[OBJECTIVES] The purpose of this study was to compare plantar flexor force due to different foot strike patterns during running in barefoot and shod condition with and without medial tibial stress syndrome (MTSS). [METHODS] Fifteen collegiate soccer players who volunteered to participate were divided into MTSS group and controls. Participants’ running at 3.3 m/s with and without shoes were recorded with a 12-camera system at a sampling frequency of 250 Hz. Each subject completed different foot strike patterns of running as the forefoot strike pattern (FFS) and rearfoot strike pattern (RFS) were collected. Plantar flexor forces were investigated by software for interactive musculoskeletal modeling. [RESULTS] Normalized plantar flexor forces in barefoot running with the FFS pattern had greater soleus and peroneus brevis muscle forces in the MTSS group than in controls during the first half of stance, although there were no statistically significant differences for other plantar flexor muscle forces between groups and shod running. In plantar flexor forces due to foot strike pattern, the FFS pattern showed higher plantar flexor forces than the RFS pattern. [CONCLUSIONS] The results of musculoskeletal modeling suggest that subjects with MTSS have greater muscle forces of the plantar flexor during running, especially barefoot running with the FFS pattern. It also suggests increased forces in some plantar flexors generated great traction force by repetitively landing on connective tissues in the deep crural fascia causing inflammation at the posteromedial site of the tibia in MTSS.

Papillary thyroid carcinoma (PTC) in children under ten years old is very rare. To date, 18 cases of PTC in children under ten years old (including our two cases) have been reported in Korea. Here, we describe two cases of recurrent PTC with follicular variant and conventional type in an 8-yearold boy and a 7-year-old boy, respectively, and discuss clinicopathologic and molecular characteristics that differ in pediatric patients from adults.

Purpose: In this study, we aimed to measure the effects of emergency department (ED) expansion at a tertiary hospital on overall ED length of stay (LOS). Methods: This study was a before and after study using data from a tertiary medical center in Seoul, a large metropolis. We used electronic medical records and administrative databases obtained from the ED. The control period (before expansion) was defined as from January 1 to April 31, 2016. The study period (after expansion) was defined as from May 1 to August 31, 2016. The number of ED beds increased from 42 to 74 after the expansion. After adjusting individual characteristics and institutional characteristics, multivariate regression analysis was carried out to evaluate the effects of expansion on overall ED LOS. Results: A total of 45,632 patients visited the emergency medical center: 20,592 patients before and 25,040 after the expansion. Although the absolute number of patients increased, the portion of medical patients, portion of non-referral patients, and ambulatory patients decreased during the study period (all p<0.001). Average visit number increased from 170.2 (standard deviation [SD], 27.3) to 203.6 (SD, 21.3) (p<0.001). The overall ED LOS increased from 332.2 (SD, 473.4) to 391.0 minutes (SD, 649.5). After adjusting for potential confounders, we found that ED expansion was associated with an increase in ED LOS by 75.8 minutes (95% confidence interval, 63.5 to 88.2). Conclusion: We found that the ED expansion was associated with a significant increase in ED LOS.
Dome-type carcinoma of the rectum mimicking a submucosal tumor: a case report and literature review

Dome-type carcinoma (DC) has been recognized as a rare variant of adenocarcinoma, which arises in gutassociated lymphoid tissue. It has a specific morphologic feature of a dome-like protrusion associated with lymphoid tissue. We report a case of a DC of the rectum in an asymptomatic 58-year-old male. A 2-cm sized, well-demarcated, round mass masquerading as a submucosal tumor (SMT) was identified in the rectum and was resected by endoscopic submucosal dissection. The tumor was revealed as an adenocarci noma with submucosal invasion of 3,700 µm, which consisted of dilated cystic glands and the lymphoid stroma with reactive germinal centers. On immunohistochemistry, the tumor cells revealed retained expression for mismatch repair proteins. Laparoscopic surgical resection was subsequently performed. DC is considered a distinctive subtype of colorectal adenocarcinoma with characteristic morphology and lowgrade malignant potential. Careful detection of the overlying mucosal lesion is crucial to differentially diagnose DC from SMT.
Medial Tibial Stress Syndrome: Focused on Tibial Fascial-traction Theory and Prevention Strategies

PURPOSE This review article purpose to define and clarify the mechanism of MTSS, especially the tibial fascial-traction theory of MTSS. Additionally, the effectiveness of prevention strategies based on the tibial fascial-traction theory of MTSS was also assessed. BODY Shin splints is a well-known name of MTSS. It is commonly described as pain which occurs in the posteromedial border of the tibia caused by repetitive stresses. Some studies have addressed the tibial fascial-traction theory of MTSS as a possible mechanism in which traction forces are applied on the tibial periosteum though the soleal aponeurosis. Also, traction forces are applied on connective tissues by the soleus, tibialis posterior, and flexor digitorum longus. Prevention strategies are an avoidance of the repetitive stresses, running re-education such as an avoidance the forefoot strike patterns in runners who have risk factors for MTSS. Additionally, shoe modification such as motion control and stability shoes, and strengthening the ankle invertors are recommended. CONCLUSIONS This review article provided an overall review of the tibial-fascial traction theory of MTSS and recommendations for prevention strategies based on this theory.

Does and convenience of booking, accessibility, subsidiary facilities use, golf club employee attitude, charge, caddie’s professionalism and mind by independent variable and conducted multiple regression analysis golf club revisiting by dependent variable whether golf course user’s selection attribute makes effect that keep in mind in golf club revisiting. Result that keep in mind in analysis result accessibility variable and employee attitude variable appeared. Factor that this study verifies difference of selection attribute by golf club user’s type in golf market and gets in golf club revisiting about that analyze actual proof registration point put.
HBV - bDNA 음성 만성 B 형 간염 바이러스 보유자의 간조직검사 소견과 임상적 특징

Background/Aims: It is common in Korea that young men carry HBV-bDNA (branched chain DNA) negative chronic HBsAg, regardless of abnormal aminotransferases. However, histologic findings of these patients were not well-known. The authors studied histologic characteristics of hepatitis activity in these patients and assessed the correlations of aminotransferases and the histologic characteristices of hepatitis activity/fibrosis. Methods: Routine biochemical liver function tests were performed for the 29 young male HBV-bDNA negative chronic HBsAg carriers. Their histologic hepatitis activity/fibrosis was scored. Results: Ninety percent of the patients showed chronic inflammation at liver biopsy, whereas 80% of the parents who had normal aminotransferase showed chronic inflammatory feature in spite of their HBV-bDNA negativity. A significant correlation was found between the levels of aminotransferases and histologic hepatitis activity (lobular activity and portoperiportal activity). The patients with abnormal aminotransferase revealed the higher grade activity than the patients with normal aminotransferase in lobular activity and portoperiportal activity. Conclusions: Most of young male HBV-bDNA negative chronic HBsAg carriers show the histologic characteristics of chronic hepatitis. Some patients could be recovered, but others may show viral replication during their clinical course. (Korean J Gastroenterol 2001;38:419-426)
