        Association of Blood Pressure at Specific Time-Points with 1-Year Renal Outcomes in Patients with Diabetic Chronic Kidney Disease

        Background: The 24-hour mean blood pressure (mBP) is the best predictor of organ damage; however, it is not easily applicable in clinical practice. The APrODiTe study suggested that systolic blood pressure (SBP) values at 7:00 AM and 9:30 PM were associated with the 24-hour mSBP in patients with chronic kidney disease (CKD). We investigated the association of the SBP values at these time-points with the renal outcomes in patients with diabetic CKD during 1-year follow-up. Methods: Ninety-six patients with diabetic CKD were included at 1-year follow-up. The renal outcomes were an increase in the random urine protein/creatinine ratio or estimated glomerular filtration rate (eGFR) deterioration, which means a decrease in eGFR ≥5 mL/min/1.73 m2 compared to the baseline values. Results: The baseline SBP values at 7:00 AM, and 9:30 PM, and the 24-hour mSBP were 135.6±24.9 mmHg, 141.7±25.6 mmHg, and 136.4±20.7 mmHg, respectively. The SBP values measured at the same time-points after 1 year were similar to those at baseline. The SBP at 7:00 AM was significantly associated with eGFR deterioration in the univariate and multivariate analyses (odds ratio [OR]: 1.032; 95% confidence interval [CI]: 1.006- 1.059; p=0.016). The SBP at 7:00AM and 24-hour mSBP did not show a concordant association with sustained proteinuria in the linear and logistic analyses. In the subgroup analysis, the association between the SBP at 7:00 AM and eGFR deterioration persisted in patients with CKD stage 3-5 (OR: 1.041; 95% CI: 1.010-1.073; p=0.010). Conclusion: The SBP at 7:00 AM, in addition to the 24-hour mSBP, is also associated with eGFR deterioration in patients with diabetic CKD, particularly in those with CKD stage 3-5.

        노래놀이 프로그램 모형 개발 연구: 유⋅초 연계 공동체를 기반으로

        Objectives This study tries to develop a new teaching model of Singing Play in kindergartens and elementary schools based on the community. Methods This Community-based Singing Play Program was developed based on the ADDIE model. The ADDIE model is a stage of analysis, design, development, implementation, and evaluation. This Singing Play Program model applied to 13 kindergarten and elementary school students in Busan, and a final Singing Play Program model was developed. Results First, it was confirmed that a community-based Singing Play kindergarten and elementary School linked program could achieve its meaningful goal. Second, a mixed-age community consisting of young and elementary school students is a strong organization that increases feelings of leadership and belonging as a member of the community, which adds meaning and value to communities. Third, the Community Singing Play Program combines traditional and modern background songs with play, thus playing a role in connecting traditional and modern culture for participants. Fourth, not only did the songs used for singing serve as a useful medium for forming communities, but also played the role of ‘play facilitator’ that encouraged the community to engage in the play. Fifth, the activities of singing programs spread to families, aided in the resolving of conflicts between families and distrust and misunderstandings about each other, leading to the restoration of family functions. Conclusions The Development of Community-based Singing Play Program Model for Connected Education of Kindergarten and Elementary School that it have meaning and value as a program that implements children's and play-oriented curriculum, and the subjects have more leadership and sense of belonging than conflict caused by leadership tattoos or competition during play. In addition, this program played a bridging role in continuing traditional and modern games in this era when play was disappearing. 목적 본 연구는 2019 개정 누리교육과정의 놀이중심, 유아중심 교육과정 구현을 위하여 공동체를 기반한 노래놀이 유⋅초연계프로그램 모형 개발하는 것을 목적으로 한다. 방법 이를 위하여 ADDIE 모형에 기초한 분석, 설계, 개발, 실행, 평가의 단계에 따라 노래놀이 프로그램을 개발하고 이 모형을 부산시 소재 유⋅초등학생 13명을 대상으로 노래놀이 프로그램을 실행하여 최종 노래놀이 프로그램 모형을 개발하였다. 결과 첫째, 공동체에 기반한 노래놀이 유⋅초 연계 프로그램이 가지는 의미 있는 목적을 달성할 수 있는 프로그램으로 확인하였다. 둘째, 유⋅초등학생으로 구성된 혼합연령 공동체는 공동체의 일원으로서 지도력 발휘 및 소속감을 느끼며 끈끈하게 연결된 강력한조직으로 의미와 가치를 지닌 공동체라 할 수 있다. 셋째, 공동체 노래놀이 프로그램은 전래놀이와 현대적 배경을 가진 노래를 놀이와 접목한 프로그램으로 전통과 현대의 놀이를 연결하는 가교적 역할을 하였다. 넷째, 노래놀이에 활용된 노래는 공동체 형성에 유용한 매개체의 역할을 할 뿐만 아니라 공동체로 하여금 놀이로 이끌어 흠뻑 빠져들게 하는 ‘놀이 이끎이’ 역할 수행을 했다. 다섯째, 노래놀이 프로그램 활동이 가족으로 확산하여 가족 간의 갈등이나 서로에 대한 불신과 오해들을 해소시켜 가족의 기능 회복을 가져오게 하였다. 결론 공동체에 기반한 노래놀이 유⋅초 연계 프로그램이 아동, 놀이 중심의 교육과정을 구현하는 프로그램으로써 의미와 가치를가지며, 대상들이 놀이 중에 주도권 타툼이나 경쟁으로 인한 갈등보다 지도력의 발휘와 소속감을 가지게 되었으며 서로를 연결하는강력한 공동체 조직을 구성해 나가는 데 큰 의미를 가지게 되었다. 또한 이 프로그램이 놀이가 사라져가는 이 시대에 전통과 현대적놀이를 이어가는 가교적 역할을 해냈다.

