        전문간호사 간호행위에 관한 간호수가체계 개발

        Purpose: This study was carried out to deveop nursing costs and to substantiate the application of appropriate activity-based nursing costs for the current clinical nurse specialists. Method: The study sample was a group of 8 home health care team of the tirtiary Hospital in Korea. The data was collected from September 2003 through December 2004. The statistical analysis was done by SPSS PC 11.0 program and calculated mean and the standard deviation. In Lee(2003)'s nursing activity analysis, nursing activities were classified into two major classification, 19 domains and 70 activities. Each activities was calculated using its work validity, physical effort, psychological effort, stress as a intensity, and the necessary time. The simple work costs was calculated the work wage per minute and the necessary time. The work load intensity was calculated using the work wage per minute and the necessary time work load intensity/100. Results: In this study, the work wage was 283 won per minute. The work validity ranged from 2.71-4.00. The highest simple work cost/work load intensity cost was 12,735won/47,374won for research activity, 10,700won/27,499won for bedsore care, and 9,727won/35,114won for deathbed care. The lowest simple work cost/work load intensity cost was 2,123won/2,038won for intramuscular injection, 2,210won/2,166won for hypodermic injection, and 2,210won/1,547won for a application of medicine(or ointment or cream). Conclusions: It revealed that the nursing cost should be considered validity, physical effort, psychological effort, stress as a intensity, and necessary time. Therefore, It is necessary to calculate nursing cost systematically based on activities.

        소설과 연극의 이야기하기(strorytelling)

        콘텐츠의 수요가 기하급수적으로 증가하면서 같은 이야기지만 다른 방식으로 새롭게 이야기하는 스토리텔링이 다양한 분야에 걸쳐 널리 활용되고 있다. 이러한 이야기 매체의 전회는 이야기를 지각하고 인식하는 태도를 바꿔 이야기 자체보다는 같은 이야기를 어떠한 방식으로 다르게 표현하고 있는가에 주목하게 했다. 그리고 이는 주지하는 바와 같이 이야기를 담아내는 그릇인 매체 환경과 새롭게 변형하는 재창작자의 시각에 크게 영향을 받게 된다. 즉, 기존의 이야기와 그것을 변주한 작품을 비교 연구하는 것은 각각의 매체가 지닌 성격을 이해하는 것이자 창작자들 사이의 시각 차이를 살피는 일이기도 하다. 본고에서는 분단이라는 특수한 한국적 상황을 바탕으로 한 김영하의 소설 『빛의 제국』을 이방인의 시각에서 새롭게 변주한 동명의 연극 〈빛의 제국〉의 이야기하기 방식을 비교하였다. 이를 통해 소설과 연극을 작동시키는 메카니즘은 무엇이며 서로 다른 배경을 지닌 두 창작자들이 분단을 바라보는 시각 차이를 살펴보았다. 김영하의 소설 『빛의 제국』은 남파공작원이라는 이색적인 소재를 다루고 있기는 하지만, 소설에서 주인공 김기영이 남파공작원이라는 사실이 그다지 중요하게 다뤄지지 않는다. 물론 그와 같은 내력이 그의 현재를 만들어낸 중요한 인자이기는 하지만, 소설에서 주요하게 다루고 있는 것은 비로소 단절되어 있던 과거를 인정하고 자기의 서사를 완성함으로써 스스로의 욕망에 솔직한 현존재로서의 실존을 깨닫게 되는 여정에 있다. 그리하여 소설은 그가 지금 여기 있는 시공간을 낯설게 하여 생경한 모든 것들에서 과거를 떠올리고 망각했던 자기를 추체험하는 방식으로 이야기를 풀어내고 있다. 이에 비해 연극 〈빛의 제국〉은 자기 서사의 완성보다는 개인의 서사 속에 담겨 있는 공동체의 서사를 복원하는 데 주력한다. 연극이 지닌 현장성과 쌍방향성을 바탕으로 지금 여기에 함께 하는 우리들의 집단적 트라우마를 이끌어냄으로써 분단에 대해 재인식하도록 한다. 이러한 분단에 대한 재인식은 세계 유일의 분단국가이면서도 이에 대한 인식이 없는 한국인들을 낯선 이방인의 시선에서 바라보았기에 가능한 일이라 할 수 있다. 분단의 시간이 길어질수록 현재의 삶에 익숙해진 우리는 어느새 우리가 분단국가라는 사실을 잊은 채 살고 있다. 그것이 우리 모두의 삶을 형성하는 데 있어 근간이 되고 있음에도 말이다. 이처럼 연극 〈빛의 제국〉은 이방인이라는 낯선 시선을 투과하여 분단이 아픈 상처이기는 하지만, 우리 모두가 함께 치유해나가야 할 우리 모두의 과거이자 현재이며 미래라는 사실을 상기하도록 이끈다. As developments in science have created numerous media, the demand for stories has increased. However, creating stories is something that machines cannot do; therefore, they are not replacements for humans in this context. Thus, stories cannot be written at high speeds despite developments in civilization. These days, a common storytelling technique is to change the goal and meaning of an existing story so that they are appropriate for the characteristics of media. This trend has become so popular that we often find it in the lines of characters in TV drams. When we take a look at stories that are retold, we focus on the way they unfold rather than on the narratives themselves. Techniques for telling stories are greatly influenced by viewpoints of the recreators whose work may cause the same stories to be understood differently or the focus of stories to change, resulting in different forms of expression. Young-ha Kim’s novel, The Empire of Light, uses the unique material of a North Korean spy; however, the fact that the main character—Ki-young Kim —is a spy is not considered very important in the novel. His identity is naturally one of the important factors that created his present status, but what is given more weight in the novel is the acknowledgment of his past, which was severed. He completes his own narration, realizing his existence as a being who can be honest about his desires. The novel unfolds in a way that defamiliarizes the reader with the time and place of the main character’s current existence by having him recall the unfamiliar past and relive forgotten experiences. In comparison, the play, The Empire of Light, focuses more on restoring the narration of community than on completing one’s narration. Plays convey the sense of being on-the-spot or bidirectional, capturing the collective trauma of an audience and making us aware again of the divisions in our nation. This reawakening regarding division is possible because the play resorts to the perspective of a stranger who views Koreans as living in the only divided nation in the world without being aware of this fact.

