        어촌지역에서 E형 간염 바이러스의 항체분포

        Serological study on HEV(Hepatitis E virus) infection for 462 cases was carried out with EIA technique, and also the possibility of mixed infection with other viral hepatitis among the HEV antibody positive cases was investigated from April to October 1992 in Korea. Positive rate of hepatitis E antibody for residents in seashore town and blood donors were shown 8.0 % and 9.5% respectively, and that of 50 to 60 years old age group was higher than the other age groups, while the lowest rate was scored at under 20 years old age group. According to investigate mixed infection with various hepatitis virus in HEV antibody positive cases, anti-HAV, anti- HBs, HBsAg, and anti-HBc EIA tests were tested and the results were as follows;79.3% of HEV positive cases coreacted with HAV antibody, 31% with anti-HBs, 13.8% with HBsAg, 65.5% with anti-HBc and 76.7% with ALT elevated cases, respectively. There was no HEV positive case among the HCV antibody positive cases and paid blood donors, and in cases of using public water spring(wells) were shown more higher positive rate of anti-HEV than the other groups such as tap water or private well user cases.

        의료진의 인지된 서비스품질이 환자와 보호자의 서비스 인식에 미치는 영향

        In this study, we examined the effects of healthcare staffs` perceived service quality(E-PSQ) on patients` perceived service quality(P-PSQ) leading to patients` satisfaction(P-CS) and the effects on guardians` perceived service quality(G-PSQ) also leading to guardians` satisfaction(G-CS). To investigate the causalities of the factors, we collected national wide samples of 144 hospitals, 721 healthcare staffs, 1456 patients, and 1455 guardians of the patients. Followed were contributions of this study. First, healthcare staffs and patients were commonly related by service quality, which meant the increase of healthcare staffs` perceived service quality led to increase of patients` perceived service quality. Second, healthcare staffs and guardians of patients were also related by perceived service quality. Third, patients` perceived service quality would lead to service satisfaction of patients, Fourth, guardians` perceived service quality would also lead to service satisfaction of guardians. The findings implied service managers of hospital settings should pay attention to healthcare staffs` perception of service quality as well as those of patients and guardians. With such strategy, hospitals could survive the drastically changing environments of current healthcare service area.

        노인요양시설의 서비스품질이 서비스만족과 의사결정에 미치는 영향

        본 연구는 사회복지시설 특별히 노인요양시설의 선택이라는 의사결정에 영향을 미치는 요인을 실증적으로 검증하였다. 노인장기요양보험제도의 실시에 따라서 시설이용자는 서비스선택이라는 의사결정에 직면하게 된다. 하지만 이용자의 의사결정이란 우리나라의 사회복지 현실에서는 비교적 새로운 개념이다. 검증을 위해서 전북지역의 20개 노인요양시설로부터 212명의 응답자에게 자료를 수집했고, 구조방정식(SEM)을 이용해서 분석했다. 연구모형으로 서비스품질은 서비스만족을 매개로 의사결정에 영향을 미친다는 모형을 제안했다. 구조방정식을 통한 분석결과 서비스품질에서 서비스만족 경로의 표준경로계수는 0.556(t=4.608, p<0.01)으로 나타났고, 서비스 만족에서 의사결정 경로의 표준경로계수는 0.785(t=5.249, p<0.01)로 나타났다. 모형의 절대적합지수는 χ² = 580.151(d.f.=317, p-value=0.000)이고, RMSEA=0.063이며, 상대적합지수 CFI=0.915로 모형은 절대적으로 뿐만 아니라 상대적으로 적합했다. 또한 서비스품질의 결정요인인 응답성, 신뢰성, 확신성, 공감성, 유형성이 검증되었고, 이들의 서비스품질 결정 가중치는 제시된 순서임이 확인되었다. 연구결과는 서비스품질이 노인요양시설 이용자의 의사결정으로 이어지는 모델과 요인간의 역학관계를 제시하고 있으며, 시설관리자가 시설운영을 위해서 고려해야 할 서비스품질, 서비스만족, 그리고 의사결정과 관련된 요소를 시사하고 있다. The purpose of this study is what are the factors affecting decision making for selection of social welfare facility, esp., elderly care facility. The resident's selection for social welfare facility are relatively new terms in the Korean social work. To explore the factors, we employed an empirical study. The collected data was analyzed by using SEM(Structural equation modeling). As the results, the standardized regression coefficient of the hypothesis that the service quality will affect the service satisfaction is equal to 0.555(t=6.723, p<0.01) and the coefficient that the service satisfaction will affect the decision making is equal to 0.766. The absolute fitness of the SEM model shows χ² = 580.151(d.f.=317, p-value=0.000) and RMSEA=0.063, so that the model is absolutely fit. Since CFI=0.915, the model is comparatively fit. Conclusively speaking, the hypothesis that service quality affects to service satisfaction was supported. The hypothesis that service satisfaction affects to decision making was also supported. By accounting for the results of the analysis, this study verified the service quality leading to service satisfaction is an important factor for resident to select a residential facility.

