        CNC선반가공에서 노오즈 반경과 냉각방법에 따른 알루미늄7075의 표면 거칠기에 관한 연구

        Current world aircraft industry studies on the precision of the product are in active progress. Particularly in terms of improving the quality of processed products in terms of the surface roughness of the dimensional accuracy, fatigue strength, and corrosion resistance, which affect a lot of research on surface roughness, has been investigated. In this study of aluminum alloy, 7075 aircraft aluminum is used in a cutting CNC lathe machine for the cutting speed and feed rate according to the cutting experiments that were conducted. Additionally, the machine tool of the cooling method soluble cutting oil, insoluble cutting oil by cooling, and cooling the workpiece by cutting surface roughness will be investigated. Through the method and soluble cutting oil coolant cooled by the cutting speed increases, the value of surface roughness showed a regular result. Tool nose radius of 0.8 mm than 0.4 mm picture of when approximately 50 of the surface roughness values were less.

        Failure of Intramedullary Nailing for Subtrochanteric Atypical Femoral Fractures Caused by Endosteal Cortical Thickening

        Purpose: Recent literature has noted incidences of subtrochanteric atypical femoral fractures (AFFs) in patients who have taken long-term bisphosphonates (BPs). Most cases of subtrochanteric AFFs have been treated with intramedullary nailing and cases of delayed union have been reported. On the other hand, there is no data available on the complications associated with endosteal thickening or cortical thickening. This study evaluated the results of surgical treatment according to the endosteal thickening of the lateral cortex in subtrochanteric AFFs. Materials and Methods: Investigation was performed at the Department of Orthopaedic Surgery, Jeju National University Hospital. The study consisted of patients with subtrochanteric AFFs, defined by the American Society for Bone and Mineral Research (ASBMR) major criteria, who underwent intramedullary nailing from March 2012 to October 2014. The cases were categorized into two groups based on the presence of endosteal thickening. The evaluation included the demographic data, radiographic data of initial reduction state, and duration of BPs. Results: The demographic data and duration of BPs were similar in the two groups. On the other hand, varus reduction (Group I: 12.5% vs. Group II: 78.9%; p=0.001), delayed union (Group I: 0% vs. Group II: 70.0%; p=0.003), nonunion (Group I: 0% vs. Group II: 47.4%; p=0.017), and union time (Group I: 5.5 months vs. Group II: 8.3 months; p<0.001) were significantly different in the two groups. Conclusion: Endosteal thickening of the lateral cortex in subtrochanteric AFFs was identified as an independent factor that decides the reduction of the fracture and nonunion. The endosteal thickening should be removed to obtain anatomical alignment for successful surgical results.

