Interarytenoid adhesion of the vocal cords, with a triangular anterior opening and a smaller posterior rounded opening, is one of the complications of intubation. The vocal cords are tethered to each other and they are restricted with narrow abduction, causing difficult intubation. A 57-year-old woman was scheduled to undergo lung wedge resection. The patient had experienced a prolonged intubation 15 years previously. Thereafter, she had undergone two operations and intubation was difficult to perform with using small size endotracheal tubes (6.5 and 6.0 mm inner diameter, respectively). Despite this past medical history, anesthesia was performed without further evaluation because of the patient's refusal. After the failure of an initial trial of intubation with a double lumen endobronchial tube, we tried to intubate with a small sized endotracheal tube (5.0 mm). But it was impossible to pass the tube through the vocal cords. Endobronchial endoscopy revealed interarytenoid adhesion of her vocal cords. After tracheostomy, she received wedge resection. Therefore, the possibility of difficult intubation due to interarytenoid adhesion of the vocal cords should be considered for a patient with a past history of intubation.
Extensive demyelinating change in cerebrum after a total knee replacement -A case report-

Demyelination is characterized by the loss of myelin with the preservation of axons. Demyelinating diseases can be classified into several categories: demyelination due to inflammation, viral infection, osmotic derangements and hypoxic ischemia. In particular, osmotic myelinolysis is representative, and is associated with hyperosmolality,hypokalemia or rapid correction of hyponatremia. Osmotic myelinolysis was reported to be associated with underlying conditions, such as alcoholism, diuretics and malnutrition. A 67-year-old woman with hypertension was scheduled to undergo both total knee replacements (TKR). She was observed to be lethargic with dysphagia and quadriplegia after the second TKR. She had been taking diuretics for a long time, and did not have an adequate amount of food intake due to patient controlled analgesia and a gastric ulcer after the first TKR. A laboratory examination revealed hypokalemia but normonatremia. T2 weighted-MRI revealed abnormal high signal intensity in the basal ganglia and periventricular area. This case was diagnosed with osmotic myelinolysis associated with hypokalemia without an apparent sodium imbalance.

This study was designed (1) to examine the results of previous qualitative research on clothing-wearing motives by quantitative approach, (2) to explore the concept of clothing utility, utility-reduction and discard, and (3) to investigate the impact of clothing-wearing motives on consumer attitudes and consuming-behaviors. Data were collected by questionnaire from 443 collegiate students between August and September of 2000, and 396 questionnaires were analyzed. 3 groups were identified on the basis of clothing-wearing motives and named by extroversion group, introversion group, and least clothing-cognizing group respectively. This supported the qualitative results in part, especially in the direction of motives. Clothing utility-reduction factors were identified as appearance demage, social-psychological damage, and abandonment -causing damage. The 3 groups were responded on utility-reduction differently, thus clothing-wearing motive variable was determined as useful in fashion marketing.

산업단지 내 주요 악취발생원으로 지적되고 있는 피혁사업장을 대상으로 직접관능법, 공기희석관능법과 기기분석방법을 이용하여 3개 피혁사업장의 3개 공정에서 채취한 7개 시료를 측정평가하여 피혁사업장의 악취특성을 조사하였다. 조사결과 피혁사업장에서 발생하는 악취강도는 2도에서 4도, 복합악취의 희석배수는 300배에서 600배인 것으로 조사되었다. 피혁업종에서 발생하는 악취의 주요 원인물질은 황화수소, 메틸머켑탄, 이소발레르알데하이드인 것으로 나타났다. Investigation of the odor source is necessary to establish of the effective counter plan for reducing the odor emitted from the industrial complexes. This study was carried out to investigate the characteristics of the odor emitted from leather manufacturing plants, using three methods which are a direct sensory technique, an air dilution sensory test and a instrumental analysis. Total 7 samples were sampled at 3 points of three factories in this study. As a results of this study, while odor intensity of 7 samples ranges from 2 degree to 4 degree, dilution ration of olfactory test shows the range from 300 to 600. It was estimated that main malodorous compounds ordered hydrogen sulfide, methyl mercaptan, i-valeraldehyde.
