      • 계태심 Conotruncus의 전기적 충격으로 유발되는 심기형, 혈류변동 및 조직학적 변화에 관한 실험적 연구

        Electrical shock stimulation of the conotruncal area induced a spectrum of Double outlet right ventricle (DORV) in embryonic chicks at developmental stages 24-27. The device used to apply electrical potentials to the heart was constructed using a 9 volt battery, a push-botton switch, and a 100 ohm, ten-turn potentiometer with a calibrated dial. The potentiometer was connected as a voltage divider with the outlet connected to an electrode pair. This allowed for a short voltage pulse (variable 0 to 9 volts) to be applied to a selected area of the heart through the electrodes. Two different methods were chosen to apply electrical shocks to the conotruncal area of the heart. One method utilized the application of an electrical shock between two points across the conotruncus of cardiac loop (horizontally oriented) and the second method between two points along the conotruncus of cardiac loop (vertically oriented). The range of voltages applied to the conotruncal area of the heart was from 2 to 4 volts. Soon after electrical shock, continuous wave Doppler was applied for evaluation of the flow pattern at the level of the conotruncus. Three distinct types of double outlet right ventricles were in the longitudinal(L) and horizontal(H) electrical shock groups: 1) A double outlet right ventricle (DORV) without a ventricular septal defect(VSD), which is associated with a prolapse of an aortic valve into the right ventricle (5/50 cases in L group, 3/40 cases in H group). 2) DORV with a subaortic VSD, with a variable degree of pulmonary stenosis and a hypoplastic left ventricle (20/50 cases in L gorup, 12/40 cases in H group). 3) DORV with a subpulmonary VSD and a pulmonary stenosis, with hypoplastic right ventricle (15/50 cases in L group, 5/40 cases in H group). Longitudinal stimulation induced more cardiovascular anomalies than horizontal stimulation, and histological examination revealed the complete disappearance of the myocardial fibers, myocardial degeneration, and the aggregation of protein material or glycogen in the myocardial cells. After electrical shock, continuous wave Doppler pattern at the level of the conotruncus demonstrated that increased peak velosity and low level of percent window (%).

        The corps of engineer battalion is in the maintenance management service for about 30 battalions subordinated to the infantry division, but the maintenance management manpower is a small number because the criterion of the maintenance management manpower is in accordance with the battle organization. So, this study suggests the reasonable maintenance management manpower estimating model according to the area, the quantity and so on of army installation. In this case study, we investigate the present states and the actual conditions for the 3 infantry divisions and 3 corps of engineer battalions, and find out improvement method based on the interview with the persons concerned.

      • 急性絲毬體腎炎의 心超音響圖에 關한 硏究

        Patients with acute glomerulonephritis often are seen with signs suggesting heart failure. Whether these signs are due to fluid overload secondary to kidney damage only, or there is associated myocardial damage has not been elucidated. 30 children with acute glomerulonephritis were studied by echocardiography during edematous phase to evaluate cardiac function in these children the period from September 1979 to April 1980. The results obtained were as follows; 1. 12 children showed increased LAD, LA/Ao ratio and LVEDd in echocardiographic findings. 2. The cases of abnormal echocardingraphic findings according to clinical manifestations and laboratory findings were 2 of 2 cases with cardiomegaly, 4 of 6 cases with oliguria, 9 of 14 cases with hypertension, 2 of 4 cases with abnormal electrocardiographic findings, respectively. Among 12 cases of increased LAD, 7 cases showed normal chest X-ray and electrocardiographic findings. 3. There was no correlation between abnormal echocardiographic, roentgenographic and electrocardiographic findings. Left ventricular function(EF and % of FS) remained normal in all children and the most consistent findings were enlargement of left atrium and increased LA/Ao ratio. This study suggests that signs of heart failure in acute glomerulonephritis are probably due to fluid overload.

