        TRISO 피복 입자에서 증착 조건이 탄화규소층의 특성에 미치는 영향

        TRISO coatings on $ZrO_{2}$ surrogate kernels were conducted by a fluidized-bed chemical vapor deposition (FBCVD) method. Effects of the deposition temperature and the gas flow rate on the properties of SiC layer were investigated in the TRISO-coated particles. Deposition rate of the SiC layer decreased as the deposition temperature increased in the temperature range of $1460^{\circ}-1550^{\circ}C$. At the deposition temperature of $1550^{\circ}C$ the SiC layer contained an excess carbon, whereas the SiC layers had stoichiometric compositions at $1460^{\circ}C\;and\;1500^{\circ}C$. Hardness and elastic modulus measured by a nanoindentation method were the highest in the SiC layer deposited at $1500^{\circ}C$. The SiC layer deposited at the gas flow rate of 4000 sccm exhibited a high porosity and contained large pores more than $1{\mu}m$, being due to a violent spouting of particles. On the other hand, the SiC layer deposited at 2500 sccm revealed the lowest porosity.

      • 슬관절 주변에 발생한 거대세포종의 치료 - 고속 바(High-Speed Burr)와 골 시멘트를 이용한 병소 내 절제술 -

        목적: 거대세포종은 대부분 대퇴골 원위부 및 경골 근위부에 주로 발생하며, 병소 내 소파술에서 광범위 절제술까지 다양한 수술 방법들이 시행되고있다. 저자들은 수술이 어렵고 재발율이 높은 슬관절 부위의 거대세포종 환자에서 재발율을 포함한 치료 성적을 분석하고, 재발율과 관련된 인자를 규명하고자 하였다. 대상 및 방법: 병리학적으로 확진 되고, 고속 바(high-speed burring)와 골 시멘트를 이용한 병소내 소파술을 시행 한 41예의 슬관절 주위 거대세포종 환자를 대상으로 하였다. 재발율과 기능적 평가를 통한 치료 성적을 분석하였으며, 성별, 연령, 종양의 위치, 크기, 연골 하골의 침범과 관절 내 침범 유무 그리고 Campanacci 분류에 따른 인자들과 재발율과의 상관관계를 통계적으로 분석하였다. 평균 추시 기간은 50(12~122) 개월이었다. 결과: 최종 추시 상 재발율은 17%였고, 재발시기는 평균 술 후 10개월이었다. Musculoskeletal Tumor Society (MSTS) Grading System에 의한 술 후의 기능 평가점수는 평균 27.8(93%)로 78%에서 만족스러운 기능을 보였다. 성별, 연령, 종양의 위치, 크기, 연골 하침범, 관절 내 침범, Campanacci 분류 등은 종양의 국소 재발과 통계적으로 유의한 상관 관계를 보이지 않았다. 결론: 슬관절 주변 거대세포종의 재발과 관련된 인자를 찾지는 못하였다. 수술적 치료로서 고속 바를 이용한 세심한 소파술과 골시멘트 충진술을 이용한 국소 절제술 만으로도 만족할 만한 결과 및 재발율을 얻을 수 있었다. Purpose : Distal femur and proximal tibia are the common sites affected by giant cell tumor of bone. There are a variety of treatment modality including wide excision and intralesional curettage. We evaluated the local recurrence rates and the post-operative functional scores of giant cell tumors around knee joint and investigated the identification of possible prognostic factors for recurrence. Materials and Methods: We reviewed 41 patients pathologically confirmed as giant cell tumors around knee joint that have undergone intralesional curettage using high-speed burr and methylmethacrylate. We evaluated the recurrence rate and post-surgical functional score and possible prognostic factors for recurrence, such as, gender, age, tumor location, size, subchondral invasion, intra-articular invasion and the Campanacci Grades. Mean follow up period was 50 (12-122) months. Results: The recurrence rate was 17% and mean recurrence onset was 10 months postoperatively. According to Musculoskeletal Tumor Society (MSTS) functional evaluation system, the average score was 27.8(93%) and 78% had excellent function. According to our study, suspected prognostic factors revealed not significant for recurrence. Conclusion: We found no significant recurrence related factors. Intralesional excision with high-sped burring and PMMA provides a low recurrence rate, similar to others in the literature, and good functional scores.

