본 논문에서는 차세대 영상 서비스 기술인 VOD(Video-on-Demand)시스템을 LAN을 이용하여 구현하였다. LAN을 이용한 VOD 시스템은 네트워크 병목현상과 이에 따른 시간 지연 때문에 기존의 네트워크 구현으로는 원활한 서비스가 되지 못하므로, 그러한 문제점을 해결하기 위한 방안으로, 본 논문에서는 토팔러지의 변경과 그에 따른 네트워크 장비의 선정, 개선된 프로토콜의 제정 등이 제시되었다. 결론적으로 구현된 시스템은 세그먼트 당 최대 3∼4명의 클라이언트를 수용할 수 있었고, 전송하고자 하는 영상 데이터의 크기에 따라 미리 네트워크 버퍼의 크기를 최적화하는 것이 서비스의 품질을 높이는데 필수적이라는 것을 알 수 있었다. In this paper, a VOD(Video -on-Demand) system is implemented using LAN(Local Area Network). The use of LAN causes the time-delay problem due to its narrow channel limitation, as a result, the service using the conventional LAN used not to be popular. In this paper, to solve these problems, we develop the modified topology, the selection methodology of the network equipment, and the dedicated(improved) protocol. Consequently, we can serve at most 3 or 4 clients at the same time, and we conclude it is necessary to optimize the network buffer size depending on the size of video data.

Extensive Systemic Sarcoidosis with Testicular Involvement Mimicking Metastatic Testicular Cancer

Sarcoidosis is an idiopathic, multisystem disease that rarely involves the genitourinary tract. Here we present an unusual case of testicular sarcoidosis with extensive lymphadenopathy that mimicked a metastatic testicular tumor. A 27-year-old male presented with a palpable right testicular mass accompanied by multiple palpable inguinal lymph nodes. The scrotal ultrasound showed a hypoechoic lesion at the inferior portion of the right testis. Extensive enlarged lymph nodes were noted in multiple areas on the abdominal computed tomography. Preoperative testicular tumor markers were within the normal range. Exploration of the right testis with a frozen section analysis of the right testicular mass and of a palpable right inguinal lymph node showed granulomatous inflammation. The testis was salvaged and the final pathological diagnosis was sarcoidosis. Treatment with high-dose corticosteroids resulted in complete resolution of the intratesticular mass and a significant decrease in the extent of the lymphadenopathy.

Basal Cell Carcinoma of the Prostate

Basal cell carcinoma (BCC) of the prostate, a rare variant of prostate cancer, is derived from the basal cells of prostatic ducts and acini. BCC generally occurs in elderly men with obstructive voiding symptoms and levels of serum prostate-specific antigen within the normal range. In most cases, diagnosis is made through transurethral resection or simple enucleation. Most cases are indolent, but local recurrence and metastasis have been reported in a few cases. Thus, radical surgery and long-term follow-up are recommended. We report a case of a 54-year-old man who underwent radical retropubic prostatectomy after being diagnosed with BCC during a transurethral resection performed for lower urinary tract symptoms. The patient has remained free of disease for 4 months after surgery.

3-MCA 誘發 上皮腫에 對한 香貝養榮湯과 貝母單味劑가 免疫反應에 미치는 影響

Hyangpaeyangyeongtang, which was first Prescribed in the book of Euijonggumgam (醫宗金鑑), has been widely applied to the treatment of scrofula and carbuncle in the neck and tumor as well in the oriental medicine. The Fritillariae roylei bulbus was also used as an anti-tumor medicine. In order to investigate the effect of Hyangpaeyangyeongtang and Fritillariae roylei bulbus extracts on the immune responses to the epithelioma induced by 3-methylcholanthrene and prostaglandin-F_2α in mice, the change of epithelioma size was observed and delayed type hypersensitivity, hemagglutinin titer, hemolysin titer, rosette forming cells, natural killer cell activity, lymphocyte transformation, productivity of interleukin-2 and phagocytic activity were also measured. The results were as follows; 1. The epithelioma size was decreased with statistical significance in the Hyangpaeyangyeongtang group at the 14th day, as compared with the control group, 2. Delayed type hypersensitivity was not shown any decrease in the Hyangpaeyangyeongtang group and the Fritillariae roylei bulbus group, as compared with the control group, 3. Hemagglutinin titer was increased with statistical significance in the Hyangpaeyangyeongtang group, but not in the Fritillariae roylei bulbus group as compared with the control group. 4. Hemolysin titer was increased with statistical significance in the Hyangpaeyangyeongtang group, but not in the Fritillariae roylei bulbus group as compared with the control group. 5. Rosette forming cells were not increased in the Hyangpaeyangyeong tang group, but increased with statistical significance in the Fritillariae roylei bulbus group as compared with the control group. 6. Natural killer cell activity at the ratio 100:1 of effector cells to target cells was increased with statistical significance in the Hyangpaeyangyeongtang group and the Fritillariae roylei bulbus group respectively, and the natural killer cell avtivity at the ratio of 50:1 and 10:1 of effector cells to target cells Hyangpaeyangyeongtang group was increased with statistical significance as compared with the control group. 7. Lymphocyte transformation was increased in the Hyangpaeyangyeong tang group and the Fritillariae roylei bulbus group respectively, as compared with the control group. 8. Productivity of interleukin-2 was increased in the Hyangpaeyangyeong tang group and the Fritillariae roylei bulbus group respectively, as compared with the control group. 9. The clinical study was done in 50 patients with acne who had been treated with acupuncture and herb-medicine. The results were as follows. There were 9 males and 41 females in sex. The peak age at onset was 21 - 30 years old(28 cases. 56%). The most commen chief complaint was itching(70%). As for digestion. 24 cases(48%) is poor and defecation disorder appeared 25cases 50%). difficult menstruation 20 cases (48.78%). Bohyulansintang(補血安神湯) was most use in treatments as 26.38%.As for treatment period. 25 cases(50%) were treated from 1 week to 3 weeks.
