본 연구에서는 electroless nickel / immersion gold (ENIG) 처리된 printed circuit board (PCB)를 무연 솔더인 Sn-3.5(wt%)Ag로 접합하였다. 리플로우 횟수를 1회부터 10회까지 다양하게 하여 리플로우 횟수가 증가함에 따른 솔더 접합부의 기계적, 전기적 특성의 변화에 대해 연구하였다. 접합부의 미세 조직 관찰을 위해 접합부 단면을 폴리싱 하여 금속간 화합물의 두께를 측정하고 종류를 분석하였다. 접합부의 기계적 특성을 평가하기 위해서 die 전단 시험을 하였는데, 리플로우 횟수가 4-5회일 때까지 전단 강도 값이 증가하다가 5회 이후로 감소하였다. 전기적 특성을 알아보기 위해 전기 저항 값을 측정하였는데, 리플로우 횟수가 증가할수록 접합부의 전기 저항 값은 점점 증가하였다. In this study, solder joints were made with Sn-3.5Ag (wt%) solder ball. Electroless nickel / immersion gold (ENIG) printed circuit board (PCB) substrates were employed in this work. The mechanical and electrical properties were measured as a function of the number of reflow. Die shear strength was measured with increasing reflow number. Until the forth or fifth reflow, shear force increased and after the fifth reflow the shear force of die decreased. The electrical resistivity of solder joint linearly increased with increasing reflow number.

Kidney cortex necrosis is a relatively rare cause of acute kidney injury and is characterized by complete or partial destruction of the renal cortex, but sparing of the medulla. Tranexamic acid has antifibrinolytic activity and is used to reduce bleeding. We report a rare case of kidney cortex necrosis caused by tranexamic acid. A 49-year-old woman complained of coughing up blood-tinged sputum. She had a history of bronchiectasis and was treated with tranexamic acid for 3 days. Four days after admission, she developed anuria and azotemia. Computerized tomography showed enhancement of the renal medulla, but not the bilateral renal cortex. The patient was treated with hemodialysis, and has since been maintained on hemodialysis for 6 months. Due to the development of kidney cortex necrosis in patients treated with tranexamic acid, all its potential complications should be considered. (Korean J Med 2012;82:503-506)

The mechanical and electrical properties of ball grid array (BGA) solder joints were measured, consisting of Sn-3.5Ag, with organic solderability preservative (OSP)-finished Cu pads and Electroless Nickel/Immersion Gold (ENIG) surface finishes. The mechanical properties were measured by die shear test. When ENIG PCB was upper joint and OSP PCB was lower joint, the highest shear force showed at the third reflow. When OSP PCB was upper joint and ENIG PCB was lower joint, the highest shear force showed at the forth reflow. For both joints, after the die shear results reached the highest shear force, shear force decreased as a function of increasing reflow number. Electrical property of the solder joint decreased with the function of increasing reflow number. The scanning electron microscope results show that the IMC thickness at the bonding interface gets thicker while the number of reflow increases.

외래에서 혈뇨 환자를 접하게 되면, 환자의 활동성 및 운동 여부를 반드시 확인하고, 혈뇨가 운동으로 유발되고, 운동 중단 후 호전되는지 확인하여야 한다. 그리고 종양, 감염, 약물, 요석, 신염과 운동 유발 혈뇨를 감별하여, 검사의 진행 여부를 결정해야 한다. 하지만 운동 유발 혈뇨가 다른 신장-비뇨기계의 증상 중 하나일 수 있기 때문에 반복적인 혈뇨, 연령 등에 따라 검사가 필요함을 염두해 두어야 한다. Exercise-induced hematuria is a phenomenon occurring in subjects who participate in strenuous exercise. Rapid resolution is an important feature of exercise-induced hematuria. We report here a case of exercise-induced hematuria presenting as gross hematuria Lasting I week in a 19-year-old male patient. Gross hematuria developed after strenuous exercise about 3 years ago. Three months ago, recurrent gross hematuria was Lasting I week, regardless of exercise intensity. Compression of the Left renal vein between the aorta and superior mesenteric artery, without prominent venous collaterals, was detected by computed tomography. However, no abnormalities were detected by renal venography, arteriography or kidney biopsy. Exercise-induced hematuria occurs with a high incidence, but is self-Limiting. In contrast, recurrent and gross hematuria can be associated with bladder carcinoma or vascular abnormality. This should be kept in mind, and urological evaluations such as cystoscopy and angiography are necessary in gross and recurrent hematuria. (Korean J Med 2012;82:95-99)