        일체형 비구면 인공수정체 Tecnis? ZCB00과 삼체형 비구면 인공수정체 Tecnis? ZA9003의 앞방 깊이 변화 및 장기 안정성 비교

        Purpose: To evaluate the postoperative outcome of Tecnis? ZCB00 1-Piece acrylic intraocular lens (IOL), the new single piece, aspheric IOL until 12 months. Methods: Sixty eyes undergone cataract surgery using two different IOLs; The Tecnis? ZCB00 1-Piece Acrylic IOL (30 eyes) and the Tecnis? ZA9003 3-Piece Acrylic IOL (30 eyes). Cataract surgery was performed in the same period. All complicated cases were excluded. Best corrected visual acuity (BCVA), refractive error, photopic and mesopic contrast sensitivity, total high order aberration, spherical aberration and anterior chamber depth were measured preoperatively and at 1, 6 and 12 months after surgery. Results: There were no statistically significant differences between two groups in BCVA, refractive error, total high order aberration and spherical aberration all the time after surgery. Both groups have negative ocular spherical aberration until 12 months after surgery. 1-piece and 3-piece groups had different tendency in axial movement. But there were no statistically significant differences in anterior chamber depth measured at 1 week, 1, 6 and 12 months after surgery. There was no statistically significant difference in contrast sensitivity at 1, 6 and 12 months after surgery. Conclusions: The Tecnis? ZCB00 1-Piece acrylic intraocular lens (IOL) showed comparable clinical outcomes to the Tecnis? ZA9003 3-Piece acrylic intraocular lens (IOL) until 12 months after cataract surgery. 1-piece and 3-piece IOL group had different tendency in axial movement. However, refraction remained stable during 12 months after surgery. Mostly, both IOL groups had negative ocular spherical aberration during whole follow-up period.         감초 물 추출물 및 Glycyrrhizin이 인체 간 Microsome에서 Cytochrome P450 약물대사효소에 미치는 영향

        Objective : The aim of present study is to evaluate the inhibitory potential of licorice extract and glycyrrhizin on cytochrome P450(CYP) in human liver microsomes. Methods : Using human liver microsomes, water extract of licorice and glycyrrhizin as an inhibitor were co-incubated with each probe drug representing selective CYP isoform activity. We measured relative metabolic activity in incubation condition compared to that with no extract of licorice using HPLC system. Results : Both water extracts of licorice and glycyrrhizin showed inhibitory effect on CYP-catalyzed reactions. CYP2C19 $(IC_{50}=126.7{\mu}g/ml)$ is most potently inhibited by water extract than other tested CYP isoforms$(IC_{50}>450{\mu}g/ml)$, but glycyrrhizin exhibited potent inhibition on CYP1A2$(IC_{50}=106.9{\mu}g/ml)$ followed by CYP2C9 and CYP2D6. Conclusion: These results indicate that water extract of licorice and glycyrrhizin have inhibitory potential on CYP-catalyzed reaction in human liver microsomes. But the mechanism of inhibition was slightly different between them Water extract of licorice mainly inhibited CYP2C19, and glycyrrhizin primarily inhibited CYP1A2. The inhibition by water extract of licorice and glycyrrhizin on CYP isoforms may cause drug interaction with co-administered drug leading to toxicity or treatment failure.

        각막 지형도를 이용한 각막 구면수차의 산출

        Purpose: To formulate an equation to estimate corneal spherical aberration using Orbscan (Bausch & Lomb Surgical, Rochester, NY, USA) parameters. Methods: The study was carried out retrospectively. The participants were 76 eyes of 76 senile cataract patients with the mean age of 57.37 ± 17.63 years. Both Orbscan and KR-1W (Topcon Corp, Tokyo, Japan) were taken as preoperative examinations. Correlation analysis between various parameters from Orbscan and corneal spherical aberrations for a 6 mm pupil by KR-1W was performed. And multivariable linear regression was performed with the significantly correlated Orbscan parameters from the correlation analysis. Results: The mean corneal spherical aberration from KR-1W system was 0.25 ± 0.08 μm. As a result of the multivariable linear regression, we could generate following equations. If the Q-value was available, estimated corneal spherical aberration = 0.389 × Q-value + (0.022 × Axial power 3 mm) - 0.633 (R2 = 0.436). If the Q-value was not available, estimated corneal spherical aberration = 0.184 × (Mean power 5 mm – Mean power 3 mm) + (0.02 × Axial power 3 mm) - 0.563 (R2 = 0.429). By using the equations, 93.4-94.7% of subjects were in the error range of 0.10 μm. Conclusions: Even when equipped with Orbscan only, an appropriate aspheric intraocular lens can be selected using the estimated corneal apherical aberration by the equations.

