        盆植栽培에 알맞는 무궁화 품종의 선택

        In order to select the most suitable cultivars of Hibiscus syriacus for pot culture, native and introduced cultivars were cultured in pot in Seoul, Korea. And growth and flowering characteristics of the Hibiscus syriacus were investigated for two years. 1.The native cultivars which was large size and large number of flower were 'Okttokky', 'Hanseo', 'Gojumong', 'Youngkwang' and 'Nanpa', and introduced ones were 'Sonde', 'Ilpyandansim', 'Hanboram', 'Hansarang', 'Sir de Charles Breton' and 'Sanchyeonyeo'. 2.The cultivars lasting flowering period(80-90 days) were 'Bedal', 'Hanseo', 'Sehan', 'Saimdang', 'Gojumong', 'Hongsoon' and 'Chilboasadal', and middle ones(70-80 days) were 'Seohohiang', 'Youngkwang', 'Okttokky', 'Parangse' and 'Nanpa'. And shorter ones(60-70 days) were 'Soonjung' and 'Sulark'. And the most earlist flowering cultivars were 'Parangse' (5th, Jul.), and the latest flowering ones were 'Chilboasadal', 'Youngkwang' and 'Hanseo'(28th, Oct.) . The cultivar with longer flowering period(85-95 days ) in introduced cultivars were 'Monstrous Plenus', 'Boule de Fue', 'Ilpyandansim' and 'Sanchyeonyeo', and middle ones (65-85 days) were 'Hanboram', 'Sonde' and 'Pompon'. And shorter ones(55-65 days) were 'Hamabo', 'Sir de Charles Breton' and 'Hansarang'. And the most earlist flowering cultivars were 'Sonde', 'Hansarang' and 'Sanchyeonyeo'(3th, Jul.), and the latest flowering ones were 'Boule de Fue' and 'Hamabo'(30th, Oct.). 3.The tallest in plant height among native cultivars was 'Hongsoon'(65cm), and the shortest one was 'Sairruang'(37.5cm). And the tallest one among the introduced ones was 'Hamabo'(55cm), and the shortest one was 'Monstrous Plenus'(34cm). 4.'Hanseo', 'Gojumong' and 'Nanpa' in native cultivars, and 'Ilpyandansim', 'Hansarang', 'Pompon', 'Sanchyeonyeo' and 'Bicolor' in introduce ones were considered as suitable cultivars far pot culture in Seoul, Korea.

        급성 내피손상을 받은 백서 대퇴정맥에서 냉동요법의 효과

        In the case of crush injury, the failure rate of replantation of free tissue transfer is high. Recently, the freezing of intact and repaired microarteries has been known to relieve spasm without inducing thrombosis in vivo. Therefore, the author thought that the vascular freezing of crushed vein showing only unrecognized endothelial damage after adequate debridement of a damaged vessel can also relieve spasm without inducing thrombosis and improve the patency rate of microvascular anastomosis. In this study, the process of vascular freezing was investigated in veins with acutely damaged endothelium using ethly chloride in a rat model. Two experiments were performed in 30 Sprague-Dawley rats, weighing about 250 gm. In the first experiment, a new crush model was established on the femoral vein of the rats. The degree of vessel injury was evaluated as crush energy. The proper crush energy was determined only by the endothelial damage without inducing thrombosis(0.037 J). In the second experiment, femoral veins were crushed with the energy of 0.037 J on the left side and then frozen after inflicted crush injury with the same energy on the right side. The gross and pathologic findings and patency rates were observed at postoperative day 2, 10 and 30. Patency in both the crushed and crush-freezing group was 100%. Pathlolgic findings in the crush-freezing group were more severe loss of the intima and most parts of the media, as well as cellular depopulation of the media when compared to the crushed group. Microthrombi adherent to the wall were demonstrated in several crush-freezing specimens, but the crush-freezing group showed relief of crush-induced vasopasm and endothelial regeneration. In conclusion, the freezing of microveins with acute endothelial damage relieves spasm without inducing thrombosis. Therefore, this study presents the posslbility of clinical application of vascular freezing in a case of crush injury requiring microvascualr anastomosis.

        헤파린이 수근관 증후 군 환자의 횡수근 인대에서 추출한 섬유아세포의 콜라겐, MMP 및 TGF-β 동위형 발현에 미치는 영향

        Purpose: Noninflammatory synovial fibrosis has been noted for main causal factor of carpal tunnel syndrome (CTS). Recently, there are some reports that heparin have not only anti-coagulative effect but also anti-inflammatory and anti-fibrotic potential and have an effect on interstitial pulmonary fiborosis. Authors examined whether heparin affects pathogenesis of CTS.Methods: First, heparin was administered to fibroblast that was cultured from patient's transverse carpal ligament. Secondly, we evaluated the expression from genes of type I, III collagen, TGF β isoforms and MMP. Fibroblasts were isolated and cultured from transverse carpal ligaments of 5 patients with CTS. Heparin (0, 1, 10,100μg/ml) was administered to cultured fibroblast and reverse transcription PCR for mRNA expression of type I, III collagen, TGF-β isoforms and MMP was done.Results: Heparin suppressed gene expression of type I, III collagen and TGF-β1, β3 but promoted gene expression of TGF-β2 and MMP-2.Conclusion: Heparin directly suppress gene expression of type I, III collagen. But, It is undetermined that heparin can present it's effect mediated by TGF β isoforms or MMP.