        사람 망막아세포종 세포에 발현된 P2Y11 수용체의 확인

        Purpose: The present study aimed to identify the characteristics and physiological function of the P2Y11 receptor, a receptor likely expressed in human retinoblastoma cells. Methods: We measured possible P2Y11 signaling in WERI-Rb-1 cells using a Ca2+ imaging technique and RT-PCR. Results: 1) 10 ?M ATP elicited a strong but transient increase in Ca2+ in the WERI-Rb-1 cells, and this Ca2+ rise was well maintained after external Ca2+-depletion. 2) ATP-induced Ca2+ response arose entirely through Ca2+ mobilization. 3) P2Y11 agonist (BzATP, 100 ?M) increased Ca2+ by 31.2±3.7 % of ATP effect. 4) mRNA for P2Y11 subtype was identified using RT-PCR. Conclusions: P2Y11 purinergic activation can increase the intracellular calcium level through calcium mobilization in undifferentiated retinoblastoma cells, which may play an important role in cell proliferation, differentiation, and even pathologic processes.

      • 여러 반송파 부호분할 다중접속 방식의 순방향에서의 적응 보냄 방식

        이 논문에서는, 순방향에서 적응 부채널 배정 방식을 쓰는 여러 반송파 부호분할 다중접속 방식을 제안한다. 제안한 시스템에서는, 한 직접 수열 파형을 여러 부채널에 나란히 보내는 대신, 쓰는이의 직접 수열 파형을 모든 부채널 가운데에서 감쇄 진폭이 가장 큰 부채널로 (채널 상태가 가장 좋은 부채널) 보낸다. 제안한 시스템의 성능과 이제까지의 여러 반송파 직접 수열 부호분할 다중접속 시스템의 성능을 견주어 본다. 또한, 신호가 가장 좋은 부채널에 정확히 배정되지 않았을 때 성능이 어떻게 바뀌는 지도 살펴본다. In this paper, we propose a multicarrier CDMA system with an adaptive subchannel allocation method for forward links. In the proposed system, instead of transmitting identical DS waveforms over a number of subchannels in parallel, each user's DS waveform is transmitted over the user's favorite subchannel which has the largest fading amplitude among all the subchannels. The proposed system is shown to have performance gain over the conventional multicarrier DS/CDMA system. We also investigate how the performance is influenced when the signal is not perfectly allocated into the best subchannel.

