        도스토예프스키의 「온순한 여자」에 나타난 ‘오만’과 ‘온순’의 구조 시학

        The primary goal of this paper is to pursue the main idea of 『A Meek Creature』 of F. M. Dostoevsky through its structural peculiarity. Many critics have analyzed this work, focusing on the eminent structure of interior monologue with which the husband of a meek woman, who committed suicide just before, narrates his own psychology. But analyzing contents of this work only under this structural frame hinders us from concrete understanding of internal world of the meek woman who attains not only the trait of meekness but also of arrogance. In the <From the author> of this work we can understand that Dostoevsky uses artistic variations which help to successfully forming the shape of the heroine of this work. From this we can understand that husband and wife of this work share the traits of meekness and arrogance. The exact meaning of meekness reveals itself on the changed shape of young wife of part two of this work and in the scene when she commits suicide with an icon in her hand. This represents symbolically that she has taken her own life with the spirit of unity with her husband who oppressed her with his own philosophy of silentness which is based on his hatred against the world. Such a silent death of meekness which takes the fault of her husband for herself represents the right essence of meekness which Dostoevsky wanted to cultivate in the chaotic russian society of that time.

        Distal Subclavian Artery Occlusion Causing Multiple Cerebral Infarcts Consequence of Retrograde Flow of a Thrombus?

        Intracranial embolization usually arises from the heart, a vertebrobasilar artery, a carotid artery, or the aorta, but rarely from the distal subclavian artery upstream of an embolus. We report on a patient who experienced left shoulder and forearm pain with weak blood pressure and pulse followed by concurrent onset of left hemiplegia. This case is a rare example of multiple cerebral embolic infarctions, which developed as a complication of distal subclavian artery thrombosis possibly associated with protein S deficiency.

      • Monitoring of Gamma-ray Bright Quasars 3C279 and 1510-089 at 22, 43 and 86GHz using KVN Single Dish Telescopes

        AGN(Active Galactic Nucleus) consists of a supermassive black hole located at its center, an accretion disk around the black hole, and bipolar jets. Since May 2011, we have performed the MOGABA(Monitoring Of GAmma-ray Bright AGN) project for observing gamma-ray bright AGN once a week at multifrequencies using KVN(Korean VLBI Network) 21m radio telescopes. The MOGABA project is the observations for measuring the degree of polarization, polarization angle, and total flux of about 20 AGN at 22, 43 and 86GHz. By this project, we are able to investigate polarization characteristics, spectral index, and variation of rotation measure at radio wavelengths of gamma-ray bright AGN and to study possible relation between gamma-ray flares and magnetic field structure change in AGN. According to previous research, gamma-ray flares of some AGN are coincident with large changes in angle of linear polarization. In this paper we report the preliminary results of linear polarization and total flux at 22, 43, 86GHz of gamma-ray bright quasars 3C279 and 1510-089 showing noticeable variation of total flux at 22GHz in late 2011, and discuss possible correlation with gamma ray light curves.

        위암의 임파절 전이 진단의 크기 기준에 대한 재평가

        Purpose: To reassess the usefulness of criterial for lymph node size in predicting metastatic lymph node of gastric adenocarcinoma and to determine appropriate size criteria. Materials and Methods: We reviewed the pathology of 1669 lymph nodes from 105 gastric adenocarcinoma patients and measured their long and short axis diameters. To determine the degree of lymph node shrinkage during fixation for pathologic examination, we measured the size of 105 lymph nodes both in the fresh state and on pathology slides after fixation and obtained their correlation equation; this was used to determine the fresh-state size of the 1669 nodes. We analysed the distribution of metastatic and nonmetastatic lymph nodes according to their long and short axis and plotted a sensitivity-specificity curve to determine the appropriate size criteria for metastatic lymph nodes. Results: There were 535 metastatic lymph nodes and their mean diameter was 8.2$\pm$4.9mm on the long axis and 5.7$\pm$3.8mm on the short axis. The mean diameter of nonmetastatic lymph nodes was 4.9$\pm$2.8mm 3.0$\pm$1.7mm on the long and short axis, respectively. The difference in size between metastatic and nonmetastatic lymph nodes was not statistically significant. The (P>0.05) distribution curve of metastatic and nonmetastatic lymph nodes overlapped over a wide range of lymph node size. We determined appropriate size criteria at the point where sensitivity and specificity curves crossed, but the form of the curves was such that this was difficult. Conclusion: Appropriate size criteria on lymph node metastasis were 6.2mm for #1-6 lymph node group and 8.1mm for #7-12 lymph node group on long axis diameter and 4.0mm, 5.3mm on short axis diameter. But, even with this size criteria the expected accuracy was low(67-70%). This results showed limitation of size criteria to diagnose metastaic lymph node.

