본 연구는 만성신부전으로 30년 이상 혈액투석치료를 받는 8명의 환자를 대상으로 투석경험의 의미와 본질을 탐색하고자 Colaizzi(1978)의 현상학적 방법을 적용한 질적 연구이다. 2017년 2월 27일부터 5월 30일 까지 심층면담에 의한 자료를 수집하였다. 연구결과 장기간 혈액투석환자의 투석경험은 전혀 다른 삶의 시작의 범주는 ‘힘든 투석생활’, ‘투석치료라는 굴레에 매임’, ‘끝나버린 이전의 삶’, 다시 일어서는 삶의 범주는 ‘투석을 받아들임’, ‘힘이 되는 울타리’, ‘사회생활로 다가서는 삶’, 현존하는 삶의 범주는 ’투석경험을 나누는 삶‘, ‘피할 수 없는 신체적 한계‘, ‘살고자 하는 마음가짐‘의 3개의 범주와 9개의 주제모음과 22개의 주제로 도출되었다. 장기간 혈액투석환자의 투석경험은 전혀 다른 삶으로 시작되는 초기 투석 경험을 통해 힘듦을 이겨내고 각 개인에 맞는 자기 관리 방법을 알고 투석경험을 나누며 장기간의 투석생활을 현존의 삶으로 이어가고 있었다. 혈액투석간호는 투석경험에 따른 다양한 교육방법의 개발과 만성질환자의 투석에 대한 적응 및 지속적인 자기관리 향상을 가져오는 간호가 필요하다. This study is a qualitative investigation that used Colaizzi’s (1978) phenomenological method to evaluate the meaning and nature of the experience of dialysis of eight long-term (>30 years) hemodialysis patients with chronic renal failure. Data were collected from February 27, 2017 to May 30, 2017 by in-depth interviews. Respondents were then divided into three categories, ‘entirely different life’, ‘getting back up again’ and ‘focusing on survival’with nine theme clusters and 22 themes. In general, patients initially experienced an entirely different life and overcame a difficult situation when beginning dialysis, then came to know methods for self-care, shared the experience of dialysis, and focused on survival while receiving dialysis over the long-term. Development of variety of education methods depending on long-term experience of dialysis and nursing care intervention to enable chronic patients to adapt to dialysis and continue their own self-management is necessary.
