        흉부손상의 임상적 관찰

        A chinical analysis was performed on 383 ases of hest injurjes eperienced at Department of thoraci Surgery, Seoul National University Hospital during 21 year period From 1957 to 1977. Of 383 patients o hest injuries, 209 cases were result from nonpenetrating injuries whereas 175 were from penetrating injuries, and there were 258 cases of hemothorax or/and pneumothorax, 162 of rib fracture, 33 of foreign body, 26 of clavicle frcture, 26 of lung contusion, 17 of diaphragmati laceration, 14 of hemoperiardium, 14 of flail chest and others. Stab wound was the most common in penetrating injuries and followed by gunshot and shell fragments. The majority of nonpenetrating chest injury paiens were traffi accident vitims and falls accounted for the next largest group of accidents. Chest injuries were frequently encountered in the age group between 16 and 50 years, and 321 patients were male comparing to 62 of female. In blunt hest injuries the patients with five or more rib fractures had a 85 per ent incidence of intrathoracic injury and 19 per cent had an intraabdominal organ damage, whereas those with four or less rib fractures had a 69 per cent and a 6 per cent incidence respectively. The principal associated injuries were cerebral contusion on 19 cases, renal contusion on 10, liver laceration on 7, peripheral vessel laceration on 5, spleen laceration on 3 and extremity fracture on 18 patients. The principles of therapy for early complications of chest trauma were rapid reexpansion of the lungs by thoracentesis (46 cases) and closed thoracotomy (125 cases) but open thoracotomy had to be done on 90 cases (23.5%) because of massive bleeding or intrapleural hematoma, foreign body, cardiac injury, diaphragmatic laceration and bronhial rupture. The over all mortality was 2.87 per cent (11 among 383 cases), 8 cases were from penetrating injuries and 3 from nonpenetrating injuries.

        국소성 흉막 섬유성 종양 -1례보고-

        A 44-year-old man was transferred to our department for mediastinal mass. He had suffered from only an easily fatiguable condition for 1 month. A physical examination and laboratory finding of the patient disclosed no abnormality. A chest radiograph showed a soft tissue tumor in the posterior mediastinum. It was well circumscribed and ovoid. Invasions to adjacent organs were not seen. Therefore it was though the 5th intercostal space. The tumor mass was attached to the visceral pleura of the right upper lobe by a pedicle and this pedunculated tumor laid entirely within the pleural cavity. Excision of the tumor which measured 7$\times$7$\times$3cm was done easily. Pathologic studies confirmed the diagnosis of localized fibrous tumor of the pleura. Localized fibrous tumor of the pleura is rare, This tumor along ith the evidence from ultrastructural and immunohistochemical studies has led most researchers to conclude that localized fibrous tumor is not of the mesothelial origin but arises in the submesothelial connective tissue.

        Galantamine 사용 후 발생한 QT간격의 연장 1예

        Among the Acetylcholinesterase inhibitors as used for Alzheimer's disease treatment, Galantamine has been recently developed and widely used owing to proven its clinical efficacy and safety. However, it has reported that prolonged QT interval, which can lead to ventricular arrythimias such as Torsade de points, has developed in Galantamine-treated patients. A 74-year-old female Alzheimer's patient been treated with galantamine for 8 months visited the hospital complaining about frequent dizziness. ECG monitor was performed promptly, it was informed that the prolonged QTc interval was increased 450 ms to 486 ms. So, we made her stop taking the galantamine, and after that QTc interval has normalized to 406 ms. In this article, we reported the first case on prolonged QT interval associated with galantamine in Korea.

        Melatonin이 MDPC-23 세포의 분화에 미치는 영향

        Melatonin is the major hormone released from the pineal gland and regulates a variety of physiological and pathophysiological processes. According to the recent studies the melatonin plays an important role in regulation of bone growth. The purpose of this study was to determine whether melatonin promotes the cell differentiation and nodules formation in MDPC-23 pre-odontoblast cell line. MDPC-23 cells were cultured for up to 15 days in growth media containing differentiation medium with melatonin or without melatonin. Cultures were stained with Alizarin-S. The expression of the mRNAs for DSPP, OC, ALP and NFI-C were analyzed by RT-PCR. The results were as follows. Cultures containing melatonin at day 15 showed extensive mineralization as compared with control cultures. Melatonin increased the expression of DSPP and OC mRNAs in MDPC-23 cells in a concentration- dependent manner. However, melatonin did not changed ALP expression. Melatonin markedly decreased mRNA expression of NFI-C in early stage cultures as compared with control cultures. These results demonstrated that melatonin is capable of promoting MDPC-23 cells differentiation and mineralization and suggested that melatonin may play an important role in dentin formation. Melatonin은 솔방울샘(pineal gland)에서 분비되는 주요 호르몬으로 신체의 여러 생리학적 및 병태생리학 적 조절에 관여한다. 최근의 연구들에 따르면 melatonin은 뼈모세포 분화와 바탕질의 석회화를 촉진시켜 골성장 조절에 있어 중요한 역할을 하는 것으로 알려졌다. 본 연구에서는 상아질모세포 전구세포인 MDPC-23 세포를 이용하여 melatonin이 상아질모세포 분화를 촉진시 키는 지를 확인하였다. MDPC-23 세포에 분화 배양액과 melatonin을 첨가하여 분화를 유도시킨 후, RT-PCR을 이 용하여 dentin sialophosphoprotein (DSPP), osteocalcin (OC), alkaline phosphatase (ALP)와 nuclear factor I-C (NFI-C) 의 발현 변화를 분석하여 다음과 같은 결과들을 얻었다. 석회화 결절의 형성은 melatonin의 농도가 증가할수록 촉진되었으며, 특히 분화 15일째에 대조군에 비해 석회 화 결절의 형성이 강하게 관찰되었다. DSPP와 OC의 발현은 melatonin의 농도증가에 따라 증가된 반면에, ALP의 발현은 melatonin의 농도에 따라 변화가 없었다. NFI-C의 발현은 melatonin의 농도증가에 따라 배양 초기에 대조 군에 비해 크게 감소하였다. 위의 결과들을 종합하면 melatonin은 상아질모세포 전구세포인 MDPC-23 세포의 분화와 석회화를 촉진시키며, 상아질 형성에 중요한 역할을 하는 것으로 생각된다.

