        늑골과 두개골막피판을 이용한 두개성형술: 증례보고

        Purpose: The goals of cranioplasty are to protect the brain from trauma and restore normal cranial contour with as few complication as possible. In patient who suffered from cranial defect, the most satisfactory form of cranioplasty may be archived with autogenous bone grafts. We report on the treatment of large frontal bone defect using pericranial flap and split-rib graft. Methods: A 29-year-old male was referred to our department. He was involved in an automobile accident resulting in large frontal bone and sinus defect and skull basal defect. The reconstruction was undertaken using galeal frontalis myofascial flap and split-rib cranioplasty using bicoronal incision. Results: The postoperative course was successful. For 9 months follow up period, there was no complication about hematoma, infection, CSF leakage and washboard deformity. He was pleased with the results even when absolute symmetry was not achieved. Conclusion: We experienced large frontal bone and sinus defect and skull basal defect patient and successfully treated with split-rib graft and galeal frontalis myofascial flap.

        Genome walking is a par ticular application for identifying sequences of unknown genomic regions adjacent to a known region. Many genome walking methods based on polymerase chain reaction (PCR) are available. Even if earlier techniques suffer from low reproducibility, inefficiency, and non-specificity, improved strategies have been developed. In this study, we present an alternative strategy: the genomic DNA is digested with restriction enzymes. After cytosine overhangs at 5' ends, the fragments are ligated to linker adaptor s had guanine overhang at 3' ends. Then nested PCR is performed. The improvements in this strategy focus on two points. The first is the C tailing method using Pfu polymerase instead of the A tailing method based on nontemplate-dependent terminal transferase activity of Taq polymerase. Therefore unintended modification of target DNA can be prevented without A tailing error. The second point is the use of C/G-specific ligation had advantage in the ligation efficiency compared with A/T-specific ligation. Therefore, the C-G linker PCR method increases ligation efficiency between digested genomic DNA and adaptor DNA. As a result, the quantity of target DNA to amplify by PCR is enriched. We successfully used G-C linker PCR to retrieve flanking regions bordering the phophinothricin resistance gene in genetically modified tobacco (GMO).

        자석 압박을 이용한 다양한 이개 질환의 치료

        Purpose: The earlobe is one of the areas which are most vulnerable to trauma. Various auricular diseases need compression treatment. We report a new compression method using magnetic disks.Methods: Seventeen patients with auricular diseases were treated from October 2002 to September 2006. The mean age was 29.1 years. The diseases details were osteochondroma in 2 patients; cauliflower's ears in 2 patients; acute otohematoma in 1 patient; and hypertrophic scars in 11 patients. The most common cause of their disease was ear piercing. The mean follow-up period was 8.9 months. All surgical procedures were performed under local anesthesia. To compress immediately, a pair of magnetic disks was applied to the anterior and posterior surface of the earlobe. Results: The results were generally good. Major complications, such as recurrence, necrosis, dehiscence, or infection, did not occur. Conclusion: A pair of magnetic disks are useful compression tool in various auricular diseases.

        Effect of Pyunkang-tang on Inflammatory Aspects of Chronic Obstructive Pulmonary Disease in a Rat Model