        한국인에서 E형 간염 바이러스의 항체 유병률에 관한 조사

        Serosurvey of hepatitis E virus (HEV) infection, known as enterically transmitted non-A, non-B hepatitis (ET-NANBH), was done in Korea between May through August, 1991 using anti HEV EIA method developed by Genelabs Inc. in 1990. The coated polypeptide antigens were expressed in Baculovirus vector from the specific HEV cDNA clones derived from two patients infected with HEV, one from Burma (antigen A) and the other from Mexico (antigen B). The 1,079 examinees was arbitrarily grouped into risk (406) and non-risk (673) group according to the serologic markers of hepatitis B virus (HBsAg, HBsAb, and HBcAb), anti-hepatitis C virus (HCV), and alanine transarninase (ALT) value but not to the risk of HEV. The overall seroprevalences of HEV in non-risk and risk group were 10.4 %, and 8.9%, respectively. Among the non-risk subgroups, the prevalence of anti-HEV was about 10 % regard- less of the result of HBsAb or HBcAb. There was no sexual and regional difference. The peak prevalence was shown at the age between 30 and 39. In the risk groups, the highest prevalence was 15% shown in the anti-HCV positive group and the lowest was the 5.9% of HBsAg positive group. There was a little higher prevalence rate in male group but there was no significant statistical difference between male and female. Anti-HEV posit,ive rates of normal group were relatively higher than the expected risk group and these results strongly suggested the possibility of existence of HEV infection in Korea, particularly in rural areas where the tap water supply facilities are not enough.

        노인요양시설 이용자의 시설선택모형에 관한 연구

        본 연구는 노인요양시설 이용자의 인지된 서비스품질이 서비스만족과 시설선택에 어떻게 영향을 미치는가를 실증적으로 검증하였다. 검증을 통해서 시설선택모형을 제안했다. 검증을 위해서 전북지역의 20개 노인요양시설로부터 212명의 응답자에게 자료를 수집했고, 구조방정식을 이용해서 분석했다. 분석결과 연구모형에 포함된 부분매개모형 즉 서비스품질이 시설선택에 부분적으로 직접영향을 미칠 것이라는 가설이 통계적으로 유의하지 않아 기각하고 서비스품질은 서비스만족을 완전매개로 시설선택에 영향을 미친다는 완전매개모형을 제안했다. 제안된 완전매개모형에서 서비스품질에서 서비스만족 경로의 표준경로계수는 0.556(t=4.608, p<0.01)으로 나타났고, 서비스만족에서 시설선택 경로의 표준경로계수는 0.785(t=5.249, p<0.01)로 나타났다. 또한 부문매개모형과 완전매개모형의 간결적합지수는 부분매개모형의 PRATIO=0.897, 완전매개모형의 PRATIO=0.900으로 나타났다. 따라서 완전매개모형에서 제시하는 서비스품질은 서비스만족완전매개로 시설선택에 영향을 미친다는 가설이 지지되었다. 또한 서비스품질의 결정요인인 응답성, 신뢰성, 확신성, 공감성, 유형성이 검증되었고, 이들의 서비스품질결정 가중치는 제시된 순서임이 확인되었다. 연구결과는 서비스품질이 노인요양시설 이용자의 시설선택으로 이어지는 모델과 요인간의 역학관계를 제시하고 있으며, 관리자가 의사결정에서 서비스품질, 서비스만족, 그리고 시설선택과 관련된 고려할 요소를 시사하고 있다. This study aimed at identifying how perceived service quality affects to service satisfaction and selection in elderly care facility. To verify the hypothesis, data was collected from 212 residents from 20 facilities in Jeonbuk province, which was analyzed by using SEM (Structural equation modeling). The SEM study has revealed the original study model is not statistically significant. Therefore, we designed a modified model that is a complete mediated model depicting that the service quality does not directly affect the facility selection and service quality does affect the service satisfaction leading to facility selection. The standardized regression coefficient of the hypothesis that the service quality will affect the service satisfaction is equal to 0.556(t=4.608, p<0.01) and the coefficient that the service satisfaction will affect the facility selection is equal to 0.785(t=5.249, p<0.01). The original model and the modified model were absolute and comparatively fit. Parsimony fit index or PRATIO of the original model = 0.897 and PRATIO of the modified model = 0900, that is, the modified model is more parsimony than the orignal. Conclusively the model we built was acceptable and satisfactory in terms of absolute, comparative, and parsimony fit. The hypothesis that service quality affects to service satisfaction was supported. The hypothesis that service satisfaction affects to facility selection was also supported.