        CuZnSOD와 APX를 엽록체에 발현시킨 담배식물체의 Highlight와 Chilling 스트레스에 대한 광합성 효율

        환경스트레스에 대한 항산화효소의 보호효과를 이해하고 효과적인 스트레스내성 식물을 개발하기 위하여 CuZnSOD와 APX를 엽록체에 발현시킨 형질전환식물체 (CA식물체)에 강한 빛 (highlight, 1,100$\mu$mol m$^{-2}$ sec$^{-1}$)과 4$^{\circ}C$ chilling을 처리하였다. CA식물체 잎절편에서의 보호효과를 CuZnSOD을 도입한 SOD식물체, APX을 도입된 APX식물체 및 비형질전환식물체 (NT식물체)의 것과 비교하였다. CA식물체의 광계2에서의 광합성 효율 (Fv/Fm)은 highlight와 4$^{\circ}C$ chilling복합처리 1시간째에서 NT식물체에 비해 약 15%의 보호효과를 나타내었고, 광계1에서의 P700 redox state는 처리 후 3시간째에 약 23%의 보호효과를 나타내었다. SOD식물체와 APX식물체의 복합처리에 대한 보호효과는 CA식물체와 NT식물체의 중간 효과를 나타내었다. 이러한 결과는 엽록체에서 CuZnSOD와 APX의 동시발현이 highlight와 chilling에 의한 산화스트레스를 극복하는 데 매우 중요하게 관여함을 직접적으로 제시하는 것이다. In order to understand the protection effects of antioxidant enzymes against oxidative stress caused by various environmental stresses, transgenic tobacco (Nicotiana tabacum cv, Xanthi) plants expressing both copper/zinc superoxide dismutase (CuZnSOD) and ascorbate peroxidase (APX) in chloroplasts (referred to as CA plants) were subjected to highlight (1,100$\mu$mol m$^{-2}$ sec$^{-1}$) and chilling at 4$^{\circ}C$. The protection effects of CA plants using leaf discs were compared with those of transgenic plants expressing either CuZnSOD or APX in chloroplasts (SOD plants or APX plants, respectively) and non-transgenic (NT) plants. CA plants showed about 15% protection in the photosynthetic efficiency (Fv/Fm) of photosystem II relative to NT plants 1 hr after treatment of both highlight and chilling, whereas they showed about 23% protection in the redox state of P700 in photosystem I at 3 hr after treatment. SOD plants or APX plants showed an intermediate protection effect between CA plants and NT plants. These results demonstrated that the coexpression of CuZnSOD and APX in chloroplasts importantly involves in the protection effects against oxidative stress caused by various environmental stresses.

      • Cloning and Expression Characteristics of Pharbitis nil COP1 (PnCOP1) During the Floral Induction

        The ubiquitin E3 ligase COP1 (Constitutive Photomorphogenesis 1) is a protein repressor of photomorphogenesis in Arabidopsisplants, and it found in various organisms, including animals. The COP1 protein regulates the stability of many of the light-signaling components that are involved in photomorphogenesis and in the developmental processes. To study the effect of COP1 on flowering in a short day plant, we have cloned a full-length of PnCOP1 (Pharbitis nil COP1) cDNA from Pharbitis nil Choisy cv. Violet, and we examined its transcript levels under various conditions. A full-length PnCOP1 cDNA consists of 2,280 bp nucleotidesthat contain 47 bp of 5'-UTR, 232 bp of 3'-UTR including the poly (A) tail, and 1,998 bp of the coding sequence. The deduced amino acid sequence contains 666 amino acids, giving it a theoretical molecular weight of 75 kD and a isolectric point of 6.2. The PnCOP1 contains three distinct domains, an N-terminal $Zn^2+$-binding RING-finger domain, a coiled-coil structure, and WD40 repeats at the C-terminal, implying that the protein plays a role in protein-protein interactions. The PnCOP1 transcript was detected in the cotyledon, hypocotyls and leaves, but not in root. The levels of the PnCOP1 transcript were reduced in leaves that were a farther distance away from the cotyledons. The expression level of the PnCOP1 gene was inhibited by light, while the expression was increased in the dark. During the floral inductive 16 hour-dark period for Pharbitis nil, the expression was increased and it reached its maximum at the 12th hour of the dark period. The levels of PnCOP1 mRNA were dramatically reduced upon light illumination. These results suggest that PnCOP1 may play an important function in the floral induction of Pharbitis nil.