Despite the splendid development of medicine, traditional alternative remedies have been widely used in Korea for a long time. Especially, the extract of elm bark is typical, common and easily obtained in a daily life. A scientific name of elm is Ulmus davidiana var. japonica. Although the study of pharmacologic effect and side effect is underway, the results are not yet enough to be applied in practice. Nevertheless, many people have the extracts of elm bark on expecting the improvement of health. We experienced a case of acute toxic hepatitis and acute kidney injury after complementary medication. She was a 48-year-old female patient who ingested 3-4 cups of extracts per day for 10 days to improve her health until admitted due to nausea, vomiting, and fatigue. The case report can suggest that ingestion of complementary medicine (elm bark root extracts) can be a cause of acute toxic hepatitis and acute kidney injury.
지속성 외래 복막투석 환자에서 발생한 Comamonas testosteroni 복막염 1예

Comamonas testosteroni has rarely been implicated as a human pathogen. We here present a case of peritonitis due to this organism in a patient on continuous ambulatory peritoneal dialysis (CAPD). A 32-year-old woman was admitted with abdominal pain and cloudy peritoneal effluent. Empirical intraperitoneal (IP) treatment with cefazolin and ceftazidime was started. The culture was positive for C. testosteroni and antibiotic was changed to ceftazidime IP. Four days after the ceftazidime treatment, the patient became asymptomatic. The follow-up culture from peritoneal effluent at 7th day was negative. This treatment was maintained for 21 days. After that, culture negative peritonitis occurred twice for 2 months in this patient, so CAPD catheter had to be removed. This is the first reported case of CAPD peritonitis caused by C. testosteroni. It is important for clinicians to recognize that CAPD peritonitis is caused by this organism which has been largely overlooked as a potential pathogen.

결절성경화증은 지능저하, 간질, 피지선종이 특징적으로 나타나는 상염색체 우성 유전 질환으로 뇌, 눈, 심장, 폐, 간, 콩팥, 피부 등 여러 장기에 양성 종양성 병변을 일으키는 질환이다. 혈관근지방종은 혈관, 평활근, 지방조직으로 구성된양성 종양으로, 결절성경화증과 연관되어 발생하는 경우도 많다. 혈관근지방종은 신 실질에 가장 흔하며, 이외 간, 비장, 자궁관, 림프조직 등의 복부 장기 및 폐, 심장에서 발생하기도 한다. 결절성경화증에서 종양이 여러 장기에 동시에 생긴예는 매우 드물다. 저자들은 결절성경화증 환자에서 신장과간에 혈관근지방종이 있고 임파관평활근종증 혹은 다발성 미세결절폐세포증식증 등의 폐 침범을 나타내는 2예를 경험 하였기에 보고하는 바이다. Tuberous sclerosis (TS) involves multiple organs. Angiomyolipoma of the liver or kidney is one of the clinical manifestations of TS. However, coexistent renal and hepatic angiomyolipoma associated with TS is a rare condition. Pulmonary involvement is extremely rare, and occurs in only 0.1~1% of TS. We report two cases of concurrent renal and hepatic angiomyolipomas with pulmonary involvement in patients with TS. The first case was a 35 year-old woman who showed multiple angiomyolipomas in the liver and both kidneys and cystic parenchymal changes in the lungs. The other case was a 27-year-old woman who showed multiple angiomyolipomas in the liver and both kidneys, and multinodular pulmonary shadows. To the best of our knowledge, this is the first reported case of renal and hepatic angiomyolipomas associated with pulmonary involvement in Korea. (Korean J Med 79:705-709, 2010)