        We investigated the anti-inflammatory effect of Pyunkang-tang extract (PGT), a complex herbal extract based on traditional Chinese medicine that is used in Korea for controlling diverse pulmonary diseases, on cigarette smoke-induced pulmonary pathology in a rat model of chronic obstructive pulmonary disease (COPD). The constituents of PGT were Lonicerae japonica, Liriope platyphylla, Adenophora triphilla, Xantium strumarinum, Selaginella tamariscina and Rehmannia glutinosa. Rats were exposed by inhalation to a mixture of cigarette smoke extract (CSE) and sulfur dioxide for three weeks to induce COPD-like pulmonary inflammation. PGT was administered orally to rats and pathological changes to the pulmonary system were examined in each group of animals through measurement of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) levels in bronchoalveolar lavage fluid (BALF) at 21 days post-CSE treatment. The effect of PGT on the hypersecretion of pulmonary mucin in rats was assessed by quantification of the amount of mucus secreted and by examining histopathologic changes in tracheal epithelium. Confluent NCI-H292 cells were pretreated with PGT for 30 min and then stimulated with CSE plus PMA (phorbol 12-myristate 13-acetate), for 24 h. The MUC5AC mucin gene expression was measured by RT-PCR. Production of MUC5AC mucin protein was measured by ELISA. The results were as follows: (1) PGT inhibited CSE-induced pulmonary inflammation as shown by decreased TNF-α and IL-6 levels in BALF; (2) PGT inhibited the hypersecretion of pulmonary mucin and normalized the increased amount of mucosubstances in goblet cells of the CSE-induced COPD rat model; (3) PGT inhibited CSE-induced MUC5AC mucin production and gene expression in vitro in NCI-H292 cells, a human airway epithelial cell line. These results suggest that PGT might regulate the inflammatory aspects of COPD in a rat model.

        수술 전 초음파를 통한 육안적 갑상선 피막 외 침윤의 예측

        Background and Objectives The American Joint Committee on Cancer (AJCC) stagingsystem 8th Edition has stated that gross extrathyroid extension (ETE) is more important thanmicroscopic ETE, and that it plays an important factor in the classification of T stages. Therefore,the prediction of gross ETE before surgery is important in establishing the clinical stageand determining the surgical extent. However, there are few studies predicting gross ETE usingpreoperative ultrasonography (US). The purpose of this study is to predict the gross ETEof papillary thyroid carcinoma (PTC) using preoperative US. Subjects and Method The preoperative US findings of 688 patients with PTC were comparedwith gross ETE confirmed during the surgery. The efficacy of preoperative US was statisticallyanalyzed. Results A total of 70 (10%) patients were confirmed of their gross ETE. Thyroid tumorswere classified into three groups (isolation, capsular contact, and capsular protrusion) accordingto their relationship with capsules, and evaluated using the preoperative US. There was asignificant difference in gross ETE between groups (p<0.001). The thyroid tumor that cameinto contact with the capsule or protruded in the anterior region showed a difference betweenthe capsular contact and capsular protrusion groups (p<0.001), whereas those came into contactwith the capsule or protruded in the posterior region showed no significant difference betweenthe two groups (p=0.187). There was no difference in the degree of protrusion (<25%,25-50%, ≥50%) in the capsular protrusion group (p=0.868), but the difference in tumor sizewas significant (p<0.001). Conclusion Preoperative US is a useful tool for predicting gross ETE and is more predictablewhen the thyroid tumor is located anteriorly.

        Man has produced various types of technical products to more and enrich his life and make in convenient. The technical culture has been kept developing up to today, with the infinite efforts, for the purpose of meeting these natural wished of mankind. The treces which the race has trod, whichever races their countries consist of, have the traditions descending from deep roots through customs and beliefs in themselves. In the heart of these traditions, there was a technical culture of wooden technical arts which was glorious in the Chosun Dynasty's era. But as the period elapsed Dynasties changed their hands it must in turn be great pities that in accordance to the changes of the period the glorious cultural heritages should be extinguished and destroyed. On theis account, I would seek to search for 'the Korean characteristic', the flavor and beauty of Korea so that we might rediscover ours in the disappearing traditions. This thesis shows that not only the kind and use of the wooden technical arts in the Chosun Dynasty's era but especially the kind of desighs, the function and name of the metal- ornament got analysed, and that in carefully studying and examining the the designs, forms and function of hinges I would appreciate the excellent tradition and wisdom of our nation borne in the hinges. As the metal-ornament bears the human in erest of a nation most naturally and plainly, as the wooden technical arts do well represent peculiar characteristics thickening the local colors, the purpose of this thesis on the metal- ornament is to establish the function and aesthestics of the metal-ornament attached in the wooden technical arts, to reanalyse our tradition on the basis of modern views and to present the modern metal- ornament, especially the direction of hinge- designs. Therefore, the result of these researches is expected to be the available data presenting the direction of the designs and the imporvements of the modern metal- ornament.
