        고혈압 건강진단의 비용분석

        To evaluate the costs of the hypertension screening program of the Korea Medical Insurance Corporation, the records of the screening examinations were used. The sample size was 49,983 of the 906,554 people insured by the Corporation and was obtained by two-stage stratification random sampling. The alternatives for efficiency of the screening program, which were divided into three categories : modification of the screening test package, application of other hypertension diagnostic criteria, and selective approach of tested groups by age, were evaluated according to the cost per patient detected. The results of this study were as follows In the hypertension screening system, the cost per patient detected was Won 30,883. The most nonsensitive test for hypertension detection was ophthalmoscopy, which was examined during the second stage of screening. If the ophthalmoscope examination was excluded, olny one person was not detected, which was 0.2% of detected persons, and the cost per patient detected decreased to Won 28,098. The most efficient modification of the screening test package was measurement of blood pressure through the first and second stages of screening. The cost per patient detected by this modification was Won 24,408. The application of other diagnostic critera, which were more restricted criteria, increased the cost per patient detected by 3.7%-6.7%. The cost per patient detected were Won 170,582 for persons less than 39 years old, Won 20,032 for persons 40 to 59 years old, and Won 8,675 for persons 60 years old and over. In conclusion, the best alternative suggested with respect to efficiency and practical application excluded the ophthalmoscope examination of second stage screening and restricted the target population to persons greater than 40 years old. The application of this alternative decreased 54.9% of the screening costs and the cost per patient detected was Won 15,222. This study was limited in that measurement of effectivenes was not of the ultimate goal of screening, which is decreasing morbidity and mortality, but was of disease detection as the short-term objective.

        문재인 정부의 건강보험 보장성 강화대책

        Moon Jae-in Government announced the Government's 5-Year Plan on July 19, 2017, President Moon directly announced the Government's Plan for Benefit Expansion in National Health Insurance on August 7, 2017. The main contents of the announced expansion include benefit coverage for all medically necessary services with control over non-covered service occurrence, a decrease in the cost-sharing upper limit, and monetary support for catastrophic medical costs. Although past governments have been continuously striving for benefit expansion in the last 15 years, this plan has its breakthrough aspect in that all medical services will be covered by the National Health Insurance. In alignment, there are important tasks to solve: attaining a proper fee schedule, reforming the healthcare delivery system, and improving healthcare quality. This plan is a symptom oriented action in that it is limited in reducing patients' out-of-pocket money, unlike the systematic approach of the National Health Insurance. The sustainability of the National Health Insurance is being threatened due to South Korea's low birth rate, rapidly aging society, and low economic growth, in addition to the unification issue of the Korean Peninsula, medical utilization of the elderly, management of non-communicable diseases, and so on. Therefore, the Government needs to plan the National Health Insurance system reformation including actions addressed toward medical consumers.

        윤석열 정부의 보건의료정책 방향과 과제

        The presidential election and the inauguration of the new government are a period of the policy window opening. The newly launched government is expected to improve the quality of life of the people. The Yoon Suk-yeol Government is also launched with new expectations with a transitional period in health care. The sustainability of health care in Korea is threatened. The environment of health care and the main policy issues of health care are difficult to secure the necessary finance for health care in spite of the increasing health care burden. Accordingly, the Yoon Suk-yeol Government's health care policy aims to provide intensive support to those in need of health and welfare and to improve the health of the people through investment in health. And for integrating fragmented health care and welfare services and creating people-centered community-based health care, a health care innovation center will be established for the evaluation platform of new delivery and payment systems, a health care development plan will be established for the blueprint of health care, and reorganizing the central & local government should be reviewed. Although we are facing unfavorable situations such as the distribution of the National Assembly, inflation, and the possibility of economic recession, we expect that announced health care policies will be implemented, recognizing that health care innovation is the only way to improve health care sustainability.

      • 국가 암 조기검진사업의 현황 및 발전 방향

        The Government bean implementing the National Cancer Screening Program(NCSP) in 1999 and expanded its target population and target cancers. The target cancers of NCSP since 2004 are the five most common cancers in Korea: stomachm liver colorectal, breast, cervical cancer. One goal of the NCSP in 2005 is to include in its target population up to lower 50% of premiu of National Health Insurance. The Government and National Cancer Center have bee developing the protocol for the NCSP with associated related academic societies Health Centers operate the NCSP with National Health Insurance Cooperation. The Particioants of NCSP in 2004 are 1.34million, 14% of target population and the detection rate 2004 is 0.07%. NCSP has three challenges. Firstly, NCSP improves the participant rate through educating cancer screening increasing the access of screening(e.g. mobile screening unit), and increasing reimbursement fee Secondly NCSP assesses the quality of screening with related academic societies and implement the intervention for quality improvement. Thirdly, NCSP continues to increase the cost-effectiveness through modification of target population, screening interval, method, and information system.

