도시와 농촌지역 당뇨노인의 자가관리에 영향을 미치는 요인 비교

Diabetes Mellitus is a chronic disease affecting on elderly diabetic patients’ quality of life. Elderly diabetic patients have many difficulties in diabetes self-management because their physical, psychological and social functions. And also it is predicted that the factors affecting on the self-management of elderly diabetes are different due to the regional and cultural characteristics between urban and rural areas. We explored the variables affecting on the self-management of the elderly diabetic patients and compared the differences between two groups. In comparison with elderly diabetic patients between two groups, family support and knowledge on diabetes were significantly higher in rural elderly diabetics rather than urban elderly diabetics. However, there were no differences in diabetes self-management. The factors on affecting elderly diabetic patient's self-management in urban were sex, smoking, exercise, diet, family support and knowledge on diabetes. In rural, only family support was the significant factor. Based on the results found, practical implications for implementing prevention and treatment programs for elderly diabetic patients are recommended. 당뇨병은 약물, 식이, 운동 등 일상생활전반에서 지속적인 자가관리가 필수적인 만성 질환이다. 하지만 노인 당뇨병 환자는 노년기 발달단계의 특성으로 인한 신체적, 심리적, 사회적 어려움으로 당뇨병 자가관리가 쉽지 않다. 본 연구에서는 당뇨병을 가진 노인을 대상으로 자가관리에 영향을 미치는 요인을 살펴보고, 지리적, 의료적, 문화적 환경이 상이한 도시와 농촌 지역 간 자가관리 정도에 차이가 있는지 조사하였다. 연구결과 도시와 농촌 당뇨노인의 자기관리 수준은 유의미한 차이가 없었으나 가족지지 및 당뇨지식의 수준은 상이하였다. 또한 자가관리에 영향을 미치는 변인도 지역간에 차이가 있었다. 본 연구의 결과는 고령화시대에 급증하고 있는 만성질환 노인에 대한 이해를 높이고, 지역사회 특성에 맞는 개입전략 수립에 기초자료를 제공할 수 있을 것이다.
