2000년 이후 국내 한의학 암 관련 연구 동향 분석 - Latent Dirichlet Allocation 기반 토픽 모델링 및 연관어 네트워크 분석
배겨레 대한한방내과학회 2022 大韓韓方內科學會誌 Vol.43 No.6
Objectives: The aim of this study is to analyze cancer research trends in traditional Korean medical journals indexed in the Korea Citation Index since 2000. Methods: Cancer research papers published in traditional Korean medical journals were searched in databases from inception to October 2022. The numbers of publications by journal and by year were descriptively assessed. After natural language processing, topic modeling (based on Latent Dirichlet allocation) and keyword network analysis were conducted. Results: This research trend analysis involved 1,265 papers. Six topics were identified by topic modeling: case reports on symptom management, literature reviews, experiments on apoptosis, herbal extract treatments of breast carcinoma cell lines, anti-proliferative effects of herbal extracts, and anti-tumor effects. Keyword network analysis found that the effects of herbal medicine were assessed in clinical and experimental studies, while acupuncture was mainly mentioned in clinical reports. Conclusions: Cancer research papers in traditional Korean medical journals have contributed to evidence-based medicine. Further experimental studies are needed to elucidate the effects of on different hallmarks of cancer. Rigorous clinical studies are needed to support clinical guidelines.

Objectives : Retroperitoneal liposarcoma (RPS) is a rare solid tumor and has a high recurrence rate after surgery. New complementary and alternative medicine is required to manage symptoms of RPS and side effects of surgery, chemotherapy and radiation therapy. The purpose of this case report is to report two cases of reduction of side effects of surgery and radiation therapy in RPS treated with Wheel Balanced Therapy (WBT) which is a traditional Korean medicine treatment program for patients with cancer. Methods : Two patients with RPS were treated with WBT. Each of patients received personalized WBT schedule including acupuncture. The changes in clinical and laboratory findings were evaluated. Results : The symptoms of patients were improved after about 2 weeks hospitalization. Conclusions : This case report suggests that WBT has a potential to treat side effects of surgery and radiation therapy in patients with RPS. Further rigorous studies are necessary to investigate the therapeutic effects of WBT on RPS.

Objective: The purpose of this study is to report the case of a patient with recurring non-small cell lung carcinoma (NSCLC) taking Samchilchoongcho-Jung with Afatinib. Methods: An NSCLC patient diagnosed with multiple bone and pulmonary metastasis was taking Afatinib (20 mg/day) and suffering from stomatitis caused by the Afatinib. The patient was treated with Samchilchoongcho-Jung (1,500 mg/ day) for 3 months. The tumor size was measured with computed tomography, and laboratory findings, including tumor markers (CEA, Cyfra 21-1), were also followed up. Stomatitis was measured by a numeric rating scale, and adverse events were evaluated using the National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE), version 4.0. Results: After combined treatment, stable disease was shown on computed tomography. The tumor marker levels of CEA and Cyfra 21-1 were decreased, and the stomatitis significantly improved. NCI-CTCAE 4.0 showed no adverse events. Conclusion: This case study suggests that Samchilchoongcho-Jung may have a synergic effect, in conjunction with Afatinib, on the treatment of patients with recurring NSCLC.

Objective : The purpose of this study is to report the possibility of treatment of Cancer of Unknown Primary Site (CUPS) patient with Traditional Korean Medicine based Samchilchoongcho-jung and Gunchilgyebok-Jung following chemotherapy. Methods : The patient is a female, who was diagnosed with CUPS, suffering from left flank pain, abdominal discomfort, nausea, vomiting and fatigue after chemotherapy. The patient was treated with acupuncture, pharmacoacupuncture, moxibution, herbal medicine and enema used with herbal decoction for 15 days. The clinical outcomes were measured by numeric rating scale(NRS). Results : After treatment, left flank pain had disappeared and nausea and vomiting was decreased from NRS 4 to NRS 1 respectively. Fatigue was also improved. Conclusion : This case study suggests that Traditional Korean Medicine is effective for treatment of CUPS patients and can improve the quality of life.

Objective: The purpose of this study is to report the effect of Nobongsangki-Jeong on skin rash caused by Olmutinib. Methods: A female Non-Small cell lung carcinoma patient (Adenocarcinoma, Stage IV, Epidermal Growth Factor Receptor positive) suffered from skin rash due to the side effect of Olmutinib administration. She was treated with Nobongsangki-Jeong for the symptom management for 14 days. The clinical outcomes were measured by numeric rating scale (NRS) and National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE) version 4.03. Results: After treatment, skin rash was improved from NRS 5 to 1. Pruritus and pain of skin were improved from NCI-CTCAE grade 2 to 1. Conclusion: This case study suggests that Nobongsangki-Jeong may have the efficacy for the treatment of skin rash caused by Olmutinib.

Objectives: The aim of this study was to develop a standard instrument of pattern identification and evaluation for chemotherapy-induced peripheral neuropathy (CIPN).Methods: The advisory committee for this study was organized by 12 professors of traditional Korean medicine from the Korean Association of Traditional Oncology. The items and structure of the instrument were designed based on a review of previous publications. We revised the instrument in consultation with the advisory committee and received additional advice via email. Results: We divided the symptoms and signs of CIPN into four pattern identifications: wind arthralgia (風痺), cold arthralgia (寒痺), dampness arthralgia (濕痺), and arthralgia of the deficiency type (虛痺). We obtained the mean weights to reflect the standard deviations from each symptom of the four pattern identifications, which were scored on a 5-point scale by the advisor committee. After we obtained the answers to discrimination between variable symptoms (變症) and ordinary symptoms (素症) from the 12 experts, we gained the final weight from the combination of the ratio of pattern identification to the number of total answers of the advisory committee and the mean weight. Conclusions: The Instrument on Pattern Identification and Evaluation for CIPN was developed through a discussion between 12 experts. There was a limitation that the validity and reliability of this instrument have not been proven. However, the significance of this study was that it is the first Instrument on Pattern Identification and Evaluation aimed at assessing CIPN in traditional Korean medicine.