        『스쩨빤치꼬보 마을 사람들』 과 『죽음의 집의 기록』에 나타난 자유의 양상과 의미

        This paper is focused on the analysis of F. M. Dostoevsky`s two novels, 『The Village of Stepanchikovo』 and 『Notes from the House of the Dead』 from the point of freedom. These works, which were written shortly after he got freedom after eight years of penal servitude and compulsory military service in Siberia, diversely express his ideas how one perceives freedom and in what way one can explore his own path to freedom. In 『The Village of Stepanchikovo』 Dostoevsky mainly focuses on the way of thinking of Nastja and Rostanev who willingly admit egoistic desire for freedom of Foma Opiskin. Not recognizing their own rights for freedom, their non-calculative attitude forms more freedom and happiness for both of them and Foma. In this way Dostoevsky develops his unique idea that real freedom can be acquired when one is free from desire of freedom which is based on the concept of right. In 『Notes from the House of the Dead』 Dostoevsky shows that one should be liberated from his stalled or fixed viewpoint on the world and people, if he really wants to recognize the real value of freedom. Gorjnachikov, narrator of this novel, realizes this in the part of < spectacle >, which is a kind of virtual reality. The < preface > of this novel can be regarded as a process of his being liberated from stalled self and thus finding real meaning of freedom. Overall, in these two works we can see how earnestly and humbly Dostoevsky began to pursue theme of freedom which will be more deeply delved in his later works like 『Crime and Punishment』 and 『Brothers Karamazov』.