        다중 슬릿 즉발감마선 카메라를 위한 이중모드 신호처리 모듈 개발

        양성자 치료 시 양성자 빔의 특성을 이용하여 치료 부위에 국부적인 선량을 부여하고 정상조직에 불필요한 선량을 줄이기 위해서는 인체 내 양성자 빔의 비정을 실시간으로 확인하는 것이 중요하다. 이를 위해 본 연구팀은 24개의 섬광검출기 배열 및 24채널의 신호 처리 시스템으로 구성된 즉발감마선 카메라 모듈을 개발하고 있다. 본 연구에서는 다채널의 섬광 검출기 신호를 처리하기 위하여 이중모드 다채널 신호 처리 모듈을 개발하여 그 성능을 평가해보았다. 성능을 평가한 결과 에너지 교정 모드를 통해 다채널의 섬광검출기에 대하여 동시에 에너지 교정이 가능함을 확인하였고, 이를 통하여 정확하게 3 MeV에 해당하는 측정 하한 값을 결정할 수 있었다. 고속 데이터 획득 모드를 통해 45 MeV 양성자 빔에서 발생한 즉발감마선 분포를 측정한 결과 $3{\times}10^9$개의 양성자 빔에서도 양성자 선량 분포와 유사한 결과를 얻을 수 있었고, 빔 비정을 평가한 결과 $17.13{\pm}0.76mm$로 EBT film을 통하여 측정한 비정인 16.15 mm와 굉장히 밀접한 관련이 있음을 확인하였다. In proton therapy, in vivo proton beam range verification is very important to deliver conformal dose to the target volume and minimize unnecessary dose to normal tissue. For this purpose, a multi-slit prompt-gamma camera module made of 24 scintillation detectors and 24-channel signal processing system is under development. In the present study, we have developed and tested a dual-mode signal processing system, which can operate in the energy calibration mode and the fast data acquisition mode, to process the signals from the 24 scintillation detectors. As a result of performance test, using the energy calibration mode, we were able to perform energy calibration for the 24 scintillation detectors at the same time and determine the discrimination levels for the detector channels. Further, using the fast data acquisition mode, we were able to measure a prompt-gamma distribution induced by a 45 MeV proton beam. The measured prompt gamma distribution was found similar to the proton dose distribution at the distal fall-off region, and the estimated beam range was $17.13{\pm}0.76mm$, which is close to the proton beam range of 16.15 mm measured by an EBT film.

        수종의 구취처치법의 구치감소효과에 관한 연구

        This study was performed to investigate the effect of three modalities commonly used for reduction of oral malodor and to review the contributing factors for halitosis. For this study, 45 subjects of good general health in their third decades were selected, and they were divided into three groups by each modality, that is, tongue scraping, $ZnCl_2$ mouth rinse, and periodontal treatment. Subjective sense of halitosis, diets, Oral hygiene index based on tooth brushing and tongue brushing habit, resting salivary flow rate, gingival index, degree of tongue coating were recored in the Halito-Chart designed by the author. Oral malodor converted from volatile sulfur compound was measured by the Halimeter$^{(R)}$(Interscan Co., USA), and the correlation between the oral factors and pre-treatment halimeter score were analysed. Data obtained were analysed by the SPSS windows program and the results of this study were as follows : 1. There were no significant correlation between oral factors such as oral hygiene index, salivary flow rate, gingival index, degree of tongue coating and pre-treatment halimeter score. 2. Difference between pre- and post-treatment halimeter score by the three modalities were significant, but correlation between pre- and post-treatment score within group was most significant in the periodontal treatment group followed by the tongue scraping group and lowest in the $ZnCl_2$ mouth rinse group. 3. Reduction of halimeter score was significant only in subjects with relatively more tongue coating in the tongue scraping group, but in the $ZnCl_2$ mouth rinse group and in the periodontal treatment group, the modality applied to each group had same significant effects without regard to the degree of oral hygiene index or gingival index. 4. Men had more tongue coating than women, and reduction of halimeter score was significant in men, but not in women.

      • 저 악성도 연골 육종에서 발생한 미분화 연골육종 - 1례 보고 -

        미분화 연골육종은 연골육종 중 가장 악성도가 높은 종양으로서, 연골육종의 약 10%에서 발생되는 것으로 알려져 있다. 미분화 연골 육종에 의해 발생된 악성종양은 방사선 및 화학요법에 반응을 하지 않아 광범위 종양 절제술만이 근치적 치료법이다. 본 연구는 근위 대퇴골에 발생한 저악성도 연골육종을 보존적으로 수술하고 추시 관찰 중, 골육종으로 미분화한 연골육종 1례를 보고하고자 한다. Dedifferentiated chondrosarcoma is a most highly malignant variant of chondrosarcoma and approximately 10% of chondrosarcomas dedifferentiated into more anaplastic lesions. In addition to the surgical treatment, adjuvant chemotherapy or radiation therapy has been used but with little success. Thus, the primary treatment remains wide excision and chemotherapy or radiation therapy can be used for palliation. This article shows the case of a dediffderentiated chondrosarcoma occuring in proximal femur after conservative surgical treatment of a low grade chondrosarcoma.