        형태기반코드를 기반으로 한 주거지 열환경 평가

        This study assesses thermal environment of detached housing area by classifying buildings types with codes based on spatial characteristics and forms of the detached housing area and applying heat island alleviation measures, especially focused on FBCs (Form-Based Codes). We analyzed shapes and materials of outdoor space with 3D-CAD, which can affect the surface temperature of the case studies, focusing on heat island alleviation measures, and performed space design by applying relevant climate factors to a simulation. As to the 3D surface temperature and HIP distribution, low-temperature distribution was shown in the case studies when we applied heat island alleviation measures. FBCs (Form- Based Codes) is being developed for the purpose of creating new urban environment. This study is significant because it pays attention to the effects of surface temperatures on accumulation of sensible heat and reviews heat island alleviation measures with outdoor space shapes/materials in order to lower surface temperatures, aiming at improved pleasantness of the detached housing area.

        급성 기도폐쇄 환자에서 일시적인 체외막산소화장치를 이용한 성공적 기도확보 3예 -증례 보고-

        Extracorporeal membrane oxygenation (ECMO) has been used for cardiac and respiratory failure for over 30 years. Recently, however, ECMO has emerged as a useful means of short-term support in the management of hypoxic patients for nontraditional indications. Here, we report the use of veno-venous ECMO as a bridge to support a patient with severe airway obstruction because of tumor compression. Case 1: A patient with extrinsic airway compression secondary to a large metastatic cancer on neck was successfully managed using ECMO. Case 2: The successful use of ECMO to support a patient with extrinsic airway compression secondary to a recurred thyroid cancer. Case 3: A pregnant woman with airway obstruction secondary to metastatic lymphadenopathy of lung cancer who underwent successful tracheal stent insertion. The 3 patients were successfully weaned off ECMO without any complication. Although these conditions are uncommon indications, ECMO is a potential option for such life-threatening conditions.

      • 건조 쪽을 이용한 모발 천연염색의 표면 염색성 및 염착량

        The purpose of this study is to determine the degree of hair dyeing according to the presence or absence of mordant use and the dyeing time using the dry indigo, the bleached hair sample was pre-mordded in alum (reagent, 5%, owh, on the weight of hair) solution for 10 minutes and then When dyeing at ℃ for 10 minutes, 20 minutes, 30 minutes, and 40 minutes, the maximum absorption wavelength of dyeing on the dry side was 420 nm. The result is as follows. First, when a mordant (Al) was used, the overall color was dyed green, and the L* value was the highest when staining for 10 minutes, followed by 20 minutes and 30 minutes. Also, both a* and b* values were found to be high in the order of 10 minutes, 20 minutes, and 30 minutes. In the case of 40-minute staining, it was the lowest. Second, when no mordant was used, all were dyed in blue green, and the L* values were high for a* and b* values in the order of 10 minutes, 20 minutes, and 30 minutes. In the case of 40-minute staining, it was the lowest. Third, in the case of K/S of the amount of dyeing, the K/S value was higher when alum mordant was not applied, and the K/S value was lower when alum mordant was applied. In both mordant and non-mordant staining, the amount of dyeing increased as the time increased for 10, 20, and 30 minutes of staining, but after 30 minutes, the dyeing was no longer combined and the dye was no longer darkly dyed. It was found that the amount of dyeing and saturation decreased as the time increased.

