        Iliocaval Fistula Presenting with Paradoxical Pulmonary Embolism Combined with High-Output Heart Failure Successfully Treated by Endovascular Stent-Graft Repair: Case Report

        A 51-yr-old man presented exertional dyspnea as a consequence of iliocaval fistula combined with paradoxical pulmonary embolism and high-output heart failure. Endovascular stent-graft repair was performed to cover iliocaval fistula and restore the heart function. After the procedure, dyspnea was improved and procedure related complication was not seen. A 6-month follow-up computed tomography showed regression of pulmonary thromboembolism and well-positioned stent-graft without graft migration, aortacaval communication or endoleak. Stent graft implantation should be considered an alternative of open repair surgery for treament of abdominal arteriovenous fisula, especially in patient with high risk for surgery.

        시뮬라르크로서의 현대 춤 디지털 이미지: 무라드 메르조키의 《Pixel, 2014》을 중심으로

        The arts of dancing has been conducting various experiments and making different attempts to overcome the limitation of expression using advantages of the digital technologies. In this process, the digital image has increasingly gained independent status from the dependent and subordinate level of tool and become the core creating and delivering its meaning on its own, just like the body. Focusing on this phenomenon, the researcher suggests the theoretical basis of existence, independence, and creativity of the digital images shown in 《pixel》 of Mourad Merzouki in the light of the concept of simulacre formulated by Gilles Deleuze and analyzes features of the images in this work. The digital images in 《pixel》 focuses on capture and disclosure of power, which can be the core of the art theory of Gilles Deleuze. Mourad Merzouki formed the digital images of various aspects embedded in ‘Power’ to represent the invisible theme of ‘Power’, structured the concept of ‘Power’ through differences and repetitions among these images, and took it to the cognitive area. This is why the images in his work are not merely accessorial means to expand and strengthen expressions, but independent and substantial ‘Dancing Partners’ that become the center of meaning-creation by working together with human dancers and forming and leading the entire work. As a result, 《pixel》 approaches how the image substantiates itself and creates its meaning in the digital era from the concept of simulacre of Gilles Deleuze, and it shows the image as a way to understand existence and such possibility by overcoming the negativity of the image which is recognized as fabrication and fake.

        발레리나의 드러난 신체와 은폐된 권력에 대한 고찰- 프랑스 낭만발레와 미국 신고전주의발레를 중심으로 -

        Dances before ballet considered the form and meaning of dance as important, not on 'how' the dancing body should be. However, as the ballet began and developed, the perception of the body has changed and the body has been standardized accordingly. This study intends to discuss the new phenomenon of standardization of body in ballet by focusing on this and examine the nature of establishment. Therefore, romantic ballet from France in 19th century, which was the movement and development of ballet, and neoclassical ballet from the United States in 20th century were considered as the subject of analysis. The causes that affected the standardization of the body in this period were examined in political·economic perspective, educational perspective as the transmission of ballet, and cultural perspective with the viewpoint of choreographer and critic. In both cases, the new standard of ballerina's body has developed, but there are significant differences depending on the period and social atmosphere. In case of romantic ballet, the delicate and frail body and soft feminine beauty were emphasized in response to bourgeoisie's tastes, who were the ruling class at that time, while neoclassical ballet of the United States stressed long and straight body with fast and powerful image, symbolizing the development of capitalism in the 20th century. However, both ballets actively engaged in the physical standardization of ballerinas in order to accommodate the needs of enjoying class in terms of its education, planning and choreograph. In consequence, as dancing has transformed from the public art of tradition and festival into the new concept of art as exhibition and display, a new form of the dancer's body has emerged in accordance with the power and viewpoint of the class enjoying it. Ballet may have created a new perception and beauty of the dancer's body, but on the other hand, the body was excessively constrained according to the viewpoint and power, providing the new beginning of the modern dance. 발레 이전의 무용들에 있어 중요한 것은 춤의 형식과 의미이었지 춤추는 몸이 ‘어떠해야’ 하는지는 논외였다. 하지만 발레가 태동하고 발전하면서 몸에 대한 인식이 변화하였으며 그에 따라 춤추는 몸에 대한 정형도 확립되었다. 본 연구는 여기에 초점을 두고 발레 속 신체의 정형화, 규격화라는 새로운 현상에 대해 그것을 견인하고 확립시킨 본질이 무엇인가를 고찰하고자 한다. 때문에 발레의 태동과 발전기인 19세기 프랑스 낭만발레와 20세기 미국의 신고전주의 발레를 분석의 대상으로 삼았고, 이 시기 신체의 정형화에 영향을 끼친 원인들을 정치․경제적 관점, 발레 전승이라는 교육적 관점, 그리고 마지막으로 안무가와 평론가의 시선이라는 문화적 관점에서 살펴보았다. 두 가지 경우 모두 발레리나의 신체에 대한 새로운 정형들이 생겨났지만 서로 간에 시대적, 사회적 분위기에 따라 커다란 차이가 있다. 낭만발레의 경우 당시 지배계층인 부르주아의 시선과 취향에 맞춰 섬세하고 연약한 신체와 부드러운 여성미가 강조된 반면, 미국의 신고전주의 발레의 경우 20세기 자본주의의 발전을 상징하듯 길고 곧은 신체와 빠르고 힘찬 이미지가 강조되었다. 하지만 두 가지 발레 모두 발레 교육이나 기획, 안무의 관점에서는 향유계층의 요구를 수용하기 위해 적극적으로 발레리나의 신체 정형화에 개입하였다고 볼 수 있다. 결과적으로 무용이 전통과 축제라는 대중예술에서 벗어나 전시와 관람이라는 새로운 층위의 예술로 변모하면서 그것을 향유하는 계층의 권력과 시선에 따라 무용수의 몸에도 새로운 형식이 생겼다. 이는 발레가 무용하는 몸에 대한 새로운 인식과 아름다움을 창조했다고 볼 수 있지만, 다른 한편으론 시선과 권력에 맞춰 춤추는 몸을 지나치게 구속하였으며 이는 자유로운 현대무용에 대한 새로운 단초를 제공했다고 할 수 있다.