        액취증에서 Superficial Suction과 Subdermal Excision의 치료효과 비교

        Osmidrosis develops in many young people due to the apocrine gland becoming active after the adolescent period and can cause difficulty in social activities and personal relationships. We compared 2 surgical treatment methods for osmidrosis, superficial suction and subdermal excision, to determine how many of apocrine glands could be removed and examine if a significant difference was present in treatment outcome. The subjects used for the present study included 62 patients of whom 46 patients underwent subdermal excision and 16, superficial suction. We counted the total number of apocrine glands in tissue samples obtained using those 2 methods, fixed in formalin and treated with usual histologic treatment. We calculated the number of apocrine glands per unit area by measuring the axillary area of the surgery site determined before surgery. The patients who underwent surgery using those 2 methods were divided into satisfactory group and unsatisfactory group by evaluating the results subjectively and objectively after surgery. The satisfactory rate was 95.7% in 46 patients who underwent subdermal excision and 81.25% in 16 patients who underwent superficial suction. An average of 127.82 apocrine glands/ cm2 were removed through subdermal excision and 72.71 apocrine glands/cm2, through superficial suction. We determined that more apocrine glands were removed with a statistical significance using subdermal excision. However, considering the result that 81.25% satisfactory rate was seen despite 57% of apocrine glands being removed with superficial suction compared with subdermal excision, superficial suction would be also effective in reducing the number of apocrine glands to below a threshold.

      • 전기화상 환자에서 견관절 절단부 재건을 위한 상지 근피부 유리피판술의 증례 보고

        The pedicled fillet flap concept has been successfully applied in both the upper and lower extremities for the treatment of difficult wounds. However, in case of complete extremity amputation in eletrical burn patient, the transfer of pedicled flaps from the amputated part is not possible. In such instances, we have designed total arm musculocutaneous free-fillet flaps from the amputated limb to provide wound coverage, when replantation of the amputated part was contraindicated. now we present such a case. This technique allows immediate wound coverage without the morbidity of an additional donor site. The flap provides the ideal combination of large surface area, muscle bulk, and long vascular pedicle. It can be dissected rapidly to minimize ischemic time and could therefore be applicable to traumatic forequarter amputations.

      • 전기화상 환자에서 전완부피판을 이용한 수부의 재건

        The hand is a frequently affected area in high voltage electrical burn injury as an input or output sites. Therefore, early debridement and synchronous flap coverage are generally accepted as a primary treatment of several electrical burns complicated by exposure of tendons, neurovascular structures, and bones. So, in order to establish convenient, promising methods for the reconstruction of hand defects in electrical burn patients, we performed various reverse forearm flaps. From March 1997 to February 2000, we reconstructed 12 cases of hand defects in high voltage electrical burn wounds with reverse forearm flaps. Reverse radial fasciocutaneous flap were 3 cases, reverse ulnar fasciocutaneous flap were 3 cases, reverse ulnar fasciocutaneous flap and STSG were 4 cases, reverse ulnar fascial flap and STSG were 2 cases. We successfully reconstructed hand defects in all 12 cases, and obtained following conclusions. 1 Various reverse forearm flaps provide well vascularized, profitable tissues and they require short operative time with relatively easy procedures. 2. Reverse fascial flap and STSG, reverse fasciocutaneous flap and STSG provide thin flaps with good aesthetics and minor donor site morbidity.

        Three-Dimensional Z-Plasty(Yoon's Method)를 이용한 내안각 췌피 성형술

        The presence of epicanthal folds in Asian eyelids is one of their unique features, in combination with the lack of supratarsal folds. Western culture has influenced many people to prefer to eliminate the prominent epicanthal fold. A number of surgical techniques have been suggested for their correction. However, difficulty with design, excessive and prominent scarring of medial canthal and nasal area, recurrence, and rigidity of application are potential problems associated with many procedures. This paper describes a epicanthoplasty with three-dimensional Z-plasty. Through epicanthal fold incision, the medial canthal tendon is medially advanced and sutured to the periosteum of nasal bone. After the transposition of the flaps, trimming of the flap is usually required. Three-dimensional Z-plasty creates the attractive eyes. From March, 1995 to March, 1997, the technique was applied to 37 patients with epicanthal fold and was performed with or without double-fold operation. There were 34 females and 3 males with ages ranging from 17 to 30 years. Through 2 years follow-up, this technique has delivered esthetically good results with minimal postoperative scar and could made the reduction of ICD from 40 ± 2.13 mm to 34 ± 1.98 mm. The advantages of epicanthoplasty procedure using three-dimensional Z-plasty(Yoon's method) are as follows; 1) simple in design 2)minimal postoperative scar in the medial canthal area 3) versatile in its application 4) no recurrence 5) no hypertrophic scar 6) preserving ethnic identity.