        코로나19 전담격리병동 신규간호사의 근무경험

        The study was conducted to increase the capacity of professional nurses for new nurses caring for patients with COVID-19 and emerging infectious diseases expected to become more serious in the future. From August to September 2022, eight new nurses with less than 12 months clinical experience working in the COVID-19 isolation ward of Advanced General Hospitals and General Hospitals located in G metropolitan city and J province were conducted one-on-one in-depth interviews, and the data were analyzed by the phenomenological method of Colaizzi (1978). The research content of this study was analyzed into five categories: ‘Special nursing starting with COVID-19’, ‘Burnout and conflicts due to overburdening’, ‘Difficulties due to lack of clinical experiences’, ‘The growth and development in the COVID-19 Patient-Specific ward’ and ‘A Wish for better nursing’. Ultimately, this study hopes that it will be used as fundamental data to provide professional and high-quality nursing to patients receiving nursing services as well as national and hospital policies for new nurses.

        Complete Freund’s adjuvant 유발 관절염 모델 흰쥐의 척수 뒤뿔내 neuronal nitric oxide synthase 발현에 대한 전기침의 영향

        To examine the effects of electroacupuncture (EA) on central nociceptive modulation, expressional changes of spinal neuronal nitric oxide synthase (nNOS) were investigated in complete Freund’s adjuvant (CFA)-injected rats. Inflammation was induced by an intraplantar injection of CFA into the hindpaw of male Sprague-Dawley rats. Bilateral EA stimulation at 2 Hz, 15 Hz and 120 Hz was applied at those acupoints corresponding to Zusanli and Sanyinjiao in man with 3-day intervals for 30 days. At 30 days after CFA-injection, effects of EA on nNOS expression were observed in the dorsal horn of the spinal cord using immunohistochemical methods. The mean integrated optical density of nNOS immunoreaction was significantly increased in the dorsal horn throughout L1 to L5 lumbar segments in CFA-injected rats. The nNOS expression was attenuated in all regions of the dorsal horn by all types of EA. Especially, these reaction was markedly decreased in the superficial laminae and nucleus proprius of L1 and L3 lumbar segments by three types of EA, but a marked decrease in the neck of the dorsal horn was observed only in 2 Hz stimulation. The marked decrease of nNOS also showed in nucleus proprius and the neck of L5 lumbar segments in 2 Hz and 15 Hz EA stimulated rats. It is concluded that EA treatment can attenuate chronic inflammatory process in CFA-injected rats through modulating expression of nNOS in the dorsal horn of the spinal cord. 전기침의 항부종 및 통각과민에 대한 효과와 척수내 nitric oxide (NO)의 관련성을 알아보기 위해 complete Freund’s adjuvant (CFA)주사한 만성 관절염 모델을 사용하여 척수 뒤뿔내 neuronal nitric oxide synthase (nNOS)의 발현을 살펴 보았다. 흰쥐 오른쪽 발바닥에 CFA 주사한 후 족삼리와 삼음교에 해당하는 혈위를 2 Hz, 15 Hz 및 120 Hz 전기침을 30 분간 3일 간격으로 30일 동안 처치하였다. CFA 주사 후 30일에 척수 중 제1번, 제3번 및 제5번 허리분절에 해당 하는 분절의 nNOS발현을 면역조직화학적으로 검색하였다. CFA를 주사한 흰쥐에서 척수 뒤뿔의 모든 부위에서 nNOS 발현이 현저히 증가하였으며 특히 superficial laminae (SDH, laminae I and II)에서의 증가가 현저하였다. 그러나 모든 전기침군에서 nNOS의 발현이 감소하였는 데 특히 제1번과 제3번 허리분절의 SDH와 nucleus proprius (NP, laminae III and IV)부위에서 모든 전기침군이, neck of the dorsal horn (NECK, laminae V and VI)부위에서 2Hz 전기침군이 유의성 있는 감소를 보여 주었다. 제5 번 허리분절에서 NP와 NECK부위에서 2Hz와 15 Hz 전기침이 유의성 있는 효과를 보여 주었다. 이로 보아 전기침은 척수 뒤뿔내 nNOS발현억제를 통한 통각과민 완화 효과를 나타내는 것을 알 수 있으며 통 각유발과 NO관련성을 유추할 수 있으며 전기침의 항염증의 효과 검정에 있어 nNOS발현이 주요 표지로 이용가능 할 것이다.
