치과치료를 위해 진정법 또는 전신마취를 시행하는 목적은 일반적인 방법으로 치과 의료를 제공받기 어려운 환자에게 양질의 치과치료를 제공하기 위해서이다. 본 연구는 전북대학교 치과병원에서 외래전신마취 하에 치료받은 소아 및 장애인 환자들의 임상진료 실태를 분석하고 향후 일반적인 치과진료가 어려운 환자들의 외래전신마취 치료에 대한 지침을 마련하고자 하였다. 2005년 1월부터 2014년 3월까지 전북대학교 치과병원에 내원한 소아 및 장애인 환자들을 대상으로 조사한 결과, 총325명의 환자들이 외래전신마취 하에서 치과치료를 받았다. 남자 환자의 비율이 62.2%로, 여자 환자의 비율(37.8%)보다높았으며, 10세 이하의 환자가 151명(46.5%)로, 소아 및 청소년 환자가 높은 비율을 차지하고 있었다. 치료내용을 보면 수복치료와 발치가 주를 이루고 있었으며 2회 이상 전신마취를 시행한 환자가 43명(13.2%)이었다. 본 연구는 일반적인 치과치료가 어려운 소아 및 장애인 환자들을 위해 효율적이며 안전한 전신마취 시스템을 확충해서 환자들에게 보다 편안한 치료 환경을 제공해야 할 필요성을 제시하고 있다. General anesthesia (GA) for dental care in handicapped patients is necessary to facilitate the provision of safe, efficient, and effective quality treatment. The aims of this study were to determine the anesthetic characteristics of handicapped patients in need of dental treatment in these day care units, and to establish for plan to provide better services. 325 patients who had outpatient general anesthesia from January 2005 to March 2014 were assessed for this study. Patients’distribution and treatment patterns were examined. The proportion of male patients (202, 62.2%) was higher than female patients (123, 37.8%) and the largest group of patients were 5 to 10 years old (85, 26.2%). The reasons for general anesthesia included mental and physical disabilities (207, 63.7%), behavior management (84, 25.8%), parent needs (14, 4.3%), and so on. Restorative treatment was the most common procedure with the average of 4.2 teeth treated per one patient and 43 (13.2%) patients underwent general anesthesia for dental treatment more than once. To expand and improve access to the dental care of the disabled, improvement of the health care system, enhancement of their training for dental care by professionals, and enlarging caregivers’understanding of the importance of oral care in the early stages are required.
학교교육과정 운영에 대한 질적 사례연구: 실천공동체 이론의 관점에서

Purpose: This study analyzed the case of a school curriculum implementation from the theoretical perspective of communities of practice. A community of practice is a group of people who share the same interests, set of problems, and passion for a particular topic and expand their knowledge and expertise in a field through continuous interaction. This study analyzed the case of a unit school as a community of practice and suggested policy measures to enhance the practice. Methods: For this purpose, an interpretive case study was conducted to analyze the curriculum management of a school that is a typical example of a community of practice that has formed over a decade. Various materials such as literature, interviews, photos, and videos were analyzed and validated through triangulation and member checks. Results: The study found that, first, the teachers had a shared educational vision of students' happy lives and sought to create a school for joyful growth; second, the school members were engaged in collaborative practices in all aspects of curriculum management; and third, the school was creating a democratic management system to overcome the constraints of the bureaucratic organization and its own curriculum document style. Conclusion: Policy suggestions for improving the effectiveness of unit schools as an educational community include improving school administration so that teachers can immerse themselves in curriculum management, establishing a customized educational support system for unit schools, allocating differential support personnel and an educational budget based on the educational needs of unit schools.

Purpose: Carcinosarcomas are rare neoplasms in which both malignant epithelial and mesenchymal elements are identified. We have found only twenty one cases of primary cutaneous carcinosarcoma in the English language literature. Therefore it has been difficult to diagnosis because its unclear etiology and low frequency. Methods: A 31-year-old young man with a protruding mass on his ankle dorsum was examined. The tumor was $7{\times}6{\times}3\;cm$ sized and arose from a burn scar. The mass was wide excised and applied split thickness skin graft. Characteristic pathologic finding was a mixture of squamous cell carcinoma and malignant fibrous histiocytoma. Results: Three months after the operation, the patient died of multiple metastasis to the liver, lung and finally of sepsis despite adjuvant chemotherapy and adjuvant radiotherapy. Conclusion: If a metastatic cutaneous carcinosarcoma is diagnosed, wide excision is required. There should be more aggressive management to minimize the risk of recurrence. Further research into the etiology and pathophysiology of the disease and a more careful differential diagnosis may allow improvement in treatment.

Purpose: Now the CT scanner and PACS program proved to be an excellent instrument for detection and localization of most facial foreign bodies above certain minimum levels of detectability. The severity of injury in penetrating trauma on the face is often underestimated in physical examination. Wood, with its porous consistency and organic nature, provides a good medium for microbial agents. This is a report of our recent experience with wooden foreign bodies in the parotid gland imaged with CT.Methods: A 9-year-old boy was referred for evaluation of possible retained foreign body within his face. One day earlier, he had fallen, face down approximately 1 miter onto ground. He had subsequently undergone an exploration of his right parotido-masseteric area at an outside hospital with repair of a right facial laceration. Enhanced 2 mm axial and coronal CT scans were obtained through the face. Axial and coronal CT images were obtained with a General Electric(Milwaukee, Wis) 9800 CT scanner at 130kV, 90mA, with a 2mm section thickness. Results: We finally decided the linear "gas" attenuation was a foreign body because of its linear configuration, which did not conform to that of an anatomic structure, and on the basis of articles that described a wood foreign body in the orbit as having the appearance of air. We found that wood was hypoattenuating(-464± 27HU).Conclusion: We recommend this type of software program for CT scanning for any patient with an injury on the face in which a foreign body is suspected.
