Background and Objective: A novel multiple antigen simultaneous test using ELISA assay (GENEDIA-ELISA) has been developed. It includes two spotted spider mite (TSM) and Japanese Hop pollen (Hop J), which are currently common allergens in Korea but were not readily available for test in the past. The aim of this study was to validate GENEDIA-ELISA assay and to compare it with MAST-Immunoblot assay which was one of the commonly used in vitro tests in Korea. Method: One hundred eight patients were evaluated with skin prick tests, MAST-Immunoblot and GENEDIA-ELISA assay. Using skin prick test results (A/H ratio>1) as the gold standard, the sensitivity, specificity, positive predictive value, and negative predictive value of GENEDIA-ELISA (cut-off=class 2) and MAST-Immunoblot (cut-off=class 2) were examined. The results were analyzed using receiver operating characteristic (ROC) analysis. Result: The overall sensitivity of GENEDIA-ELISA was higher than that of MAST-Immunoblot (61.4% vs. 38.4%; P<0.05) while the overall specificity of each test was similar (88.5% vs. 93.9%). There was a higher correlation between GENEDIA-ELISA and skin prick tests than that obtained between MAST-Immunoblot and skin prick tests (0.47 vs. 0.39). In ROC analysis, the overall result of GENEDIA-ELISA yielded an area under the ROC curve (AUC) of 0.75, which indicated that this assay is more accurate test than MAST-Immunoblot (AUC=0.66). The sensitivities for TSM and Hop J on GENEDIA-ELISA were 83.3% and 83.3%, respectively, and AUC were 0.73 and 0.89, respectively. Conclusion: GENEDIA-ELISA assay may be a sensitive and acceptable tool of specific IgE against common inhalant allergens in Korea. (Korean J Asthma Allergy Clin Immunol 2007;27:26-32)

N/A The elderly patients with reflux esophagitis are less likely to feel or report their symptoms than the younger patients, even though they are more prone to complications such as bleeding, stricture or Barrett`s esophagus. In this study we tried to investigate risk factors in the elderly patients who had endoscopically diagnosed reflux esophagitis. Methods: Clinical data of six hundred and eighty seven patients with reflux esophagitis were reviewed. Results: Among the 687 patients with reflux esophagitis, two hundred and thirteen (31%) were 60 or more than 60 years old. The frequency of accompanying hiatal hernia in the elderly group (age≥60) was significantly higher than the younger group (age<60) (16.9%, 36/213 vs. 3.4%, 16/474, p<0.05). There was significant difference according age in the female group (0.9%, 1/114 vs. 22.7%, 22/97, p<0.05). The frequency of atropic gastritis was significantly higher in the elderly group than in the younger group , while other factors such as smoking, H. pylori & peptic ulcer disease were not different between the two groups. Conclusion: Among the risk factors of reflux esophagitis, the existence of hiatal hernia seems to be suggested more relevant to the development of this condition in the elderly group.(Kor ean J ournal of Gastrointestinal Motility 2002;8:139-145)

의료 관련 감염을 예방하기 위한 필수적 방법은 일관성이 있는 감염감시 시스템을 구축하고 효율적인 감시 통제를 수행하기 위해 신뢰할 수 있는 상황에 대해 진단을 향상시켜나가는 것이다. 또한 의료 종사자의 손과 의복 및 장비는 환자 관리 및 환경과 접촉하여 병원체가 오염되는 요인이다. 병원체를 가진 시설 표면(예: 침대 레일, 침대 옆 탁자), 음용수, 냉각탑용수, 내시경기구, 급식위생, 공기매개, 멸균검사, 내독소검사, 및 의료 장비를 포함한 의료 환경의 오염은 일반적으로 발생한다. 또한 이러한 감염원을 활동 감시를 통해 MLST, PFGE의 기법으로 역학 분석을 수행한다. 따라서 HAI 예방을 위한 환경 감시배양 검사는 환자의 안전과 감염원의 차단을 향상시킬 뿐만 아니라 국가의 감염관리 시스템을 통제하여 최적의 효율적인 감염관리 예방을 가능케 하고 국가의 감염관리 시스템의 안전을 향상시킨다. 결론적으로 감시배양 검사의 표준화를 통해 실효성 있는 감염관리체계에 이바지하고 감염관리 전문인력으로서의 전문성을 확보하고자 한다. 이를 통해 표준화 마련의 일차적인 목표는 의료관련 감염을 줄이고 국가적 의료관리 체계를 향상시키는데 있다. An essential measure to prevent healthcare-associated infections (HAI) is to develop a consistent system of surveillance, thereby promoting a reliable situation diagnosis to perform efficient control for the problem. Patient-to-patient transmission of pathogens within the hospital plays a substantial role in the epidemiology of HAIs. Contamination of healthcare environments commonly occurs, including facilities surfaces (e.g., bed rails, bedside tables), drinking water, cooling tower water, endoscopic instruments, food, airborne, endotoxin test, sterile test and medical equipment, with pathogenic organisms. In addition, epidemiological analysis is performed by multi locus sequence tying, pulsed-field gel electrophoresis for active surveillance. Therefore, an environmental surveillance culture test for prevention improves patient safety and blocks infection agents. Effective infection control and increased safety are possible by controlling the national infection control system. In conclusion, this study contributes to an effective infection control system through the standardization of active surveillance culture laboratory and secure expertise as infection control specialist. The primary objective of the standardization is to improve the safety of the nation’s healthcare system by reducing the rates of HAIs.

