        Zenker 게실 환자에서 내시경적 접근법을 통해 Energy Device를 이용한 게실절개술 1예

        Zenker’s diverticulum (ZD) is an abnormally protruding space in the Killian triangle due toweakness between the inferior pharyngeal constrictor muscle and cricopharyngeal muscle. Arare disease that is found more often in the West than in the East, it occurs in the elderly, inducingdysphagia, globus symptom, reflux and etc. For the treatment, various transoral endoscopictechniques have been developed over the past 20 years: these include flexible and rigidendoscopy are used and several devices like scissors, staplers, CO2 laser and energy device. We report here the first case using the endoscopic energy device assisted diverticulostomy ofZD in Korea. Transoral endoscopic treatment is less invasive than classical transcervical approachtreatment, showing better clinical course and results. Both rigid endoscope and energydevice are also very familiar to head and neck surgeons, so this approach can be one of thebest treatments for ZD.

        양성후두점막질환과 대사증후군의 상관관계

        Background and Objectives:The aim of this study is verify the correlation between benign laryngeal mucosal disease and metabolic syndrome. Materials and Method:Data for patients diagnosed with benign laryngeal mucosal disease and metabolic syndrome from 2006 to 2015 were selected for analysis from the National Health Insurance Service database. Results:The prevalence of Metabolic syndrome was 2,179,785 out of 6,437,051 patients (33.86%). The prevalence of benign laryngeal mucosal disease was 516,594 out of 6,437,051 patients (8.03%). Metabolic syndrome was a risk factor for benign laryngeal mucosal disease [hazard ratio: 0.99, 95% confidence interval: 0.984-0.997] after adjusting for age and other variables including age, gender, smoking status, alcohol intake, exercise, body mass index, and diabetes. The number of metabolic syndrome components was also risk factor. Conclusion:Metabolic syndrome was related to the incidence of benign laryngeal mucosal disease. However this correlation did not seem to be high.

        Takayasu 동맥염의 경피경관 혈관성형술 : 합병증과 장기추적소견

        Purpose : To evaluate the complications and results of long-term follow-up of percutaneous transluminalangioplasty(PTA) for the treatment of stenosis related to Takayasu arteritis. Materials and Methods : BetweenDecember 1985 and August 1997, 21 patients(two men, 19 women; age range, 10-48 years) with Takayasu arteritisunderwent PTA. There were 37 instances of stenosis(in 20 renal arteries, 10 descending aortas, 3 subclavianarteries, 3 common carotid arteries, and 1 axillary artery). In order to detect restenosis, all patients werefollowed up angiographically and clinically ; the period of this ranged from 8 to 121(mean 47) months. One or tworedilatations were performed for restenotic arteries, and the complications and results of long-term follow-upwere determined. In order to evaluate the effectiveness of PTA in Takayasu arteritis, primary and secondarypat-ency were calculated by the Kaplan-Meier method. Results : Among 37 stenotic vessels, 29 were sucessfullydialted and six partially so. Complications were aortic dissection(n=4) in aortic PTA, aortic dissection(n=1) inrenal PTA, intimal tears(n=6) in renal PTA, and hemorrhagic infart(n=1) in common carotid PTA. Restenosis occurredin 17 vessels(49%), and 12 vessels were restenosed within one year of PTA. Among 17 restenotic vessels, 14 wereredilated with PTA, but after further PTA, five were restenosed. The Kaplan-Meier method showed primary patency of62% one year after redilatation, and 46% five years after, while for secondary patency, the corresponding figureswere 94% and 76%. Conclusion : In PTA of Takayasu arteritis, the possibility of restenosis is high, butredilatation can increase the patency rate. Intimal tearing, such as aortic dissection, may occur in PTA ofTakayasu arteritis.

        전자소자 열분석을 위한 열반사 현미경 기술

        With the advent of technologies based on big data, the trend of electronics towards high performance and high integration density continues. However, this development of electronics suffers from overheating issues, which seriously threaten the reliability of the devices. To develop effective strategies for thermal management, it is crucial to accurately evaluate the temperature distribution and design the heat dissipation path within the device in the operating condition. This paper introduces thermoreflectance microscopy that can observe the temperature distribution of a device with high spatial and temporal resolutions in a non-contact way. Specifically, the working principle and various forms of thermoreflectance microscopy are presented along with the latest research trends to improve the temperature, space, and time resolutions. We further review several examples in which thermoreflectance microscopy is applied to investigate the temperature and thermal characteristics of electronic devices. 최근 거대 데이터 기반의 미래 기술이 발전함에 따라 전자 소자의 고성능 및 고집적화 추세가 지속되고 있는데, 이는 심각한 발열 문제를 수반하여 소자의 신뢰성을 위협하는 주요 요인으로 작용하고 있다. 효과적인 열관리 대책을수립하기 위해서는 소자의 구동 환경에서 온도 분포를 정확히 평가하고 방열 경로를 설계하는 것이 필요하다. 본 논문에서는 소자의 온도 분포를 비접촉 방식의 높은 공간 및 시간 분해능으로 관찰할 수 있는 열반사 현미경 기술을 소개한다. 구체적으로 열반사 현미경의 원리 및 구동 형태를 알아보고, 온도, 공간, 시간 분해능 향상을 위한 최신 연구 동향과 다양한 전자 소자의 온도 및 열적 특성 분석에 적용된 사례를 함께 살펴본다.

        Long-term Treatment with Oriental Medicinal Herb Artemisia princeps Alters Neuroplasticity in a Rat Model of Ovarian Hormone Deficiency

        Artemisia princeps (AP) is a flowering perennial used as a traditional medicine and dietary supplement across East Asia. No study has yet assessed its effects on synaptic plasticity in hippocampus and much less in a model of ovarian hormone deficiency. We examined the influence of chronic oral AP ethanol extract treatment in ovariectomized rats on the induction of long-term depression in a representative synapse (CA3-CA1) of the hippocampus. Ovariectomized rats demonstrated lower trabecular mean bone mineral densities than sham, validating the establishment of pathology. Against this background of pathology, AP-treated ovariectomized rats exhibited attenuated long-term depression (LTD) in CA1 relative to water-treated controls as measured by increased field excitatory post-synaptic potentials (fEPSP) activation averages over the post-stimulation period.         Electrohydrodynamically Atomized pH-Responsive PLGA/ZnO Quantum Dots for Local Delivery in Lung Cancer

        Targeted drug delivery to cancerous tissues using nanomaterials is a challenging task that is fast gaining attention. Here, we report the fabrication of ZnO quantum dots (QDs) encapsulated in the biodegradable copolymer poly(DL-lactideco- glycolide) (PLGA) nanoparticles using an electrohydrodynamic atomization process to enhance cytotoxicity against the metastatic cancer A549 cell line. The encapsulation and the small size increased the cellular uptake of the PLGA/ZnO QDs. Furthermore, the PLGA/ZnO QDs are pH-responsive and can therefore target cancerous cells based on the acidic tumor microenvironment, leaving healthy cells less affected. PLGA/ZnO QDs induced cell death and obstructed cell invasion and migration at low concentrations (7.5 μg/mL). We verified these activities through thiazolyl blue, tetrazolium blue (MTT), scratch assay, and clonogenic assays on metastatic A549 cells, with normal human lung cells, Human Small Airway Epithelia Cells (SAECs) as the controls. Thus, PLGA-encapsulated ZnO QDs, owing to their selective localization, ability to enter cells, cytotoxicity, and retention effect, could provide a potential platform for tumor-directed drug therapy.