        심리적 계약위반이 근로자의 몰입과 노조참여행동에 미치는 영향

        이 연구는 근로자의 노조참여행동을 심리적 계약위반에 대한 반응으로 보고, 회사의 심리적 계약위반뿐 아니라 노조의 심리적 계약위반까지 고려하여 노조참여행동을 설명하고자 하였다. 구체적으로 이 연구의 목적은 이들 심리적 계약위반 변수가 조직몰입과 노조몰입을 매개로 노조참여행동에 미치는 영향을 파악하는 것이다. 이를 위해 6개 조직 3,650명을 대상으로 설문지를 배포하여 자료를 수집하고 그 중 유효설문지 805부를 사용하여 공변량 구조분석 방법으로 가설을 검증하였는데, 본 연구에서 나타난 주요 연구 결과는 다음과 같다. 첫째, 회사의 심리적 계약위반은 조직구성원의 조직몰입에, 그리고 노조의 심리적 계약위반은 노조몰입에 각각 부(-)의 영향을 미치는 것으로 나타났다. 둘째, 예상과 달리 회사의 심리적 계약위반은 노조몰입에 부(-)의 영향을 미치고, 노조의 심리적 계약위반은 조직몰입에 부(-)의 영향을 미치는 것으로 나타나 가설과는 정반대의 결과를 보여주었다. 셋째, 조직몰입은 노조참여행동에 한계적 유의수준에서 부(-)의 영향을 미치는 것으로 나타났다. 넷째, 노조몰입은 노조참여행동에 정(+)의 영향을 미치는 것으로 나타났다. 마지막으로, 회사와 노조의 심리적 계약위반은 조직몰입과 노조몰입을 통한 간접적 효과 외에 직접적으로도 노조참여행동에 정(+)의 영향을 미치는 것으로 나타났다. 이상의 결과를 바탕으로 결론 부분에 이 연구의 이론적ㆍ실무적 시사점과 한계점, 그리고 향후 연구 방향을 제시하였다. This study examined union participation behavior from the perspective of psychological contract breach of both union and company, based on the assumption that employees’ union participation is made as a response to the psychological contract breach. The research aimed to verify how these psychological contract breach variables affect union participation behavior mediated by organizational commitment and union commitment. For this purpose, a questionnaire survey was conducted on a sample of 3,650 employees from six organizations. The results of covariance structure analyses upon 805 usable questionnaires are as follows: First, company’s psychological contract breach was found to have a negative impact on the organizational commitment. At the same time, psychological contract breach of union had the same negative impact on workers’ union commitment. Second, contrary to the hypothesis, however, company’s psychological contract breach had a negative impact on union commitment, and union’s psychological contract breach caused a negative impact on the member’s commitment to the organization. Third, organizational commitment negatively affected union participation behavior at the marginal level of significance. Fourth, union commitment was found to have a positive influence to union participation behavior. Lastly, psychological contract breach by the company and union was found to have direct positive impact on union participation behavior. On the basis of above results, the academic and practical implications, limitations, and future research direction are offered in the conclusion.

        한국보건행정학회 30주년 기념 특별호

        The Korean Academy of Health Policy and Management (KAHPM) has shown remarkable achievements in the field of health policy and management in Korea for the last 30 years. The KAHPM consists of experts in various fields of health policy and management, and has been the leading academic discussion forum for health policy agendas of interest to the public. Health Policy and Management (HPM), the official journal of the KAHPM, published the first issue of volume 1 in October, 1991 and is publishing the second issue of volume 28 as of 2018. Currently, it is one of Korea’ main journals in the field of health policy and management. HPM has published a special issue in commemoration of the 30th anniversary of the KAHPM. The HPM invited authors, including former presidents of the KAHPM and current board members, to write about main issues in health policy and management. Although the HPM tried to set up an invited author on all subjects in the health policy and management field, 19 papers are published, that completed the peer review process by August, 2018. The authors of the special issue of the 30th anniversary of the KAHPM include six former presidents, a senior professor, and 12 board members. The subjects of this issue are reform of the healthcare delivery system, health insurance and medical policy, reform of health system governance, the role of National Health Insurance Service (NHIS), the Korea Institute for Health and Social Affairs (KIHASA) and the National Evidence-based healthcare Collaborating Agency (NECA), ethical aspects of health policy change, regional disparities of healthcare, healthcare accreditation, new healthcare technology evaluation system, globalization of the healthcare industry, the epidemiological investigator system, the quarantine system, safety and disaster, and official development assistance. There are some remaining topics to deal with for the KAHPM: aged society, anti-smoking, non-infectious disease, suicide, healthcare resources, emergency medical care, out-of-pocket money, medical fee payment system, medical aid system, long-term care insurance, industrial accident compensation insurance, community-centered health welfare system, and central government and local government of health. The HPM will continue to publish review articles on the main topics in health policy and management. This is because the KAHPM, which has been the leading academic society of Korea’s health policy and management for the last 30 years, feels responsible for continuing its mission for the next 30 years.
