        <목적> 감시림프절 생검은 불필요한 림프절 곽청술을 줄여 이환율을 감소시키고자 많은 암에서 연구되어왔다. 그러나 위암에서 감시림프절 생검의 타당성에 대하여는 아직 확립된 바가 없다. 이에 저자들은 위암에서 감시림프절 생검의 실현가능성과 정확도를 평가하고자 본 연구를 수행하였다. <대상 및 방법> 2002년 3월부터 장막층 침범이 없고 단일 병변인 위암으로 근치적 위절제술을 시행 받은 환자 중 56예에서 감시림프절 생검이 시행되었다. indocyanine

        자궁내막암 두 가지 형태의 임상적 특성

        N/A Objective : The endometrial cancer can be divided into two large groups on the base of carcinogenesis according to the pathologic findings. The hormone induced cancer group(type 1) represents the cancers developed under the effects of unopposed estrogen. And the other group(type 2) can be developed by the mechanisms of divergent variable events such as mutations or gene deletions. These two different types show definite differences in clinical characteristics. Methods : Clinical features of the 43 patients who were diagnosed and were treated as endometrial cancer in Gynecologic department of Ajou University Hospital from June 1994 to November 2000 were evaluated according to the pathologic findings. 22 patients of type 1 and 17 patients of type 2 were classified and analysed statistically. Results : Age, cell types, grade, cancer stage, endometrial thickness, invasion depth of uterine walls were definitely different with statistic significance. The lymphovascular space invasion, the number of lymph node metastasis, pap smear of cervix, adnexal involvement, diabetes, hypertension, CA125, gravida, height were not different between two groups statistically in correlations. BMI shown marked difference but had no statistical significance. Conclusion : The classification of endometrial cancer into the two types is very significant because it can allow avoidance of preceding risk factors and can provide the alteration of treatment tendency and the expectation of benign or malignant prognosis, clinically.

        LED 구동 IC를 위한 레벨 시프터 방식의 전하펌프 회로 설계

        In this paper, we designed a charge pump circuit using level shifter for LED driver IC. The designed circuit makes the 15 V output voltage from the 5 V input in condition of 50 kHz switching frequency. The prototype chip which include the proposed charge pump circuit and its several internal sub-blocks such as oscillator, level shifter was fabricated using a 0.35 um 20 V BCD process technology. The size of the fabricated prototype chip is 2,350 um ${\times}$ 2,350 um. We examined performances of the fabricated chip and compared its measured results with SPICE simulation data.

        The supply of oxygen for aeration is the largest energy consumer at activated sludge wastewater treatment plants. Replacement of less efficient aeration systems with fine pore aeration devices can save up to 50 percent of aeration energy costs. The purpose of this study was the diagnosis and evaluation of a domestic wastewater aeration system by the off-gas method which had been studied by US EPA and ASCE. For this study, an off-gas analyzer and its hood were made to collect off-gas. Also, a vacuum pump was connected to the analyzer to make suction of off-gas. Experiments were conducted at a domestic activated sludge wastewater treatment plant which had a single spiral roll aeration system installed with P.E tube diffuser. Data on OTE(f), SOTE(pw), OUR, and air flow rate were obtained from these experiments. In case of replacing an aeration system, it is recommended that it should be replaced with perforated membrane disc or ceramic disc fine bubble diffusers installed in a full floor coverage or grid pattern.

