지주막하출혈에 의한 지연성 허혈신경장애의 진단에 있어서 경두개도플러 검사의 신뢰도

Objective : The reliability of transcranial doppler sonography(TCD) for predicting delayed ischemic neurologic deficit( DIND) was investigated in patients with aneurysmal subarachnoid hemorrhage(SAH). Methods : The velocity of blood flow through the middle cerebral artery was measured by TCD in 70 patients treated surgically within 72 hours due to ruptured cerebral aneurysm. A correlation between measured maximal mean blood flow velocities and clinical factors including age, hypertension, Hunt-Hess grade, Fisher grade, DIND, and outcome was made. Results : An age-dependent reduction of the measured maximal mean velocities was found(r=-0.4043, p<0.001). Flow velocities in hypertensive patients were significantly lower than in the normotensive individuals(two-tailed T test, p<0.05). There was no significant difference between the flow velocities and evaluated other clinical factors. When the flow velocities of 14 patients who developed DIND were compared with those of patients without deficits, no significant difference was seen. A significant increase in flow velocities in the days before the onset of DIND was found only in 4 of 14 cases. High flow velocities did not necessarily mean impending neurological deficits : 6 of 70 patients tolerated flow velocities over 160cm/s. Conclusion : We suggest that TCD readings have limited utility for predicting DIND following SAH, especially in older or hypertensive patients. More direct measurements of blood flow are necessary to adequately predict which patients are at high risk of DIND.

According to Hessen's theory, value is always value for anyone. If we say X has value, so it means X has value for anyone. Therefore he insists that the intrinsical essence of value is the subject-relation. But the subject-relation means that value has relation with the universal human spirit. According to his theory, there is no value without spirit. So to speak, there is value because there is universal human spirit. The universal human spirit means the spirit of heart feeling value. Man lives to experience value by heart. This heart-spirit of value-emotion is said by him what the innercst structure of human beings is. In shorts, man has the spirit of feeling value. And Hessen wanted to elucidates the meaning of value by analysing value-phenomenon. The phenomenon of value has also the relation with the phenomenon of human spirit. Man lives to evaluate all things he meets in the world. Man can not live without evaluating. As evaluating, men feels and experiences value with his inner heart. And what makes man feel value is value-quality. The whole-total of value-quality is said value-idea by Hessen. Hessen insists that experiencing value, value-quality and Value-idea is the three sides of value-phenomenon. The three sides of value-phenomenon have the relation with the phenomenon of universal human spirit. Therefore, he says that the value-relation with univeral human spirit is the essence of value and being-form of value.
편평족 장애자의 골반회전각 분석을 통한 발보조기 효과에 관한 고찰

인체는 발의 근육과 관절의 상호작용을 통해 올바른 보행 자세를 유지하게 되는데, 최근 현대인들에게서 일반적으로 나타나는 운동시간의 감소는 체중증가와 근육 및 인대의 약화를 유발하고 발의 지지구조를 불안정하게 하며 편평족의 증가를 초래하고 있다. 대부분의 편평족 장애는 보행시 좌우 비대칭에 의한 비정상적 보행으로 인해 슬관절의 내측, 고관절의 외측, 요천추부의 통증을 유도하게 되며, 보행 시 에너지의 소비가 증가되고, 기능적 능력은 현저히 감소하게 되는 원인이 된다. 본 연구는 편평족 장애인들의 몸의 비대칭과 좌·우 불균형이 비정상적인 보행의 원인으로 작용하고 통증의 시발점이 될 수 있음에 주목하여 보행 시 좌·우측 골반 회전각도 변화를 주요 평가 파라미터로 선정 평가함으로써 편평족 장애인에게 기능성 발보조기의 효과를 추정하였으며, 그 결과 발 보조기의 착용효과로서 보행 중 전체적인 골반회전각도의 범위인 10 도 내에서 양쪽의 골반 회전각의 차이를 현저하게 감소시켜 줄 수 있음을 확인하였다. The purpose of this study is to investigate the effect of Biomechanical Foot Orthoses (BFO) over the pelvic transverse motion and translation for the individuals with flatfoot. Initially radiological examination and biomechanical examination were performed for 14 healthy adults, aged 20 to 25, to fabricate the BFO to control foot pronation and pelvic range of motion. 3 Dimensional gait analysis (Vicon system) were followed to analysis and measure the difference of pelvic transverse motions. Those are internal and external pelvic motion before and after the wearing of BFO. A paired t-test was used to analysis the data and the test results are as follows; 1. The difference of pelvic internal and external rotation before and after the wears of BFO was an average of 0.72 (before BFO : 3.70, after BFO:2.98) is an average of decrease 19.5%. 2. No significant differences in total range of motion (ROM) pelvic rotation before and after the BFO were found. However there is a sight increase (0.07) in left pelvic internal rotation and a decrease tendency (0.35) in right external pelvic rotation. These results suggested that in order to minimized the asymmetry and imbalance of the pelvic transverse motion, BFO were effective (p<0.05) Therefore to pursue the healthy and vibrant walking life, the results of the study can be used as basic reference for the design of BFO for flatfoot.
습식법에 의한 Hydroxyapatite 분말제조와 그 분체특성

The hydroxyapatite powders were prepared by the reaction of Ca(CH3COO)2$.$H2O and H3PO4 in aqueous solution. Effect of Ca/P mole ration and pH value on the powder characteristics were investigated. For phosphoric acid was dissociated to PO43- ion above pH 10, the preparation condition of hydroxyapatite by H3PO4 solution had this value. Dried powders were agglomerated each other. The mean agglomerate particle diameter was minimum when the condition was pH 11, Ca/P=1.75 and its specific surface area was 97㎡/g. All powders had poor crystallinity and small CO2. It prevented CaCO3 from using aspirator in preparation and maturing time. In most cases, hydroxyapatite was decomposed into ${\beta}$-TCP at more than 800$^{\circ}C$, into ${\alpha}$-TCP at 1200$^{\circ}C$. However non-decomposed hydroxyapatite was remained in Ca/P=1.85.