        진해만에서 물리화학적 환경요인이 식물플랑크톤 색소 및 군집조성의 시공간적 변화에 미치는 영향

        The aim of this study was to investigate the spatial and temporal distribution of phytoplankton biomass and community composition in Jinhae Bay on the southern coast of Korea. Phytoplankton pigment analysis was conducted using ultra performance liquid chromatography (UPLC) were conducted from April to December 2019 at seven stations. Temperature, salinity, and dissolved oxygen (DO) and inorganic nutrients (dissolved nitrogen, dissolved phosphorus, and orthosilicic acid) were measured to investigate the environmental factors associated with the structure of phytoplankton community. Phytoplankton biomass (Chl-a) was the highest in July (mean 15.4±4.3 µg/L) and the lowest in December (mean 3.5±0.6 µg/L). Fucoxanthin was the most abundant carotenoid and showed a similar variation pattern to Chl-a, peridinin, and Chl-b. Phytoplankton community composition analysis showed that diatoms were a predominant group with an average abundance of 70% whereas chlorophytes, cryptophytes, and dinoflagellates often appeared with lower averages. Further, the dominance of diatoms was closely correlated with water temperature and N:P ratio, which might be influenced by high temperatures in the summer and nutrient loading from the land. Additionally, freshwater and nutrient input by rainfall was estimated to be the most important environmental factor. Hence, the spatial and temporal variations in the composition of phytoplankton pigments and phytoplankton community were correlated with physicochemical and environmental parameters. 본 연구에서는 ultra performance liquid chromatography(UPLC)를 이용하여 진해만의 식물플랑크톤 생체량 및 군집구조의 시공간적 분포에 미치는 환경요인의 영향을 조사하였다. 이를 위해 2019년 4월에서 12월까지 총 5회에 걸쳐 7개 정점에 대한 식물플랑크톤 색소분석과 수온, 염분, 용존산소(DO), 영양염(DIN, DIP, Si(OH)4) 등의 환경요인 분석을 행하였다. 조사기간 중 식물플랑크톤의 생체량(Chl-a)은 7월(평균 15.4±4.3 µg/L)에 가장 높았고, 12월(평균 3.5±0.6 µg/L)에 가장 낮았다. 보조색소의 경우 fucoxanthin이 가장 많이 검출되었고 그 다음으로 peridinin, Chl-b 순으로 나타났으며, 이들의 월 변동은 Chl-a와 유사한 경향을 보였다. 식물플랑크톤 군집분석결과, 규조류가 평균 70%로 가장 우점하였으나, 일부 녹조류, 은편모조류, 와편모조류가 출현하기도 하였다. 우점종인 규조류는 특히 수온 및 N:P ratio와 밀접하게 연관되어 있어서 여름철 고온환경 및 육상으로부터의 영양염 유입에 민감하게 반응하는 것으로 추론되었다. 또한 식물플랑크톤 색소 및 종조성은 전반적으로 계절에 따른 물리화학적 환경요인의 변화 및 지형적 특성과 연관되어 있으며 강우로 인한 담수 및 영양염 공급에 큰 영향을 받는 것으로 추정되었다.