Mastocytosis is a rare disease characterized by pathologically increased mast cells in various tissues. Cutaneous mastocytosis, the most common manifestation of the mastocytosis, is confined to the skin without involvement of the other organs. Cutaneous mastocytosis is mostly found in children. It usually presents as symmetrically distributed red-brown macules or papules. Wheals and erythema may often develop with pruritus if mechanical irritations such as stroking (Darier`s sign), heat or sudden change of temperature are given to the skin lesions. Clinical symptoms include flushing, urticaria, angioedema, anaphylaxis, peptic ulcer disease, abdominal pain, diarrhea and bone pain caused by the release of mast cell mediators. We report a case of adult-onset cutaneous mastocytosis that presented as generalized urticaria developed after hot bath. (Korean J Asthma Allergy Clin Immunol 2005;25:328-332)

Background: T-bet is considered as a master transcriptional factor that appears to regulate lineage commitment of CD4+T cells in part by activating the TH1 cytokine IFN-V. As the IFN-V is also produced by CD8+ T cells, CD56+CD3-NK cells and CD56+CD3+ NKT cells, T-bet may also be expressed in these cells. However, expression and cellular sources of this factor are still unknown in asthmatics. Objective: Our aim was to investigate the expressions of T-bet in the peripheral blood mononuclear cells (PBMCs) according to presence of atopy and asthma. Method: PBMCs were obtained from nonatopic controls, atopic asthmatics and nonatopic asthmatics. Expressions of T-bet according to lymphocytes subtype were analyzed using three-color flow cytometry. 122 Result: Among the T-bet expressing lymphocytes, percentage of CD3+ T cells and CD56+CD3- NK cells were similar in nonatopic controls. CD3+ T cells were prominent source of T-bet compared to CD56+CD3- NK cells in nonatopic asthmatics (P<0.05). CD56+CD3- NK cells were prominent sources of T-bet compared to CD3+ T cells in atopic asthmatics (P<0.05). The proportion of T-bet positive CD56+CD3+ NKT cells of both controls and nonatopic asthmatics had a high tendency compared to the atopic asthmatics. Conclusion: This result provides the first evidence about cellular sources of T-bet in PBMCs of asthmatics. The main source of T-bet is different according to presence of atopy. 우리나라 성인 남성에서 흡연량과 공복혈당장애 또는 제2형 당뇨병 발생과의 연관성

Objectives: To investigate whether smoking and the smoking status are predictors of the incident impaired fasting glucose (IFG) or type 2 diabetes in Korean men. Methods: A cohort of 1,717 Korean men without IFG or diabetes, who underwent annual periodic health examinations for 4 years (2002-2006), were retrospectively investigated. IFG and diabetes were defined as a serum fasting glucose concentration of 100-125 mg/dL and more than 126 mg/dL, respectively. Cox's proportional hazards model was used to evaluate the association between smoking and development of IFG or type 2 diabetes. Results: A total of 558 cases (32.5%) of incident IFG and 50 cases (2.9%) of diabetes occurred. After controlling for the potential predictors of diabetes, the relative risk for IFG, compared with the never smokers, was 1.02 (95% CI=0.88 to 1.19) for the ever-smokers, 0.96 (95% CI=0.79-1.16) for those who smoked 1-9 cigarettes/d, 1.15 (95% CI=1.01 to 1.30) for those who smoked 10-19 cigarettes/d, and 1.31 (95% CI=1.10 to 1.57) for those who smoked 20 or more cigarettes/d (the P value for the current smokers was only p<0.002). The respective multivariate adjusted relative risks for type 2 diabetes, compared with the never-smokers, were 1.07 (95% CI=0.64 to 1.92), 1.47 (95% CI=0.71 to 3.04), 1.84 (95% CI=0.92-3.04), and 1.87 (95% CI=1.13-3.67), respectively (the P value for the current smokers was only p=0.004). Conclusions: The smoking status and the number of cigarettes smoked daily are associated with an increased risk for developing IFG or type 2 diabetes in Korean men.

We have researched a character of BIM(Building Information Modeling) and GIS(Geographic Information System) data for integrated utilization of urban spatial data and building data for $CO_2$ reduction. We consider literature on integrated utilization of BIM and GIS and measure method of $CO_2$ in urban spatial unit. Among them without distortion of standard data structure, derive method how to install between BIM and GIS standard data. Rhino & Grasshopper is utilization tools of DB platform. The compatibility with existing BIM and GIS data format, graphic and numeric results can be output at the same time. And using the existing ARCHSIM, it can be easily combined for building energy analysis program. Based on BIM and GIS data to run an energy analysis target on Majang-dong, it could be poly synthetically derived the value information and graphic information.

Recently, the demand of the Intelligent Transportation System(ITS) has been increased to a large extent, and a real-time traffic information service based on the internet system became very important. When ITS companies carry out real-time traffic services, they find some traffic data missing, and use the conventional method of reconstructing missing values by calculating average time trend. However, the method is found unsatisfactory, so that we develop a new method based the spatial and spatial-temporal models. A cross-validation technique shows that the spatial-temporal model outperforms the others.