        Gold 제제 사용후 발생한 약진 1 예

        Gold preparation had been used frequently in the past in the treatment of rheumatoid arthritis, tuberculosis and lupus erythematosua, etc. It is still used. when other more potent remedies, such as steroid, failed. We report a case of drug eruption following the use of gold preparation. A 38 year-old female was referred to our department from orthopedic surgery because of generalized, multiple, pin head sized, violaceous and flat topped papules of 1 x 2 mm in size and mucosal erosions and ulcers following gold therapy for rheumatoid arthritis. She had received 5 intramuscular injections of gold preparation (Myochrysine'), 220mg in total amounts. Our clinical impression was lichen planus like drug eruption, but histopathological picture disclosed nonapecific dermatitis. The eruption had disappeared gradually following oral administration of antihistaminics and prednisolone and daily starch tub baths.

        A case of 33 years old male was reported in this paper. The patient had been suffered from weight loss, anemia and severe abdominal pain. The nature of the pain was colicky and radiated to left side of chest time to time. In the physical examination, no remarkable sign was detectable except slightly anemic conjunctiva and mild emaciation. The- upper . GI series was -as follows; Esophagus was normal. Stomach was outlined and there was noted irregular filling defect in the body of stomach with encircle. Duodenal bulb was not unusual. The X-ray diagnosis was suspected fungating gastric cancer or leiomyosarcoma. In the endoscopic findings, however, all of the mucosal pattern in the stomach were within normal limit. Finally, the extra gastric adhesion was confirmed and surgical procedure was done followed by laparatomy.

        문화관광해설의 구성요인 및 해설전달과정 분석

        This study investigated the components of cultural heritage interpretation and analyzed the cultural interpretation delivery process. Based on a review of related literature, this study developed a structural model between visitors' experiences of four different dimensions of cultural interpretation, visitor satisfaction and revisit intention. In order to examine the causal relationship between the theoretical constructs, data were conveniently collected from 240 visitors of Jeonju Hanok Village. Results showed that three dimensions of the interpretation factors(i.e. informative and educational, easy to follow, entertaining factors) positively affected kindness of the interpreters. Results also indicated that the three factors did not directly but indirectly affect the interpretation satisfaction via the kindness factor. Moreover, findings showed that the respondents' satisfaction on the interpretation turned out to be a significant predictor of their revisit intention. In conclusion, the interpretation delivery process verified in this study could be summarized as follows: Three heritage interpretation factors → kindness factor → interpretation satisfaction → behavioral intention. 관광문화해설의 해설요인이 문화해설만족으로 이어지는 인과적 과정과 해설만족이 행동의도에 미치는 영향을 실증적으로 검증했다. 검증을 위해서 전주한옥마을에 방문하여 문화해설을 이용한 관광객 289명을 조사했고, 분석요건을 충족하는 247응답을 분석했다. 요약된 연구결과는 첫째, 문화관광해설요인인 정보교육성, 이해용이성, 흥미성은 친절성에 긍정적인 영향을 미쳤다. 둘째, 정보교육성, 이해용이성, 흥미성은 흥미성을 매개로 문화해설만족에 긍정적인 영향을 미쳤다. 셋째, 문화해설만족은 방문객의 행동의도에 긍정적인 영향을 미쳤다. 주목할 점은 문화관광해설의 모든 구성요인이 친절성을 매개로 해설만족으로, 해설만족을 매개로 행동의도 이어지는 서비스전달과정(Service delivery process)이 확인되었다. 즉 문화관광해설을 구성하는 요인사이에 역학관계 및 요인사이의 인과성을 확인할 수 있었다. 이와 같은 검증은 문화관광해설의 교육과 실무에서 구성요인 중에서 정보교육성을 우선해야 하며, 친절성이 중요함을 시사하고 있다.