        A case of Kikuchi's disease with skin involvement

        Histiocytic necrotizing lymphadenitis, which is also commonly referred to as Kikuchi's disease (KD), is a self-limiting disease of unknown etiology. It affects individuals of all ages, although it is usually seen in young women. However, only a few descriptions of this disease are available in the pediatric literature. KD is clinically characterized by cervical lymphadenopathy, high fever, myalgia, neutropenia and, rarely, cutaneous eruptions. Cutaneous manifestations have been reported in 16-40 percent of KD cases. The specific skin changes occurring in cases of KD have yet to be completely characterized. In most of the reported cases thus far, the lesions have been located on the face and upper extremities. In this report, we describe a case of pediatric Kikuchi's disease, occurring in a 9- year-old boy. The boy exhibited transient erythematous maculopapular skin lesions over the entirety of his body, including his lower extremities. 일과성의 홍반성 피부병변을 동반한소아 Kikuchi병 1례고려대학교 의과대학 소아과학교실, 병리학교실*장지민·우철희·최정우*·송대진유 영·이광철·손창성Kikuchi병은 주로 젊은 여성에서 호발하고 소아에서는 비교적 드문 질환으로 원인 및 발생기전은 자세히 알려진 바 없으나 특별한 치료 없이도 수개월 이내에 자연적으로 회복되는 양성질환이다. 진단은 침범된 림프절의 특징적인 병리조직학적 소견을 확인하면 가능하다. 대부분의 환자에서 임상증상으로 림프절 비대와, 발열이 나타나고 피부증상은 16-40%에서 동반되며 주로 안면부, 상지, 상부 체간에 발생하고 대부분 작은 반점, 구진, 드물게 판과 결절 형태로 나타난다. 국내에서는 1983년 아급성 괴사성 임파선염이라는 이름으로 처음 언급한 이래 드물지 않게 보고되어왔으나 기존의 보고들은 피부증상을 동반하지 않거나 얼굴과 상지에 국한된 피부병변을 가진 증례 보고가 대부분이었다. 저자들은 지속적인 발열과 우측 경부 림프절 비대를 주소로 내원한 9세 남아에서 림프절의 조직검사결과 Kikuchi병을 진단할 수 있었고 이후 하지를 포함한 전신에 반점과 구진형태의 홍반성 병변이 발생한 Kikuchi병 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

        지표를 활용한 육군시설 유지관리인력 산정모델

        So far, the corps of engineer battalion is in charge of facility management servics for about 30 battalions subordinated to the infantry division. He operates an irregular organization for construction of new facilities and maintenance of the existing facilities. In 1990, the Ministry of National Defense established a plan for modernizing and improving military facilities. According to the plan, military facilities have been reinforces and increased. However, the facility management manpower has not been increased, although they have retained up-to-data facilities, equipments necessary manpower. Despite new requirements on manpower, even the criteria to estimate facility management manpower in the Army does not exist. Therefore, this study p개poses a reasonable manpower estimating model for the military facility management using the macroscopic and microscopic indices.

      • 국민학교 아동의 혈중 지질에 관한 연구

        A total 440 school children who was 10 to 12 years of age from three different areas (Hongchon, Suwon and Seoul) participated in our cross-sectional study of serum lipid analysis. After fasting for 12 hours or more, serum cholesterol, high density lipoprotein (HDL) cholesterol and triglyceride levels were ascertained. After then, height, weight, head and chest circumference, skinfold thickness, midarm circumference and blood pressure were checked by usual method. Low density lipoprotein(LDL) cholesterol and so called atherogenic index were calculated by some equation. The results obtained were as follows : 1) Average total serum cholesterol level was 163.82±28.49mg /dl, HDL cholesterol 55.77± 11.63mg/dl, LDL cholesterol 93.07 ±38.90mg/dl, triglyceride 82.35 ±41.44mg/dl respectively. Average atherogenic index was 2.03±0.70, which was increased by aging(p<0.05). 2) Serum LDL cholesterol level and atherogenic index were higher in female than male children (p<0.05, 0.01) 3) Total serum cholesterol and HDL cholesterol levels were higher in children in Seoul area(p<0.001, 0.001), and LDL cholesterol level was lower in children in Hongchon area(p<0.001) Atherogenic index was higher in children in Suwon area (p<0.05). 4) There were decreased tendency by aging in incidence of abnormal high serum total chloesterol and LDL cholesterol levels (p<0.001, 0.05). And there were some increased tendency by aging in incidence of abnormal high triglyceride and atherogenic index levels. 5) There had correlations between serum cholesterol level and subscapular skinfold, abdominal skinfold, and triglyceride level also had correlation with both systolic and diastolic blood pressure, height, weight, chest circumference, midarm circumference, subscapular skinfold, abdominal skinfold and triceps skinfold. Atherogenic index also had correlations with body weight, midarm circumference, chest circumference, subscapular skinfold, triceps skinfold and abdominal skinfold.