        공정변화에 따른 LDO 레귤레이터의 특성 분석

        본 논문에서는 LDO 레귤레이터의 공정변화에 따른 특성변화를 1 ㎛ 20 V 고 전압 CMOS 공정을 사용하여 시뮬레이션 하였다. 공정변화에 따른 3종류의 SPICE 파라미터(문턱전압과 실효채널길이가 평균적인 Typ(typical), 평균 이하인 FF(fast), 평균 이상인 SS(slow) 파라미터)를 LDO 레귤레이터 시뮬레이션에 활용하였다. 시뮬레이션 결과,SS 파라미터 사용의 경우, 라인레귤레이션이 3.6 ㎷/V, 부하 레귤레이션이 0.4 ㎷/㎃, 부하전류 변화에 따른 출력전압이 안정화되는 시간이 평균 0.86 ㎲였다. 그리고 Typ 파라미터 사용의 경우, 라인 레귤레이션이 4.2 ㎷/V, 부하 레귤레이션이 0.44 ㎷/㎃, 부하전류 변화에 따른 출력전압이 안정화되는 시간이 평균 0.62 ㎲였다. 마지막으로 FF 파라미터 사용의경우 라인 레귤레이션이 7.0 ㎷/V, 부하 레귤레이션이 0.56 ㎷/㎃, 부하전류 변화에 따른 출력전압이 안정화되는 시간이 평균 0.27 ㎲였다. 향후, 이러한 공정변화에 따른 회로 특성의 변화를 고려한 효율적 회로설계가 필요할 것으로 사료된다. In this paper, we have examined electrical characteristics of LDO regulator according to the process variation using a 1 ㎛ 20 V high voltage CMOS process. The electrical analysis of LDO regulator have been performed with three kind of SPICE parameter sets (Typ : typical, FF : fast, SS : slow) by process variation which cause change of SPICE parameter such as threshold voltage and effective channel length of MOS devices. From simulation results, we confirmed that in case of SS type SPICE parameter set, the LDO regulator has 3.6 ㎷/V line regulation, 0.4 ㎷/㎃ load regulation and 0.86 ㎲ output voltage settling time. And in case of Typ type SPICE parameter set, the LDO regulatorhas 4.2 ㎷/V line regulation, 0.44 ㎷/㎃ load regulation and0.62 ㎲ output voltage settling time. Finally, in the FF type SPICE parameter set, the LDO regulator has 7.0 ㎷/V line regulation, 0.56 ㎷/㎃ load regulation and 0.27 ㎲ output voltage settling time.

        농촌지역의 법정-기간내 출생신고율과 신고된 생년월일의 정확도

        우리 나라의 최근 농촌 지역에서의 출생 신고의 법정 기간내 신고율과 출생 신고의 정확도를 알아보기 위해 경북 군위군에 거주하는 모든 유배우 가임 여성 4,014명을 1985년 4월 1일부터 1987년 3월 31일까지 2년간 면보건요원들이 추적 조사하여 찾아낸 모든 출생아 766명의 출생일자와 1987년 9월 30일자로 면사무소의 주민 등록 대장을 열람하여 얻은 동기간내 출생한 것으로 신고된 944건의 출생 신고 기록을 대조하였다. 추적 조사한 766명중 576명(75.2%)이 출생후 6개월 이내에 신고된 것으로 확인되었으며 96명 (12.5%)은 출생신고를 하였다고 진술하였으나 기록이 없었다. 추적 조사에 등록되어 있고 생후 6개월 이내에 출생신고가 확인된 576명의 법정 신고 기간인 30일 이래 신고율은 61.3%였다. 법정 기간내 신고율은 산모의 연령이 20세 미만이거나 35세 이상인 경우, 교육 수준은 국민학교 졸업자에서 그리고 출산 순위가 늦을수록 낮았다. 출생 월별 신고율은 10월부터 3월 시아의 출생아가 4월에서 9월 사이의 농번기의 출생아에 비해 높았다. 신생아기에 사망한 7명은 모두 출생 신고를 하지 않았다. 추적 조사에 등록되어 있고 6개월 이내에 출생 신고가 확인된 576명의 실제 출생 연월일과 주민등록표의 출생 연월일의 일치율은 78.0%였고 6.8%는 실제 출생일보다 더 이른 날짜로 신고되어 있고 15.3%는 더 늦은 날짜로 신고되어 있었다. 일치율은 산모의 연령이 35세 이상인 경우에 54.5%로 그리고 출산 순위가 4순위 이상일 때 56.3%로 특별히 낮았다. 또 산모의 학력이 높을수록 일치율이 높았으며, 남아가 여아보다 높았다. 3월에 출생한 경우에 실제 출생일보다 이른 날짜로 신고하는 율이 17.4%로 다른 달에 비해 높았고 이들은 대부분이 음력으로 신고하였는데 이것은 취학 연령을 고려한 것으로 생각된다. 최근 우리 나라 농촌 지역의 법정 기간내 신고율 및 신고의 정화도는 향상되고 있으나 아직도 선진국에 비해 낮은 편이며 출생 신고 자료에 의한 영아 사망률은 매우 부정확한 실정이다. 법정 기간내 신고율과 신고 내용의 정확도를 높히기 위해 분만 개조를 한 의료인으로 하여금 부모의 주소지로 직접 출생 신고를 하도록 하고 비의료인에 의한 가정 분만의 경우에는 현행 신고 제도대로 하는 것이 권장된다. To measure the birth registration rate and the validity of birth registration data in rural area, all of the 4,014 married women under 49 years of age who had not been sterilized in Gunwee county of Kyungpook province were followed by Myun health workers for 2 years from April 1, 1985 to March 31, 1987 and 766 births were detected. All of the birth registration records of Myun offices were reviewed on September 30, 1987 and 944 births which occurred within the above mentioned period were found. Actual birth date obtained by follow-up study were compared with the birth date on registration card. Among 766 births detected by follow-up study,576 births(75.2%) which were reported within 6 months after birth were ascertained on the official registration records and 96 births(12.5%) were not found on the records although mother stated that the birth was registered. The registration rate within legal due date was 61.3% among 576 births detected by follow-up study and also ascertained on the official records. The registration rate within legal due date was lower in mothers under 20 years of age and above 35 years and in mothers who had only primary education. It was decreased as the birth order increased. The registration rate was higher in births occurred from October to March than births occurred from April to September. All of the births of 7 neonatal deaths were not reported. The registered birth date was consistent with the actual birth date in 78.0%. Birth date on record was earlier than the actual birth date in 6.8% and later in 15.3%. The consistency rate was lower in mothers above 35 years of age(54.5%), and in infants of 4th birth order and above(56.3%). The rate was increased as the maternal education level increased. The rate of boys was higher than that of girls. A higher percentage(17.4%) of infants born in March was registered with earlier date than the actual birth date and most of these registered birth dates were lunar calendar date. This might be related with the age for entering the primary school. The study findings revealed that the birth registration rate within legal due date and accuracy of report have been increased in recent years, but the infant mortality rate derived from the birth registration seems to be very inaccurate. It is suggested to let the medical personnel who delivered the baby report the birth by mail directly to the current address of parent while infants delivered at home without professional attendant may comply with the present registration system.