        COVID-19 폐렴에 속발한 고안압성 앞포도막염 및 각막내피염에 대한 증례

        Purpose: We report a patient diagnosed with COVID-19-associated hypertensive anterior uveitis and endotheliitis of the cornea. Case Summary: A 71-year-old male visited our hospital with decreased visual acuity, ocular pain, eyelid swelling of in his right eye, and headache for 8 days. He had been diagnosed with COVID-19 pneumonia 10 days before the visit. The best-corrected visual acuity was 0.04 in his right eye and 0.15 in his left eye. Severe conjunctival injection, moderate corneal edema with Descemet membrane folding, diffuse keratic precipitates and mild anterior chamber reaction were observed in the right eye, which were not present in the previous examination. Pupils in both eyes were moderately dilated and slow to react in response to direct light. The density of endothelial cells in the right eye was 711 cells/mm2 and an owl’s eye morphology of endothelial cells was observed via specular microscopy. During follow up, elevated intraocular pressure (IOP) in the right eye was noted. Polymerase chain reaction (PCR) examination of the aqueous humor for cytomegalovirus was negative. After 2 weeks of applying prednisolone eye drops and anti-glaucoma eye drops, corneal edema decreased, and his vision improved to 0.5. However, the density of the corneal endothelium remained low. Conclusions: Although there have been worldwide case reports of conjunctivitis, keratitis, or uveitis after COVID-19 infection, this study presents the first reported case of COVID-19 associated hypertensive anterior uveitis and endotheliitis of the cornea. 목적: 코로나바이러스 폐렴에 속발한 고안압성 앞포도막염 및 각막내피염을 진단하여 보고하고자 한다. 증례요약: 71세 남환이 10일 전 COVID-19 확진 뒤 2일 후부터 우안 시력저하, 통증, 눈꺼풀 부종 및 두통을 주소로 내원하였다. 내원시 나안시력 우안 0.04, 좌안 0.15, 모두 교정불가하였다. 전안부검사상 우안 결막충혈, 상측의 각막부종에 동반된 데스메막 접힘, 색소성 각막후면침착물, 전방염증이 확인되었다. 양안 동공은 산대되었으며, 대광반사는 저하되었다. 우안 각막내피세포검사상 세포밀도 711 cells/mm2로 감소해 있었고, 전반적으로 세포들의 크기가 증가하였고, 어두운 음영의 핵을 가진 세포들이 관찰되었다. 추적관찰 중 우안 안압상승이 보여 이에 우안 고안압성 앞포도막염 및 각막내피염 진단하 시행한 거대세포바이러스검사에서 음성이 확인되었다. 약 2주의 스테로이드 안약 및 안압하강제 점안 치료 후 호전되었다. 결론: 코로나바이러스 감염에 의한 결막염, 각막염 및 포도막염이 최근 세계적으로 보고되고 있으나 본 증례는 COVID-19 확진 후양안의 동공 산대를 동반한 단안성 각막내피염 및 고안압성 앞포도막염이 발생한 최초의 보고이다.

        Long-Term Clinical and Radiological Outcomes of Minimally Invasive Transforaminal Lumbar Interbody Fusion: 10-Year Follow-up Results

        Background: Many studies have reported that minimally invasive transforaminal lumbar interbody fusion (MI-TLIF) provides satisfactory treatment comparable to other fusion methods. However, in the case of MI-TLIF, there are concerns about the long-term outcome compared to conventional bilateral PLIF due to the small amount of disc removal and the lack of autogenous bone graft. Long-term follow-up studies are still lacking as most of the previous reports have follow-up periods of up to 5 years. Methods: Thirty patients who underwent MI-TLIF were followed up for > 10 years (mean, 11.1 years). Interbody fusion rates were determined using a modified Bridwell grading system. Adjacent segment disease (ASD) was defined as radiological adjacent segment degeneration (R-ASDeg) as seen on plain X-rays; reoperated adjacent segment disease referred to the subsequent need for revision surgery. Clinical outcomes after surgery were assessed based on back and leg pain as well as the Oswestry disability index (ODI). Results: The overall radiological fusion rate, at the 1-, 5-, and 10-year follow-up was 77.1%, 91.4%, and 94.3%, respectively. The incidence of R-ASDeg 1, 5, and 10 years after surgery was 6.7%, 16.7%, and 43.3% at the proximal adjacent segment and 4.8%, 14.3%, and 28.6% at the distal adjacent segment, respectively. R-ASDeg at either the proximal or distal segment was determined in 50.0% of the patients 10 years postoperatively. All clinical parameters improved significantly during follow-up, although the ODI and the visual analog scale (VAS) for leg pain at the 10-year follow-up were significantly worse in the R-ASDeg group than in the other patients (P = 0.009, P = 0.040). Conclusion: MI-TLIF improved both clinical and radiological outcomes, and the improvements were maintained for up to 10 years after surgery. However, R-ASDeg developed in up to 50% of the patients within 10 years, and both leg pain on the VAS and the ODI were worse in patients with R-ASDeg.

