There is a constant need for negative baric pressure suction apparatus in neurosurgical practice. This drainge unit should be able to drain fluid into a sterile and closed reservior. We have studied a technique employing the Wangensteen suction apparatus for postoperative drainage in craniotomy patients, particularly patients with subdural hematomas and patients having possibility of subgaleal hematomas. The advantages of this technique are; 1) Local postoperative complications such as hematoma, secondary infections and skin flap necrosis are reduced. 2) This technique leads to decrease in healing time. 3) Bulky bandages and redressing can be avoided. 4) It is a simple, economic and readily available technique. 5) The study of the suctions inside the transparent tubes gives good informations about the local situation in the wound.

Intracranial epidermoid cysts are rare, consisting of 0.2%∼1% of all intracranial tumors and are frequently called cholesteatoma or pearly tumor. The favorite sites are the C-P angle, parapituitary, diplo¨e, cerebellum and rarely the cerebral hemisphere, lateral ventricle and intraspinal region. Epidermoid cysts of 3rd ventricle or pineal region are extremely rare and only a few cases have been reported in the literature. The authors have reviewed the literature and report the surgical removal of a case of epidermoid cyst which had developed in the pineal region extending to the posterior 3rd ventricle.

13 cases of intracranial Aneurysm were obliterated intracranially with Heifetz clip under Halothane/O₂ Deliberate Hypotensive anesthesia. Deliberate hypotension in the surgery of intracranial aneurysm is used to diminish bleeding and render an aneurysmal sac slack to make clipping easy. Herein is presented a review of clinical experience of deliberate hypotension in conjunction with postural drainage. Clinical investigation has shown that this method offers a wider margin of safety. All factors stressed again the needs for careful, adequate and ideal clippage of the neck of aneurysm preferably with the use of Heifetz clip. Deliberate hypotension and adequate clippage of the neck of aneurysm subsequently allowed a favorable clinical result with an operative mortality by 7.7%.

To provide a guideline of accurate diagnosis and proper methods of treatment of meningioma, the most common benign intracranial tumor, the author studied 57 cases of meningioma who had been diagnosed and operated at the Department of Neurosurgery, Inje Medical College, Paik Hospital, Pusan from July, 1983 to June, 1988. The results were as followings. 1) The ratio of male to female was 1 to 2.2 and 31.6% of the 57 patients were in the 5th decade of age and 29.8% in the 6th decade. 2) The predilection sites were parasagittal and falx(33.3%), convexity(26.3%) and sphenoid ridge (21.1%) areas. The most frequent histological type was meningotheliomatous type(47.4%) which was followed by transitional(15.8%) and fibroblastic(10.5%) type. 3) The common initial clinical features were headache(29.3%), visual disturbance(24.6%), seizure (22.8%) and motor weakness(10.5%). In radiologic diagnostic tests, there were abnormal findings in 47.4% on plain skull X-ray, the possibility of diagnosis as the meningioma was 91.2% by means of brain CT scan only, which was regarded as the most accurate and safe method. 4) The extent of operation were total(84.2%), subtotoal(12.3%) and partial(3.5%) removal. The surgical outcome were excellent(49.1%) and good(29.8%). 5) The preoperative embolization of feeding artery was effective to reduce the operative bleeding, and the CO₂ laser was thought to be effective surgical tool to remove the meningioma, with its unique properties of non-mechanical bloodless evaporation of tumor and minimizing of brain edema.

Malignant melanoma of the spinal cord is a rare tumor. The melanomas that originate in the meninges bear a general structural resemblance to those arising elsewhere, but it is questionable whether they ever metastasis outside the central nervous system. If can be also argued that the primary central nervous system melanoma could actually be a metastasis from an occult primary site in the skin, esophagus, rectum or other sites. We are reporting a case of malignant melanoma involving the cervical spinal meninges and hard palate.
