        의약분업 이후 약국의 운영현황 분석

        It has been 2 years since the implementation of the separation policy of drug prescription and dispensing. This study analyzes the changes in community pharmacy operation after the implementation of the policy. The main purposes of the analysis are to determine whether the changes in community pharmacy operation have occurred and to evaluate that the changes are consistent with the intention of the policy, if the changes actually have occurred. For the study a survey on 961 pharmacies chosen by stratified sampling method has been performed. Of the 961 sample pharmacies, 438 pharmacies were responded resulting 45.6% response rate. The sample pharmacies are classified by the location that the pharmacy are operating: the pharmacies around large size hospitals, the pharmacies around clinics or medium to small size hospitals and the pharmacies with no hospitals or clinics around. Based on the classification, the numkr of pharmacies, number of prescriptions processed, the personnel structure, the changes in facility, and other operational characteristics are compared. The results showed that the pharmacies were tended to concentrate around hospitals and clinic since the implementation of the policy. The number of pharmacists per pharmacy was increased, the size of pharmacy was increased and the fachties were improved to accomodate the requirements of the policy. The work hours a pharmacist spent on dispensing drug have increased almost twice, however, there was no corresponding increase in the time spent on patient education and medication history management, indicating a problem in the provision of quality pharmaceutical services. Based on the results, suggestions to minimize the negative effects of the policy are provided.

        의사들의 전문의약품 사이버영업사원 사용의도에 대한 영향요인

        Objectives: The purpose of this study was to investigate influencing factors on physician`s intention to use cyber salesperson(cSP) to promote ethical drugs of pharmaceutical company. We considered influencing factors based on technology acceptance model(TAM) such as usefulness, easy of use, and attitude about cSP, and included trust and joyfulness of cSP. Methods: Measure items were developed from the previous studies. Data were collected from 1,012 physicians by using web-based self-administered structured questionnaire. Results: The major findings were as follows; First, attitude and intention to use cSP were not significantly varied by socio-demographic variables, such as sex, age, location, and Internet surfing time per day. Also, physician`s characteristics such as training and professional level and type of employment were not significant to the variation of attitude and intention to use cSP. Second, structural equation modeling(SEM) analysis shows that usefulness and joyfulness were influencing factors of intention to use. Attitude was revealed as significant mediating factor to intention to use. Usefulness has indirect effect on intention to use via attitude. However, trust has not significant effect on intention to use, due to suppression effect. Conclusions: This results implies that cSP will be a useful agent to promote the ethical drugs to physician. Usefulness and joyfulness should be considered as important factors in designing and operating cSP in the Internet.

        의료기관의 균형적 경영전략, 조정 및 학습 기전의 경영성과에 대한 영향

        The purpose of this study was to find out the differences and relationships among balanced strategy, coordinating and learning mechanism, and perceived performance of hospitals in Korea, and provide some directions to establish effective strategic management of hospital. Measure items on balanced strategy, coordinating and learning mechanism, and perceived performance were developed from previous studies. Questionnaire was sent and received through Internet site and e-mail during May, 2008. Data were collected from key informant in each institutions, and analyzed using frequency analysis, T-test, ANOVA, correlation and regression analysis. The major findings of this study were as follows: 1. The level of strategic selection and external learning mechanism of private hospital was lower than that of medical corporation, and others corporation hospital. 2. There was little difference between hospitals in metropolitan and those in small cities. 3. Hospitals that have under 100 beds were statistically lower level in strategic selection and external learning mechanism than hospitals has over 100 beds. 4. Formal coordinating and external learning mechanism, and foundation form(medical corporation) were significantly influenced on profitability from specialized field. 5. Strategic selection and adaptation mechanism were significantly affected on total profitability. 6. Strategic selection and external learning mechanism were significantly influenced on competitive power around its local market. Hospitals that are to be competitive by specialization should have to establish mechanism for management such as balanced strategy, coordinating and learning mechanism.

        의약분업 이후 병원 약제부서의 업무내용 및 인력구조 변화 분석

        It has been 2 years since the implementation of the separation of prescription and drug dispensing policy. This study analyzes the effects of the policy on the job contents and personnel structure of hospital pharmacy. The main purposes of the analysis are to determine if the policy has causes the increase of professional activities of pharmacists in hospital and to investigate whether the hospital pharmacy is equipped with enough manpower to provide high quality pharmaceutical service as intended by the policy. The level of professionality of pharmacists activities is measured by the number of activities of direct involvement in inpatient care such as participation in patient rounding, medication consultation, the number of hospital committee the pharmacists involved and the number of continuous education pharmacists took. The adequacy of personnel structure to provide high quality pharmaceutical care is measured by the level of compliance to the governmental standard of hospital pharmacy personnel. In order to collect the data, surveys were performed for two periods: year 1999 (before the implementation of the policy) and year 2001 (after the implementation of the policy). The results show that the pharmacists participation in inpatient rounding decreased and that the inpatient medication history management activities, operation of ward pharmacy, participation in hospital committee increased. In personnel structure, the average number of pharmacist per hospital decreased and the number of prescription processing per pharmacist increased. Based on the results this study concludes that the professional activities of hospital pharmacists has increased a little and there were structural changes in hospital pharmacy service activities to increase the professionalism in providing care. However, the pharmacy departments were understaffed hampering the strive to increase the provision of professional pharmaceutical service in hospitals.