        좌심실 수축 기능 보전 심부전증의 치료

        More than 50% of patients who are diagnosed with heart failure have preserved ejection fraction (HFpEF), and they have an equally poor prognosis when compared to patients with heart failure with reduced ejection fraction (HFrEF). However, a comprehensive understanding and awareness of heart failure with preserved ejection fraction is still limited and there are currently no optimized treatments to improve morbidity and mortality in these patients. This review summarizes the differences in the epidemiology, pathophysiology, diagnosis, and prognosis between HFpEF and HFrEF. We also review current management strategies of HFpEF patients according to evidence-based treatment guidelines. (Korean J Med 2015;88:135-141)

        신경섬유종증과 관련된 폐고혈압은 심각한 기능적, 혈역 학적 장애를 초래할 수 있다. 따라서 신경섬유종증 환자에서 호흡곤란, 기절, 피로 등 폐고혈압의 증상이나 징후가 나타 난다면 임상의들은 가능한 조기에 심초음파 등의 검사를 하여 폐고혈압을 발견하고 치료를 시작할 수 있도록 해야 한다. 또한 신경섬유종증과 관련된 폐고혈압의 병태생리적 이해를 위해 체계적이고 규모 있는 연구가 필요하며 이를 바탕으로 생존율 향상에 도움이 되는 추가적인 치료법에 대한 논의가 이루어져야 할 것이다. Neurofibromatosis type I is a genetic disease caused by mutations in the neurofibromin 1 (NF1) gene. Although it is characterized by a number of distinct clinical features, including cafe au lait macules, freckling in the axillary or inguinal regions, neurofibromas, and Lisch nodules (iris harmartomas), it can affect all physiological systems in the body [1]. Neurofibromatosis-related pulmonary hypertension has also been reported, and some patients showed a poor prognosis despite having received proper medical treatment [2-4]. We herein describe a case of pulmonary hypertension in a patient with neurofibromatosis type I who had no identified risk factors of pulmonary hypertension. To our knowledge, this is the first such report in Korea. (Korean J Med 2013;85:521-525)

        다수의 경우 골수성 육종은 진단 내리기 쉽지 않고 감염 이나 다른 고형 종양으로 오진하기 쉽다. 특히 이전에 혈액질환의 기왕력이 없는 경우는 더욱 그러하다. 임상의들은 고형 종괴에 대해 검사를 진행함에 있어서 진단이 명확치 않은 경우 드문 질환이긴 하지만 골수성 육종의 가능성을 염두 하여야 하며 조직 검사를 통한 진단을 위해 노력을 기울여야 할 것이다. 이전 혈액 질환의 기왕력이 없던 환자에서 골수성 육종이 진단되었을 경우에는 동반되어 있을지 모르는 혈액 질환에 대한 조사가 동반되어야 한다. 또한 이전 완전 관해를 이루었던 환자에서 골수 외 재발이 확인되었을경우에는 골수 외 재발이 골수 재발과 동반되거나 선행하여 나타날 수 있으므로 골수에서의 재발 여부에 대한 평가도 다시 이루어져야 할 것이다. Myeloid sarcoma is an uncommon extramedullary tumor of immature myeloid cells or myeloblasts. It may occur alone or concurrently with an underlying hematological malignancy. Although it can develop anywhere in the body, common sites include bones, particularly the skull and vertebra, soft tissues, and lymph nodes. However, there have been few reports of myeloid sarcoma occurring in the respiratory system, especially the large airways. We describe a case of endobronchial relapse of acute myeloid leukemia in a patient who achieved complete remission after allogeneic stem cell transplantation. To our knowledge, this is the first such report in Korea. (Korean J Med 2013;85:318-323)

        몸의 확장성과 섹슈얼리티 이미지 연구

        This article aims to figure out what roles the body plays in generating and interpreting sexual images and what the characteristics of the body are in the intervention to it. However, unlike the reality that sexual images are excessive, theoretical approaches to the proper understanding of sexuality or the expressive methods of it still lack, and particularly, the study on ‘the body’ centering around sexual images is very rare; thus, this study not only analyzes actual works but conducts theoretical tasks, too. Therefore, this study largely consists of theoretical discussion about what the sexuality is and how it works and the analytic process to figure out the body’s roles in the process of generating sexual images and acquiring meaning through movies and dance works. In the theoretical discussion, this paper introduces the concept and theory of sexuality. Based on that, this study divides sexuality by its functions into the common sexuality which extends and reproduces the ruling discourse of the society, evolving sexuality which expresses resistant discourse, and the mixed sexuality which either coexists or is interpreted totally differently. These are adopted as the actual criteria to analyze the works. In the analysis process, this paper analyzes three movies, 「Original Sin」, 「Tomb Raider」, and 「Girls’ Night Out」, and three dance works, 「Romeo and Juliet」, 「Carmen」, and「Groosland」to figure out how they produce the diverse sexual images expressed in them and how they take meaning in the association with the body. In this process, it was found that sexual images are generated not just by the actor’s or the actress’ body and the dancer’s body but by organic harmony with various objets appearing on the screen or stage such as costume or props, too. This study views the characteristics of the body organically harmonized with surrounding environment as ‘extensibility’. Also, the fact that as the objects of expansion are more, it is easier to interpret the images and as it gets fewer, the symbolic meaning becomes stronger and it is harder to interpret them means that the body’s extensibility is not just the source of sexual images but the passage of interpretation as well. Thus, it can be said that the fact that sexual images are generated and interpreted around the ‘extensibility’ of the body provides constant directivity to the creator’s position about how to realize sexual images and also provides information to the receptor’s position about how to interpret the sexual image. In other words, it can be said that the body’s extensibility provides ways for dance art centering around the body to communicate with people.