      • 호산구증가증 환자에서 Toxocara canis에 대한 특이 IgE 항체의 진단적 의의

        Background: Toxocariasis is one of the major causes of peripheral eosinophilia and it provokes eosinophilic infiltration in the internal organs. Diagnosis of toxocariasis has relied mainly on the immunological methods including measurement of serum specific IgG to Toxocara canis excretory-secretory antigen (TES-Ag) in which the detection rate by ELISA was 68% in patients with eosinophilia. Objective: The aim of this study is to measure serum specific antibodies to TES-Ag in patients with peripheral eosinophilia in order to evaluate the clinical significance. Method: Twenty-one patients with peripheral eosinohilia (>500 cells/uL or >10% of total white blood cell count), who have no identifiable cause of eosinophilia such as drugs, well known parasite infection, malignancy or allergic diseases, were enrolled. Serum specific IgE, IgG1 and IgG4 antibodies to TES-Ag were determined by ELISA (Phadia, Uppsala, Sweden) which were compared to those of specific IgE by immunoCAP system (Phadia), and specific IgG to TES-Ag by ELISA (Bordier). Serum total IgE and ECP levels were measured by immunoCAP system. Result: The detection rate of serum specific IgE by ELISA was the highest (90.5%) followed by serum specific IgG1 (65%) and IgG4 (75%). Concordance rate of specific IgE by ELISA and immunoCAP was 100%, while that between specific IgE by ELISA (home-made) and specific IgG by ELISA (Bordier) was 60%. The most commonly involved organ was liver (57.1%). Serum total IgE level in patients with liver involvement was significantly higher than those without it (P<0.01). Conclusion: Toxocariasis should be considered as one of the major causes of the patients with eosinophilia in this country. Measurement of serum specific IgE to TES-Ag may be useful to evaluate the Toxocara canis infection. (Korean J Asthma Allergy Clin Immunol 2009;29:105-111)