        Introduction : Pneumonia is an inflammation of alveolus and pneumal parenchyma and it is one of common pediatric diseases. According to, pneumonia belongs to the Haesu(咳嗽), Chunsik(喘息), Phechng(肺脹), Pungon(風溫). Purpose : The purpose of this study is to find out whether Socheongryongtanggamibang has any effects on children with pneumonia. Method : The subjects are in regard to children with pneumonia who visited Dae Jeon Oriental Medicine Hospital from December 2002 to April 2003. The herbal medicine therapy are applied for one week. Result and Conclusion : After treatment, the symptoms (coughing, rhinorrhea, sputum, fever) of pneumonia were improved. More studies about the Oriental Medicine treatment and conception on pneumonia are needed.

        비후성 유문 협착증에 의한 가성 Bartter 증후군 1례

        We report a 3-month old boy admitted to our hospital with Bartter's syndrome like symptoms and laboratory findings, which were vomiting, failure to thrive, hypochloremic and hypokalemic metabolic alkalosis associated with hyperreninemia, hyperaldosteronism and normal blood pressure. However, the urine chloride level was low. Hypertrophic pyloric stenosis was diagnosed through abdominal ultrasonography. Fredet-Ramstedt operation was done after electrolyte correction. After surgery he made a good recovery and gained body weight. The electrolytes maintained within a normal limit without any potassium supplementations after surgery. Differential diagnosis from Bartter's syndrome was made on the basis of a decrease in urine chloride and the non-necessity for potassium supplementation after surgery. It is relatively rare for hypertrophic pyloric stenosis to induce pseudo-Bartter's syndrome. The importance of considering this diagnosis in such cases is discussed. 저자들은 성장 장애, 만성적인 구토, 저칼륨혈증, 저염소성 대사성 알카리혈증, 고레닌혈증, 고알도스테론혈증, 정상 혈압과 부종결여 등의 Bartter 증후군의 임상형태를 보였으나 요중 클로라이드 농도가 감소되어 있고 초음파 검사상 비후성 유문 협착증으로 진단수술 후 칼륨과 프로스타글란딘 억제제 등의 약물치료 없이도 회복된 가성 Bartter 증후군을 1례 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

        양신탕(養神湯)의 항(抗)스트레스 효과(效果)에 대(對)한 실험적(實驗的) 연구(硏究)

        Objective: The study aimed to evaluate the anti-stress effect of YST on the cold-stressed Mice. Method: The experimental animals were stressed in cold room ($4{\pm}0.5^{\circ}C$) for 1 hour in a day during 2weeks, and administerd 115mg/100g YST extract for 2weeks before stress. For 2weeks, we conducted a research about the change of weight and content of norepinephrine, epinephrine, dopamine, cortisol and GOT, GPT in the mice serum. Results: YST significantly inhibited the decrease of body weight induced by stress, compared with the control group. YST had no significant decreasing effect on the change of content of the serum GOT But decreased the content of the serum GPT. YST decreased the content of the serum cortisol. YST decreased the content of the serum dopamine a little and decreased the content of the serum epinephrine significantly. Conclusion: According to the above results, it is concluded that YST will be useful as a remedy against stress disease.

        쥐의 만성 녹내장 모델에서 산화질소 신타아제 동위효소들의 발현

        Purpose: This study investigated the expression of three isoforms of nitric oxide synthase (NOS): neuronal (nNOS), inducible (iNOS), and endothelial (eNOS) in an experimental rat model of chronic glaucoma. The aim was to research the role of nitric oxide (NO) as a neurotoxic molecule in connection with damage to and the degeneration of retinal ganglion cells in glaucoma. Methods: Retinal tissues were obtained after inducing chronic elevation of intraocular pressure by cauterization of episcleral vessels. We then performed quantification and localization of NOS isoforms through western blot and immuno-fluorescence staining of the tissues. Results: The expression of nNOS and iNOS increased significantly but that of eNOS did not. nNOS expressed in the amacrine and displaced amacrine cell of the normal retinal tissue, as well as in retinal ganglion cells in the experimental group. iNOS that expressed in the microglia of the normal retinal tissue was also expressed in the cell thought to be an astrocyte or Muller cell end-feet in the experimental group. Administration of L-NAME (NG-nitro-L-arginine-methyl-esther), a non-specific NOS inhibitor, tended to reduce retinal ganglion cell loss, but this result was without statistical significance. Conclusions: These results showed that the cytotoxicity of excessive NO took part in retinal ganglion cell loss in glaucoma, and the expression of nNOS in retinal ganglion cells suggests that it may play an important role in the selective death of the retinal ganglion cell.