        군관사거주와 민간주택거주 직업군인의 주거만족도 차이분석

        The Ministry of National Defense(MND) has been providing residences or subsidizing to professional military personnels. One form is the military housing built and maintained by the government, and the other is the renting type from civilian housing market the government subsidizes for. According to a recent survey, the military housing has not been satisfied although it requires much resources. On the contrary, those living in rented civilian houses reported positive feedbacks except for the expensive costs. Therefore, military housing policies should include less of traditional government–built and maintained and more of subsidizing for civilian housing market with more financial supports. 직업군인을 위해 주거지원을 제공하고 있다. 주거지원에는 국가가 건립하고 유지관리하는 군관사 형태가 있고 전세자금을 지원하여 민간주택에서 거주하는 형태가 있다. 직업군인 1만여명 대상 주거만족도를 조사하였는데, 총 21개 만족도 항목 중 19개가 군관사보다 민간주택의 만족도가 더 높았다. 결론적으로 군관사는 줄이되, 격오지를 고려하여 군주거정책지원을 다차원적으로 대응할 필요가 있다. 기존 노후 군관사의 재고량을 조절하면서 전세대부의 군인 비용부담을 낮추어 군인의 주거만족도를 높이면서 국가재정부담을 줄이는 방향으로 개선해야 할 것이다.

      • 금지급 과실파리류의 예찰 및 방제에 관한 고찰

        과실파리류 국내 유입에 대비하여 체계적으로 대응하기 위해서는 예찰방제 매뉴얼 수립 뿐만 아니라 예찰과역학조사, 방제와 관련된 제도적인 정비와 물리적인 기반 구축이 필요하다.우선 과실파리류의 효율적인 예찰과 방제를 위해서 국내에 생리․생태를 연구할 수 있는 연구시설을 확보할필요가 있다. 이런 연구시설을 통해 우리나라 기후에서 월동가능성 확인, 각 지역별 예찰의 시작 및 종료 시기결정, 방제방법 등에 관한 세부적인 연구가 진행되어야 한다.또한, 웅성불임충 대량 방사를 통한 방제를 실시해야하는 상황에 대비하여 이에 관련한 제도적, 물질적 기반을미리 갖추어야 한다. 현행 법령 규정상 살아있는 과실파리의 도입이 불가능하므로 법률상 근거를 마련하기 위한개정작업을 서두를 필요가 있다.다음으로 역학조사 차원에서 과실파리류의 유입경로로 작동할 수 있는 외국인 노동자와 다문화가정의 출입국정보를 확보하기 위한 법률 근거를 마련할 필요가 있다.마지막으로 과실파리류 방제지침 작성 및 운영의 책임기관인 농촌진흥청과 과실파리류 연구자들이 공동으로발생상황이 될 경우 과실의 수거·폐기 범위, 약제 살포 범위, 절개조사 범위, 이동제한 조치후 출하과정에서 소독처리범위와 검사실시 범위 등을 어느 정도까지 설정할 것인지 빠른 시일 내에 결정할 필요가 있다.

        오가피(五加皮) 추출액(抽出液)이 면역(免疫), 항암(抗癌) 및 비만(肥滿)에 미치는 실험적 효과

        This experimental Study was designed to investigate the effects of Acanthopanacis Cortex Extract(ACE) on the immunity, anti-cancer and obesity in mice. The results were as follows; ACE was significantly increased in the proliferation of thymocytes and splenocytes, and NO production from peritoneal macrophages in normal mice. ACE was significantly increased in the proliferation of thymocytes and splenocytes, and NO production from peritoneal macrophages in L1210 cells transplanted mice. ACE was significantly decreased in the proliferation of L1210 cells in L1210 cells transplanted mice. ACE was significantly inhibited body weight and tumor weight in S-180 cells transplanted mice. ACE was significantly increased in the mean survival days in S-180 cells transplanted mice. ACE was significantly decreased in the body weight in rats fed high fat diet. ACE was significantly decreased in the serum total cholesterol level, free fatty acid level, total lipid level, phospholipid level in rats fed high fat diet. According to above results, the authors suggest that ACE is able to be used for the herb of physiological-action.