      • 선천성 심장질환에서의 혈장Alpha-Atrial Natriuretic Polypeptide 농도에 관한 연구 : 좌-우 단락을 중심으로

        Atrial natriuretic polypeptide has been shown to have many important biologic effects, including natriuretic, diuretic, and smooth muscle relaxant activity. Stretching of the atrial wall by increased atrial volume or pressure is a well-documented stimulus for the release of ANP. The relationship between different hemodynamic parameters and plasma alpha-ANP level in congenital heart diseases with left to right shunt were evaluated. The results were obtained as follows; 1. Plasma levels of alpha-ANP in patients in congenital heart diseases(CHD) with left to right shunt were significantly higher than those in control groups (86.0±1.3pg/ml vs. 29.3±7.0pg/ml) 2. Each plasma levels of alpha-ANP in patients with ventricular septal defect(VSD), atrial septal defect(ASD), patent ductus arteriosus(PDA), and congestive heart failure were 81.5±3.5pg/mi, 95.1±2.3pg/ml, 88.4±9.3pg/ml, and 222±1.2pg/ml, respectively. 3. Plasma levels of alpha-ANP were positively and significantly correlated with Qp/Qs ratio in patients with congenital heart disease, such as ventri cular septal defect, atrial septal defect, patent ductus arteriosus (CHD; r=0.72, p<0.001, VSD; r=0.67, p<0.01, ASD; r=0.91, p<0.01, PDA; r=0.88, p<0.01). 4. Plasma levels of alpha-ANP were positively and significantly correlated with mean gight atrial pressure and peak pulmonary artery pressure(r=0.63, p<0.01, r=0.62, p<0.01, respectively). 5. After surgical correction of congenital heart dexeases, plasma alpha-ANP levels were decreased sigmificantly(p<0.05).

      • 中·高生에서의 血淸 脂蛋白値에 關한 硏究

        Lipoprotein(a) 〔Lp(a)〕 is a class of lipoprotein particles having the lipid composition of plasma low density lipoprotein(LDL), but with a distinct protein moiety composed of two proteins linked together by a disulfide bridge. The two proteins are apo B-100, the protein moiety of LDL, and apo(a), a heavily glycosylated protein that is specific for Lp(a). Apo(a) has a strong structural similarity to plasminogen and has a wide-size polymorphism that has a genetic origin and is partially responsible for the size and density heterogenecity of Lp(a). High plasma levels of Lp(a) are associated with a n incrased risk for cardiovascular disease that is related to atherogenic and thrombogenic potentials of this lipoprotein enhanced by the presence of other risk factors, among which are high plasma levels of LDL or low levels of high density lipoprotein. The factors determining of plasma levels of Lp(a) has not been clearly indentified. The study subjects were 86 boys and 80 girls aged 12 to 17 years. We measured serum concentration of Lp(a), apolipoprotein A-1 and B using the ELISA method. Median serum Lp(a) concentration was 12.0mg/dL, and median serum concentration of apolipoprotein A-1 and apolipoprotein B were 33.4mg/dL and 51.2mg/dL respectively. Girls had higher Lp(a) then boys(p<0.05), but sex-related differences were not, seen in apolipoprotein A-1 or apoprotein B. Age-related changes were not apparent for Lp(a) or apolipoprotein A-1 or apolipoprotein B. Lipoprotein Lp(a) apparently exerted atherogenic effects when it carried above 30mg/dL. In this study, above 30mg/dL of Lp(a) were 12(7.2%) subjects, who should be followed other cardiovascular risk factors and be treated. Lp(a) should be advised to correct to modifiable risk factors in order to decrease the cardiovascular pathogenecity of the lipoprotein class.
