Purpose: To evaluate the effect of 9 mm and 7 mm bilateral recession of the superior rectus muscle in patients with bilateral dissociated vertical deviation (DVD) who had a deviation difference between the non-fixing and fixing eyes of less than 8PD. Methods: The subjects were chosen from the patients who visited our hospital for DVD between January 2001 and November 2003. We reviewed the surgical records of 19 patients who had undergone 9 mm recession of superior rectus muscle in the non-fixing eye and 7 mm in fixing eye, and who had been followed up for at least 6 months. Results: A residual DVD less than 8PD was regarded as successful. The overall success rate was 73.7%. The success rate of 10~14PD preoperative deviation of the non-fixing eye was 80.0%, and that of 15~19PD preoperative deviation of the non-fixing eye was 75.0%. However, the success rate was 66.7% in recession for the deviation over 20PD. Conclusions: Bilateral recession of 9 mm and 7 mm of the superior rectus muscle was effective for bilateral DVD in which the deviation difference of the non-fixing eye and fixing eyes was less than 8PD .

1개월 동안의 양안 이물감 및 소양감을 주소로 내원한 2세 된 남아의 양안 상, 하안검 첩모에서 세극등 검사상 사면발이의 성충과 충란을 관찰 후 확진 하였다. 치료로 충란과 성충의 직접제거 및 첩모제거술, 20% fluorescein dye 도포와 세척을 시행하였고 광학현미경과 주사전자현미경으로 충란과 성충을 관찰하였다. 1주 간격으로 1달간 추적관찰 하였고 세극등검사상 양안의 안검과 첩모에서 성충 또는 충란이 발견되지 않아 완치 판정을 내렸다. 본 증례를 문헌 고찰과 함께 보고하는 바이다. A two year old male child complained of foreign body sensations bilaterally, and an itching sensation at both upper and lower eyelids for 2 months of duration. He was diagnosed with Phthiriasis palpebrarum after observation of adult louse and nits on slit-lamp examination. The treatment was accomplished by mechanical removal of the lices and nits by forceps and scissors, and then irrigation with 20% fluorescein was done under general anesthesia. Antibiotic eyedrops and ointment were applied to both eyes. The morphologic evaluation was performed with light microscopy and scanning electron microscopy. The follow-up interval was every weeks for one month and no louse or nit was found under slit-lamp examination. So we concluded that the Phthiriasis palpebrarum was eradiated completely. We report a case of Phthriasis palpebrarum had been review of the literatures.

Purpose: To identify the characteristics of adult patients who develop acute concomitant esotropia during adulthood. Methods: Using a retrospective noncomparative method, this study evaluated the medical charts of 10 adults who were diagnosed with acute onset esotropia between January 2003 and May 2005. All patients presented the symptoms of estotropia when they were older than age 18. All patients in the study group had no previous ocular history, no cause of interruption of fusion, no history of systemic disease or head trauma, and no abnormal findings on neurologic examination. Results: The average follow-up period was 10.8 months (range, 5~17 months). The mean age was 44.7 years (range, 18~62 years). The average refractive error was -1.43 diopter (range, -5.50~+0.50 diopter). An operation was performed in one case. The prism diopter decreased according to follow-up in other patients. Conclusions: There are characteristics of acute onset esotropia of adulthood that differentiate it from existing esotropia. Almost all patients were myopic, showed nearly the same prism diopter between near and far vision, and deviation improved with follow-up.
상사시에서 수직근 후전술 후 사시각의 변화에 대한 고찰

Purpose: To determine the extent of vertical rectus muscle correction in hypertropia showing good prognosis. Methods: A retrospective study was performed with a total of 16 patients who underwent superior or inferior rectus muscle recession surgery with a follow-up of more than 6 months. Vertical muscle recession of 1 mm per 2.5 to 3.0 prism diopters was performed according to the surgeon's discretion. Results: At 1 week after surgical correction, undercorrection, orthophoria, and overcorrection was observed in 4, 7 and 5 cases, respectively. At the final examination, ocular deviation was decreased in the undercorrected cases and maintained orthophoric except in 1 case where only a small amount of deviation recurred. However, in the cases of postoperative overcorrectionn, ocular deviation increased; 2 cases required surgical correction for consecutive hypertropia. Conclusions: When performing vertical rectus muscle recession in primary hypertropia, the amount of correction for orthophoria or undercorrection should be determined. Purpose: Anisometropia can lead to monocular deviation if the refractive error is not corrected. Therefore, the authors evaluated the change in angle deviation after visual acuity improvement by refractive correction in monocular deviated patients with anisometropia. Methods: Changes in angle deviation were collated retrospectively for 9 patients with anisometric monocular deviation, 7 with monocular exotropia and 2 with monocular esotropia, according to medical records. The patients were admitted for strabismus surgery performed using cataract extraction or clear lens extraction (8 patients) or were treated nonsurgically using contact lenses for visual acuity recovery (1 patient). Results: Prior to corrective measures, patients with exotropia had, on average, exodeviation of 40.43 PD, and those with esotropia had, on average, esodeviation of 27.50 PD. After corrective measures were taken, all 7 exotropia patients had decreased angle deviation, and, upon final evaluation, exodeviation had decreased to 21.71 PD, on average. In two exotropia patients, measures taken to correct refractive error shifted the exotropia to exophoria. There was no change in angle deviation after Corrective measures in 2 esotropia patients. Conclusions: In cases of exotropia caused by decreased visual acuity, correction of the visual acuity should be performed first if the correction is possible. After the recovery of visual acuity, a significant decrease in angle deviation occurs. 목적: 굴절부등에서 시력교정을 충분히 시행하지 않는 경우 한눈사시가 발생할 수 있다. 저자들은 굴절부등을 동반한 한눈사시 환자에 서 시력회복 후 사시양상 및 사시각의 변화에 대해서 알아보고자 하였다. 대상과 방법: 1997년부터 2007년까지 사시수술을 위하여 본원에 내원하였던 환자 중 시력회복이 가능하였던 한눈 외사시 7명, 한눈 내사시 2명에서 투명 수정체 적출술을 포함한 백내장 수술 또는 콘택트렌즈를 통한 시력회복 후 사시형태 및 사시각의 변화를 후향적 으로 조사하였다. 결과: 8명에서는 백내장 수술, 1명에서는 콘택트렌즈를 이용하여 시력을 교정하였다. 교정 전 외사시를 보인 7명의 평균 외사시각은 40.43PD이었고 내사시 2명의 평균 내사시각은 27.50PD이었다. 교정 후 외사시 환자 7명 모두 점차 사시각이 줄어들어 최종 관찰시 평균 외사시각 21.71PD 으로 감소하였고, 2명은 사위로 변화하였다. 내사시 환자 2명에서는 시력회복 후에도 사시각의 변화를 관찰 할 수 없었다. 결론: 한눈시력의 감퇴로 생긴 외사시의 경우, 시력회복 후 사시각이 감소 할 수 있으므로, 시력회복이 가능하다면 사시 교정술 보다 시력 교정이 먼저 선행 되어야 하겠다.