        신 허혈 및 재관류 손상의 역동적 자기공명영상 소견

        Purpose: To evaluate the usefulness of magnetic resonance imaging (MRI) in a renal ischemia-reperfusion injury. Materials and Methods: Twenty-four rabbits were randomly divided into four groups, including a sham operated group (n=3). Renal ischemia was induced for 30 minutes (group 1), 60 minutes (group 2) and 120 minutes (group 3). MR imaging was performed before ischemia as well as one hour, 24 hours, and 72 hours after reperfusion. A 99mTc-dimercaptosuccinic acid (DMSA) scintigraphy was performed before ischemia, as well as 24 hours and 72 hours after reperfusion. The signal-to-noise ratio (SNR) on the T2WI, time-relative signal intensity (%RSI) curve on dynamic enhanced images, and relative left renal uptake (%) on DMSA scan were obtained and compared to the histologic findings. Results: The SNR of the cortex on the T2WI changed significantly over the course of the reperfusion time (p<0.001), but was not significantly different among the ischemia groups. The area under the time-%RSI curve gradually decreased from cortex to inner medulla before ischemia, which was reversed and gradually increased after reperfusion. The areas under the time-%RSI curve of outer and inner medulla were significantly different among the ischemia groups (p=0.04, p=0.008). The relative renal uptake (%) of left kidney decreased significantly over the reperfusion time (p=0.03), and was also significantly different among the ischemia groups (p=0.005). Tubular cell necrosis was observed in 16 rabbits (76.2%). The histologic grades of group 3 were higher than those of group 1 and group 2 (p=0.002). Even in rabbits without tubular cell necrosis, the areas under the time-%RSI curves of the cortex, outer, and inner medulla after a 72 hour reperfusion time were significantly lower than those before ischemia (p=0.007, p=0.005, p=0.004). Conclusion: The results of this study suggest that dynamic enhanced MR imaging could be a useful tool for the evaluation of renal ischemia and reperfusion injury. 목적: 신 허혈과 재관류 손상 평가에 있어 자기공명영상의 유용성을 알아보고자 하였다. 대상과 방법: 실험군 토끼를 3군으로 분류하고 각 군당 7마리 토끼의 좌 신장을 일시적으로 결찰하여 신 허혈을 발생시킨 후 1군은 30분, 2군은 1시간, 3군은 2시간 후 결찰을 제거하여 재관류를 유발하였다. 자기공명영상은 결찰 전과 재관류 1시간, 24시간, 72시간 후에, 신 피질 스캔은 99mTc-dimercaptosuccinic acid(DMSA)를 이용하여 결찰 전과 재관류 24시간, 72시간 후에 시행하였다. T2 강조 영상과 역동적 조영 증강 영상을 얻었다. T2 강조영상에서의 신호대잡음비, 역동적 자기공명영상에서 시간에 따른 상대신호강도 곡선과 DMSA 스캔에서 상대적 신 섭취비를 분석하였으며, 이를 조직학적인 소견과 비교하였다. 결과: T2 강조 영상에서 피질의 신호대잡음비는 재관류 시간에 따른 변화는 유의하였으나(p < 0.001) 허혈군 간의 차이는 유의하지 않았다. 역동적 자기공명영상에서 상대신호강도 곡선은 재관류 후 정점까지 도달하는 시간이 지연되거나 곡선의 형태가 편평하게 바뀌었으며, 허혈 시간이 길수록 이러한 변화가 뚜렷했다. 재관류 후 역동적 자기공명영상에서 피질의 상대신호강도 곡선 아래 면적보다 수질의 곡선 아래 면적이 더 커져 역전되었으며 시간 경과에 따라 그 정도는 감소하여 회복되는 양상을 보였다. 외수질과 내수질 모두에서 상대신호강도 곡선 아래 면적은 허혈 군에 따라 유의한 차이를 보였다(p=0.04, p=0.008). DMSA 스캔에서 상대적 신섭취비는 허혈군과 재관류 시간에 따른 변화가 유의하였다(p=0.03, p=0.005). 세뇨관 세포의 괴사는 16마리(76.2%)에서 관찰되었다. 3군의 조직학적 등급은 1군이나 2군보다 유의하게 높았다(p=0.002). 조직학적으로 세뇨관 세포의 괴사가 관찰되지 않은 5마리에서도 재관류 72시간 후의 피질, 외수질, 내수질의 상대신호강도곡선 아래 면적은 각각 신동맥 결찰 전보다 유의하게 낮았다(p=0.007, p=0.005, p=0.004). 결론: 역동적 자기공명영상은 신 허혈 및 재관류 손상 평가에 유용할 것으로 생각한다.

        자발적으로 발생한 심장골회선 동맥 손상에 의한 복벽 혈종의 코일 색전술 치료: 증례 보고

        복벽에 발생하는 혈종은 비교적 드문 질환이며, 외상 혹은 자발적으로 발생한다. 주로 복직 근초나 측복벽에서 하복벽 동맥이나 장골회선동맥의 손상에 의해 발생하며, 자발적으로 발생 한 복벽 혈종은 임상적으로 다른 급성 복증을 일으키는 질환과의 감별이 어려울 수 있으므로, 조기에 CT와 혈관조영술을 시행하여 진단하는 것이 중요하다. 저자들은 심한 기침 후 심장골 회선동맥 손상에 의해 발생한 전측복벽의 혈종을 코일을 이용한 혈관 색전술로 치료한 1예를 보고하고자 한다. Abdominal wall hematoma is a rare but well-known disease, usually caused by trauma or, on rare occasions, occurring spontaneously. Hematomas of the rectus sheath and the anterolateral abdominal wall are commonly associated with injury to the inferior epigastric artery and the deep circumflex iliac artery, respectively. The diagnosis of spontaneously developed abdominal wall hematoma is sometimes delayed, due its clinical manifestations being similar to those of other causes of the acute abdomen. CT and angiography can be helpful in the diagnosis of the hematoma and the injured vessel. Herein, we report on a rare case of a spontaneously developed anterolateral abdominal wall hematoma treated with microcoil embolization of the left deep circumflex iliac artery