Kaposi`s sarcoma (KS) is an unusual multifocal neoplasm of vascular endothelial cell origin. The trunk, arms, head, and neck are the most common sites. It is common in men and has four distinct variants: classic, Africa-endemic, immunosuppressive drug-associated, and acquired immunodeficiency syndromeassociated KS. KS appears to develop immunosuppressed patients, but is uncommon in patients on dialysis. A 79-year-old man on hemodialysis for 2 months presented with pruritus over the entire body and multiple, discrete, variable-sized, dark blue papulonodules (papuloplaques, maculopapules) on the left arm and shoulder. A biopsy specimen form the left arm showed spindle cells with slit-like spaces and extravasated red blood cells. The specimen was positive for CD 34 antigen, and human herpesvirus 8 was detected. We report a case of KS that occurred in a 79-year-old patient on hemodialysis.

시중에 유통되고 잇는 콩나물요 콩중의 잔류농약을 조사하고원료콩에 잔류하는 농약이 재배산물인 콩남루로 이행하는지에 대해 알아보기 위해 콩나물용 콩, 농약을 처리한 콩 및 농약을 처리한 콩으로 재배한 콩나물중의 잔류농약을 비교븐석하였다. 전국 9개 대도시에서 5회에 걸쳐 수집한 국내산 콩 45시료와 농수산물유통공사와 국립농산물검사소를 통하여 수집한 수입산 콩 20시료 중의 carbendazim의 잔류분석을 실시한 결과 수집한 시료 65건 모두에서 carbendazim이 검출되지 않닸다. 콩나물 원료콩에 농약잔류허용기준만큼의 농약이 잔류되도록 하기 위해서 원료콩에 benomyl, thiophanate-methyl과 thiram 등 3가지 서로 다른 농도치 농약용액에 1시간동안 침지한 후 3일간 건조시켰다. Benomyl 용액 0.5,20,50ppm으로 수침한 후 건조한 몽에서는 각각 nd, 0.58,1.99ppm이 잔류하였으며,thiophanate- ethyl용액 0.5,5,50ppm으로 수침한 후 건조한 콩에서는 각각 nd 0.22,2.35ppm이 잔류하였다. 그리고 thiram용액 0.j, 5, 50ppm으로 수침한 후 건조한 콩에서는 각각 0.12, 0.45, 5.88ppm이 잔류하였다. 콩나물은 경기도 고양시 화정구 소재 콩나물농장의 암실에서 일반적인 방법으펀 재배하였는데,농약을 처리하여 건조시킨 원료콩을물에 불린 후 Ik9씩 재배통에 분배하여 1회에 15분동안 1일 5치씩 물을 주면서 7일동안재배하였다. 그레나, 이러한 농약을 처리한 원료콩으로 콩나물을 재배하여 재배산물인 콩나물에 잔류되어 있는 농약을 분석한 결과 benomyl, thiophanate-Hlethyl과 thiram의 모든 경우에서 농약이 긷출되지 않았다. To assure whether pesticide residues of beaa sprouts were transferred from the raw commodity, beans or not, the pesticide residues were determined in the domestic and the imported beans, in the pesticide-treated beans and in the cultivated bean sprouts. The domestic beans (45 samples; 9 cities x 5 times) and the imported beans (15 samples of USA and 5 samples of China) were collected and analyzed as carbendazim. No carbendazim residue was defected in all 65 bean samples. Beans containing carbendazim residues as much as nd, 0.58 and 1.99ppm were obtained as follows : pesticide-free beans were washed, soaked in 0.5,20 and 50pprn of benomyl solutions for 1 hour and dried for 3 days. By treating beans with 0.5, 5 and 50ppm of thiophanate-methyl solutions, beans containing carbendazim residues as much as nd, 0.22 and 2.35ppm were obtained. In addition beans containing 0.12, 0.45 and 5.88ppm of thiram residue were obtained by treating beans with 0.5, 5 and 50ppm solutions of thiram. However, after cultivating the pesticide-treated beans for 7 days in the dark, no pesticide residue was detected in bean sprouts.
