수근관 증후군 환자에서 공간확장테이핑 치료가 임상적·전기생리학적 변화에 미치는 영향
The purpose of this study was to evaluate the effects of taping therapy for carpal space expansion on clinical and electrophysiological changes in patients with carpal tunnel syndrome. The study subjects were 20 women (26 hands) in their 40s to 60s diagnosed with carpal tunnel syndrome. They were divided into the experimental group (10 women (14 hands)) with taping therapy for carpal space expansion twice a week for 4 weeks and the control (10 women (12 hands)) followed without treatment for 4weeks. The results were analyzed by measuring the nerve conduction study, physical examinations (Phalen test and Tinel's sign), and pre- and post-test changes in the visual analogue scale(VAS). Average (M) and standard deviation (SD) each variable were calculated and compared. A paired t-test was conducted to examine the significance level of clinical and electrophysiological changes before and after taping therapy for carpal space expansion within the groups, and an independent t-test was performed to examine the significance level between the groups. In both cases, the statistical significance level was set to p<.05. The study results are as follows: 1. Electrophysiological Changes 1) Compared to before taping therapy for carpal space expansion, the distal motor latency (DML) of median motor nerve conduction studies showed significant differences (p<.001). 2) Compared to before taping therapy for carpal space expansion, the amplitude of the compound muscle action potential (CMAP) of median motor nerve conduction studies did not show significant differences. 3) Compared to before taping therapy for carpal space expansion, Finger-Wrist sensory nerve conduction velocity (SNCV) of median sensory nerve conduction studies showed significant differences (p<.001). 4) Compared to change before taping therapy for carpal space expansion, Finger-Wrist sensory nerve action potential (SNAP) of median sensory nerve conduction studies showed significant differences (p<.05). 2. Clinical Changes 1) Compared to before taping therapy for carpal space expansion, visual analogue scale (VAS) showed significant differences (p<.001). 2) Compared to before physical examination findings (Phalen test and Tinel's sign) did not show significant differences. Based on the above results, taping therapy for carpal space expansion had a positive effect on improving the physical examination findings of nerve conduction studies and reducing pain as an important factor. Therefore, it is considered that taping therapy for carpal space expansion has a significant impact on the pressure reduction by reducing the pressure on the carpal tunnel improving the damaged median nerve and preventing the progression. If taping therapy for carpal space expansion is appropriately used for patients with mild carpal tunnel syndrome as an early preventive measure, it will help to prevent the progression to severe stage and provide scientific data for conservative treatment.