      • 회사정리제도로서의 기업개선작업과 법정정리제도의 비교 연구

        본 연구에서는 전통적인 부실기업 정리방법인 법정관리·화의 등 법적정리 절차와 채권금융기관과 부실징후를 보이는 기업이 긴밀히 합의하여 기업의 정리재건을 효율적으로 도모하기 위하여 시행된 사적 부실기업 정리절차인 workout(기업개선작업)이 증권시장에서 어떻게 받아들여지는지를 해당기업의 산업별 주가지수를 이용한 시장모형(market model)에 의하여 구한 초과수익률을 비교 분석함으로써 알아보았다. 그 결과 법적정리 신청기업의 법적정리 신청기업의 법적정리 신청을 전후한 누적평균초과 수익률과 workout기업 선정시의 누적평균초과 수익률이 모두 유의적인(-)의 반응을 보였다. 그러나 workout기업의 workout대상 선정시의 누적초과수익률이 법적정리 신청기업의 법적정리 신청시의 그것에 비하여 유의적인 (+)의 차이를 보여 기업의 정리재건제도로서 workout이 법적정리절차보다 다소 호의적으로 받아들여지는 것으로 나타났다. This paper empirically examine comparative efficiency between workout and legal procedures in company reconstruction by event study methodology using market model. The empirical results of this study shows that the cumulative abnormal average returns of workout enterprises is significantly negative in the event date of becoming workout object and that the cumulative abnormal average returns of enterprise in the law process is significantly negative in the event date of applying for legal management. Comparing the cumulative abnormal average returns of workout enterprises in the event date of becoming workout object to those of enterprises in the law process, we find that the stock price's response of workout enterprises is significantly positive difference to those of enterprises in the law process.

        법랑모세포 분화와 성숙과정에서 OD314의 발현

        법랑모세포는 법랑질을 형성하고 유지하는 세포로, 법랑질의 유기기질을 분비하고 법랑질 석회화 과정에도 관여한다. 치아 발생과정에서 법랑모세포의 분화는 순차적인 상피-간엽 상호작용에 의하여 조절되나, 분화나 성숙과정의 정확한 기전은 아직까지 잘 알려져 있지 않다. 최근에 상아모세포에서 처음 발견된 OD314가 치아 발생과정에서 상아질을 형성하는 상아모세포 뿐 아니라 법랑모세포에도 발현된다고 하였다. 이에 본 연구에서는 생쥐 하악 전치의 다양한 시기의 법랑모세포를 이용하여, 형태학적 분석과 in-situ hybridization에 의한 OD314 mRNA의 발현 그리고 OD314 항체를 이용한 면역조직화학적 분석을 통하여 OD314 유전자의 법랑 모세포 분화와 성숙과정에서의 역할을 연구하여 다음과 같은 결과를 얻었다. 1. 형태학적으로 법랑모세포는 분화 단계에 따라 분비 전단계 법랑모세포, 분비기 법랑모세포, 성숙기의 평탄끝 법랑모세포와 성숙기의 주름끝 법랑모세포로 구분되었다. 2. OD314 mRNA는 분비기의 법랑모세포에서부터 발현되기 시작하여 법랑모세포가 성숙해갈 수록 그 발현이 증가하였다. 3. OD314 단백질은 분비 전단계의 법랑모세포에서는 발현되지 않고, 분비기의 법랑모세포에서는 세포질에 전체적으로 발현되었다. 성숙기의 평탄끝 법랑모세포와 주름끝 법랑모세포에서는 세포의 근심과 원심끝단에 OD314 단백질이 강하게 발현되었다. 이상의 결과를 종합하여 OD314는 법랑모세포의 분화와 성숙과정에서 세포질 내부에서 특징적인 역할을 하는 것으로 사료된다. Ameloblasts are responsible for the formation and maintenance of enamel which is an epithelially derived protective covering for teeth. Ameloblast differentiation is controlled by sequential epithelial-mesenchymal interactions. However, little is known about the differentiation and maturation mechanisms. OD314 was firstly identifled from odontoblasts by subtraction between odontoblast/pulp cells and osteoblast/dental papilla cells, even though OD314 protein was also expressed in ameloblast during tooth formation. In this study, to better understand the biologcal function of OD314 during amelogenesis, we examined expression of the OD314 mRNA and protein in various stages of ameloblast differentiation using in-situ hybridization and immunohistochemistry. The results were as follows : 1. The ameloblast showed 4 main morphological and functional stages referred to as the presecretory, secretory, smooth-ended, and ruffle-ended. 2. ○D314 mRNA was expressed in secretory ameloblast and increased according to the maturation of the cells. 3. OD314 protein was not expressed in presecretory ameloblast but expressed in secretory ameloblast and maturative ameloblast. OD314 protein was distributed in entire cytoplasm of secretory ameloblast. However, OD314 was localized at the proxiamal and distal portion of the cytoplasm of smooth- ended and ruffle-ended ameloblast. These results suggest that ○D314 may play important roles in the ameloblast differentiation and maturation.