        공공부문 u-Health 서비스 효과성 분석

        Objectives: This study was to evaluate the effectiveness of u-Health service in public area based on an extended technology acceptance model (TAM), and test the acceptance behavior of the u-Health service. Methods: Data were surveyed from the experienced of u-Health service for three months (7.1~9.30, 2010) in Seocho city, Seoul, Korea. We measured personal innovativeness, subjective norm, output quality, and perceived playfulness with likert-scale multiple items, perceived usefulness, perceived ease of use, attitude toward use, and the behavioral intention as mediator variables; and actual use and u-Health loyalty were used as dependent variables. Results: The results were as follows; Service quality of u-Health in public area was the significant key factor to the usefulness, the ease of use. Most of the hypotheses from the original model of TAM were supported. Behavioral intention significantly affected actual and continuing use of u-Health service. TAM variables mediated between independent variables and dependent variables. Conclusions: We evaluated u-Health services for public health management by survey data from the users. The quality of u-Health service was significantly effective by controlling the other influencing factors. Results will be useful for decision making about u-Health service in public area.

        임상간호사의 서번트 리더십과 임파워먼트가 인사공정성과 조직몰입에 미치는 영향

        Objectives: The purpose of this study was to investigate the effects of servant leadership and empowerment to justice of the performance appraisal and organizational commitment of clinical nurses. Methods: Data were collected from the nurses in university-affiliated hospitals located in Seoul and Gyeonggi province, Korea, and structured questionnaire was used. Likert 6-point scale was applied to measure research variables, such as servant leadership, empowerment, justice of performance appraisal, and organizational commitment. Total valid 299 cases were analyzed by IBM SPSS win 19.0. Results: Servant leadership(listen, collaboration and support) and empowerment of clinical nurses were positively influenced to perceived justice of performance appraisal. Servant leadership (listen) and empowerment of clinical nurses were positively influenced to affective and continuance commitment of them. Conclusions: We may suggest that the results would be used to make leadership of nurse leaders in hospitals.

        u-Health 수용요인의 교차타당성 검증: 공급자와 수요자 집단간 분석

        Objectives: The purpose of this study is to analyze structural relationships between the service quality of u-health and the attitude toward u-health between potential demanders and supplier groups of u-health service through cross-validation method. Methods: We used a survey data, collected from medical doctors and latent u-health users by Korea Institute for Health and Social Affairs (KIHASA) to confirm and show the demand for u-health in 2011. We analyzed data from 731 medical doctors and 600 potential users of u-health by using SPSS 19.0 and AMOS 20.0 programs. Parameter estimates of the service quality and the attitude toward u-health were cross-validated through multi-group confirmatory factor analysis between u-health suppliers (medical doctors) and latent u-health users. Results: We showed that multi-group confirmatory factor analysis was appropriate for both groups, u-health suppliers and users. Service quality of u-Health was a significant factor to attitude toward u-health in both groups. Conclusions: Cross-validated measures between suppliers and users for the service quality and the attitude toward u-health should be standardized and useful in future research on u-Health.

        농어촌지역 만성질환 원격의료서비스에 대한 지불의사금액 추정

        Objectives: The purpose of this study is to estimate willingness-to-pay (WTP) of rural telehealth service for chronic diseases by the contingent valuation method (CVM). Methods: A survey was conducted for 451 peoples with chronic diseases (hypertension & diabetes) who are using trial telehealth services at 20 community health practitioner posts in Gangwon province. Before estimating WTP for telehealth service, reliability was tested by following of NOAA recommendations. WTP was measured and estimated by using a double-bounded dichotomous choice contingent valuation method and survival analysis. Results: We estimated the average WTP for one-time telehealth service as 4,062won, which is larger than normal service charge in health post, in case with covariate by survival analysis. Conclusions: The results of this study means that telehealth services for peoples of chronic disease in the underserved rural area would be effective and valuable in Korea. To be accepted and implemented successfully, there must be more detailed consideration or test about social psychological and cultural aspects, medical safety, disbursement policy, and separation of duty.
