference statistically between groups(p<.019). According to the results above, the performance level of Clinical Nurse Specialist's role was significantly lower than the importance level. Therefore, developing a plan to improve the actual performance level up to the importance level is needed, but this should firstly begin with defining roles of broad-based working Clinical Nurse Specialist.

        급성 뇌졸중 환자 대상 근거중심 간호 가이드라인 수용개작의 적용 평가

        This study was done to evaluate nursing guidelines for patients with acute stroke, developed by adapting the guidelines of Registered Nurses Association of Ontario, Canada to clinical settings on a large scale and evaluating the effectiveness as a research study. Method: The general characteristics of the 319 patients and the effectiveness of guideline application were evaluated in terms of structure, process, and outcome using questionnaires on the guidelines application with reference to the medical records of patients with acute stroke hospitalized on a ward of the stroke center of S General Hospital in Seoul. Results: Structures as a guidance system for assessment were consistent with the recommendations. With respect to the process of the guidelines, for items on nursing assessment, improved performance was found to be statistically significant. For outcomes of the guidelines, complications occurred in 8 patients (5.3%) prior to application of the guidelines and 11 patients (6.5%) after application of the guidelines, but this result was not statistically significant (p=.841). Conclusion: The results of the study indicate that for the effectiveness of the guidelines, accessibility to the guidelines and effectiveness of quality improvement need to be evaluated, in addition to complications of a stroke.

        근거중심 급성 뇌졸중 간호 가이드라인 수용개작

        Purpose: This methodological study translated, adapted, and validated the evidence based guideline of RNAO (Registered Nurses Association of Ontario) in Canada, for acute stroke care to domestic circumstances. Methods: Quality of the RNAO guideline was evaluated using AGREE tool. Then the guideline was translated into Korean and a preliminary guideline was established After checking content validation by an expert group, the Korean version of the guideline was applied to clinical settings to see its applicability. Results: The RNAO guideline in Korea was deemed appropriate. The Korean version of the guideline was drawn up with 56 items in 3 areas in stroke system related nursing, nursing of acute stroke inpatients, and stroke nursing assessment. A questionnaire survey was performed on appropriateness, enforceability, and applicability of those recommendations. A total of 54 recommendations were finalized after deleting 2 items below CVI 0.7 and correcting 3 items by taking professional advices. After trial application of the guideline to 40 stroke patients hospitalized at a ward of a hospital in Seoul, its performance was improved but was not statistically significant. Conclusion: This guideline is expected to contribute to improving nursing quality by offering it as a guide to evidence based practices for acute stroke care in Korea.

      • 뇌실외 배액술 관리에 대한 이해 및 업무수행 실태

        The objective of this study was to investigate the managing realities of EVD and provide a descriptive survey with the attempt to obtain basic data needed for the standardization of EVD management. For that purpose, 220 questionnaire copies were distributed to those nurses who were working with intensive care units at university hospitals, general hospitals, semi-general hospitals in Seoul, Kyongki-de and local areas. Of 158 returned questionnaire copies, 139 were used for analysis. Data were collected during the term of academic seminars of Neurosurgery Nursing Committee and KICA (Korea Intensive Care Association), ranging from Oct. 26 through 27. 2000. The tools utilized a structured questionnaire form consisting of 21 questions on general characteristics, the level of understanding, the level of work performance, standard management guidelines, etc. The data collected were analysed with frequency, percentage, mean vague, chi -square test using the SPSS 10.0 program Findings were revealed, as follows: For managing the prevention of EVD infection, aseptic technique was found to be of the most importance (48.9%),'the technique should be carried out at an operation room'(92.8%), but it was actually performed at intensive care units (over 51.5%). 'The term of antibiotics use should be less than 7 days' (81.8%), while it was actually executed for less than 7 days (49.5%). 'The proper time of quittance from it should be within less than 7 days'(74.1%) but actual quittance was executed only by 51.1%. 'Set exchange is needed' by 54.0% but actual exchange was implemented merely by 28.1%. 'The subject of management should be nurses' was reflected by 40% but actually 52.0% were carried out. As a result of hating analysed the understanding of the management of EVD by general characteristics, a significant difference was found in the necessity for set exchange by hospitals, in the time of quittance of a catheter, and in ablution items by work experience. The result of hating analysed the work performance levels of EVD management by general characteristics revealed that a significant difference in administration facilities of medicine by the classification of hospitals, especially the use of antibiotics and the quitting time of a catheter by the classification of intensive care units, and in the use of antibiotics and ablution items by the standard management guidelines.
