담도경 및 자기공명 담도조영검사가 진단에 유용했던 조기 담도암 1예

Most patients with bile duct carcinoma are diagnosed at an advanced stage, commonly after the appearance of jaundice. The prognosis is generally poor when the diagnosis is made at this stage. Early diagnosis of cholangiocarcinoma at a nonicteric stage may more likely allow curative resection. Initial workup of suspected biliary tract obstruction begins with noninvasive radiologic examinations including US and CT, but these and with even ERCP have a limitation in the diagnosis of early bile duct carcinoma. A case of intrahepatic early bile duct carcinoma without jaundice in a 53-year-old man, is herein reported. The lesion could be early diagnosed, especially with the use of MR cholagiography and percutaneous transhepatic cholangioscopic examination, and eventually could be completely resected. (Korean J Gastrointest Endosc 2002;24:117-121) 본 증례에서 소개한 간내 담도 내의 조기 담도암은 복부 초음파나 전산화 단층 촬영 등의 방사선학적 검사에서 간내 담도의 국소적 확장 소견을 보였으나 임상 증상이나 혈액 검사에서 이상 소견이 뚜렷치 않았다. 더구나 내시경적 역행성 담도조영술에서도 조기 담도암에 동반된 담도 협착 때문에 해당 병소는 조영되지 않았다. 그러나 자기공명 담도조영검사를 시행함으로써 병변이 포함된 간내 담도를 확실하게 묘출할 수 있었으며, 이어서 경피 경간 담도경 검사를 시행하여 조직학적 확진을 얻을 수 있었다. 간내 담도암의 근본적 치료는 간의 부분 절제이므로 이러한 큰 수술을 시행하기 위한 근거로써 본 증례의 경우에서와 같이 자기공명 담도검사와 경피 경간 담도경 검사의 역할은 지대하다고 하겠다.
P-29 Larger testicular volume is independently associated with favorable indices of lung function

Background & objective: Men with chronic obstructive pulmonary disease have reduced endogenous testosterone levels, but the relationship between pulmonary function and endogenous testosterone levels is inconsistent. Testicular volume is a known indicator of endogenous testosterone levels and male fertility. In the present study, we investigated whether a relation exists between testicular volume and pulmonary function. Methods: One hundred and eighty-one South Korean men aged 40-70 years and hospitalized for urological surgery were enrolled irrespective of the presence of respiratory disease. All study subjects underwent pulmonary function testing prior to procedures and testicular volumes were measured by orchidometry. Results: Univariate analysis conducted using linear regression models showed that testicular volume was correlated with FVC, FEV1, FEV1/FVC, and FEV1 % predicted, and multivariate analysis using linear regression models showed that testicular volume was a significant predictive factor for FEV1 % predicted (β =0.159, p=0.041). However, serum testosterone and free testosterone level were not correlated with lung function. The 181 study subjects were divided into two groups by testicular volume, the larger testes group had better lung functions (FVC % predicted: 98.2±15.2% vs 93.8±13.1%, p=0.040; FEV1 % predicted: 105.4±19.5% vs. 95.9±21.2%, p=0.002). Conclusion: Larger testicular volume was independently associated with favorable indices of lung function. These results suggest that testicular volume might predict adult lung function.
Impact of birth season on second-to-fourth digit ratio, prostate volume, and prostate cancer

Purpose: The second-to-fourth digit ratio (digit ratio), which is determined in utero, is associated with exposure to visible sunlight during early pregnancy and the season of birth. The digit ratio is also associated with benign prostatic hyperplasia (BPH) and prostate cancer. This suggests that BPH and prostate cancer may be related to the birth season. Therefore, this study aimed to determine whether prostate volume and prostate cancer were related to the birth season. Materials and Methods: A total of 858 male patients with lower urinary tract symptoms were enrolled. The right digit ratio was measured, and the month of birth was surveyed. Serum prostate-specific antigen (PSA) levels were measured, and prostate volumes were measured by transrectal ultrasonography. Patients with suspected prostate cancer underwent prostate biopsy. Results: The mean age, digit ratio, prostate volume, and serum PSA level of 858 patients were 61.6 years, 0.947, 36.2 mL, and 4.24 ng/mL, respectively. Age, serum PSA levels, prostate biopsy rates, and cancer detection rates did not differ significantly according to the birth season. However, compared with the summer birth group, the winter birth group had lower digit ratios (0.951±0.040 vs. 0.941±0.040; p=0.014), larger prostate volumes (33.4±14.9 mL vs. 38.2±20.7 mL; p=0.008), and more prostate cancer (5.3% vs. 11.3%; p=0.031). Multivariate analysis showed that birth season independently predicted prostate cancer. Conclusions: The relationships of birth season with prostate volume and prostate cancer may be due to differences in the amount of light exposure during early pregnancy.
