A Case of Stent Thrombosis Occurred at 5 Years after Sirolimus-Eluting Stent Implantation

Drug-eluting stents (DES) have reduced the rate of repeated revascularization of target lesions. For this reason, DES are considered to be superior to bare-metal stents in reducing the restenosis rate. However, some problems have been reported after implantation of DES. One of them, stent thrombosis, has arisen as a fatal complication. Dual antiplatelet therapy is recommended for at least 12 months after implantation of DES to prevent stent thrombosis. Here, we report a case of very late stent thrombosis that occurred 1 week after discontinuation of clopidogrel at 5 years (1832 days) after implantation of a sirolimus-eluting stent.
Unexpected Pathologic Diagnosis of the Mitral Valvular Mass

A 59-year-old man with multifocal cerebral infarction was found to have the large obstructive mitral valvular mass. Although benigntumor was under suspicion before surgery, he was finally diagnosed as chronic infective endocarditis by microscopic evaluation. The precise diagnosis and the proper management of a cardiac mass are very important since even the benign tumor maycause fatal complications. However, primary cardiac mass has the broad spectrum from pseudo-tumor to malignancy and the differentialdiagnosis using non-invasive methods is not easy even with the currently available imaging techniques.
Prevention of Cognitive Impairment in the Midlife Women

Forgetfulness is common symptom with age. Especially for midlife women, hormonal cessation by menopausal change is one of the causes in cognitive disorders. And neuropathological changes in brain can lead to mild cognitive impairment (MCI) and eventually dementia. Prevention of MCI is important for decreasing progression to dementia. This article presents therapeutic approaches based on pathophysiologic changes in brain for preventing cognitive decline. (J Menopausal Med 2015;21:19-23)
Inhibition of trichostatin A-induced antiangiogenesis bysmall-interfering RNA for thrombospondin-1

Expression of thrombospondin-1 (TSP-1), which is a known inhibitor of tumor growth and angiogenesis, is reciprocally regulated by positive regulators, such as VEGF. Additionally, trichostatin A (TSA) sup - presses tumor progression by altering VEGF levels and VEGF-mediated signaling. Thus, understanding TSA-regulated TSP-1 expression and the effects of altered TSP-1 levels might provide insights into the mechanism of action of TSA in anti-tumorigenesis, and provide an approach to cancer therapy. Here, we examined the effect of TSA on TSP-1 expression, and the effects of TSA-induced TSP-1 on cell motility and angiogenesis, in HeLa and bovine aortic endothelial cells. TSA remarkably increased TSP-1 expression at the mRNA and protein levels, by controlling the TSP-1 promoter activity. Both TSA and exogenous TSP-1 reduced cell migration and capillary-like tube formation and these activities were confirmed by blocking TSP-1 with its neutralizing antibody and small-interfering RNA. Our results suggest that TSP-1 is a potent mediator of TSA-induced antiangiogenesis.

Gait disturbance in infants and toddlers results from various diseases, such as musculoskeletal, neuropathic, metabolic, and inherited disorders. These disorders lead to myopathies with muscle weakness. Nutritional deficiency can cause myopathy but it is very rare, because of early diagnosis and effective treatment. Vitamin D deficiency also can induce myopathy. Rickets, the clinical term of chronic vitamin D deficiency, results in secondary hyperparathyroidism that can show the characteristics of myopathy, such as muscle weakness, especially on the proximal muscles of lower extremities and the following waddling gait. We report a rare case of iatrogenic rickety myopathy. (J Korean Acad Rehab Med 2010; 34: 480-482)
