Many disorders in neuropsychiatric field demonstrate variable motor disturbances as their clinical feature or in their courses of illness and also due to psychopharmacological treatment. Although association of such motor disturbances with the pathophysiological aspect of various neuropsychiatric illness are still lacking, some form of motor disturbance offer a window through which pathophysiologic mechanism of such illnesses can be viewed. Cognitive control of motor functions are briefly reviewed in this article and the importance and method of motor function assessment in major neuropsychiatric disorders are also discussed. Motor dysfunction of major neuropsychiatric illness such as schizophrenia and mood disorders may offer a chance of a deeper understanding on the pathophysiologic aspect of their clinical presentation.

Symptoms of dementia vary widely according to the etiology, nature, progression of the causative disease. Not only various cognitive and non-cognitive features are present in any given point of evaluation, but also it is very common to note the ups and downs of such symptoms in an individual patient as the disease progress. Although cortical degenerative dementia such as Dementia of Alzheimer's Type typically show cognitive deficit in language, memory, praxis etc. in its early stage, it is unwise to confine such symptoms to be characteristic only in cortical dementias. Likewise, neurologic symptoms or changes in affect, personality and behavior, frequently seen in vascular dementia or other subcortical degenerative dementias should be understood as a part of whole cognitive and behavioral symptomatic gamut. As a logical conclusion, when evaluating the symptoms of a dementic patient, all the physical, neurologic, and neuropsychiatric evaluation must be applied as well as neuropsychological assessments. In addition, attention must be paid on the patients' functionality of daily living, reversibility, and appropriateness of services by family members or caregivers.

N/A Enhanced HLA-DR expression is a characteristic feature of inflamed rheumatoid synovium. Among the cytokines demonstrated within synovial fluid and tissue, gamma-interferon (IFN-γ) is known to be a potent inducer of HLA-DR antigens in vitro. Gold compounds have been used in rheumatoid arthritis treatment for mamy decades, but their mechanism of action, especially on immune response, remains unk- own. Thus we investigated the potential of gold sodium thiomalate (GST) to interact with INF-γ on rheumatoid synovial cells in culture. The results were as follows: 1) IFN-γ increased the proliferation and HLA-DR expression of RA synovial cells in a dose-dependent manner. 2) GST inhibited synovial cell proliferation. How-ever, GST alone in various concentrations was found to stimulate DR expression by synoveal cells. 3) Simultaneous GST treatment of IFN-γ stimulated synovial cells inhibited the increase of HLA-DR at the concentration of 0.1~10 mcg/ml. 4) A higher concentration of GST (25mcg/ml) in- duced DR antigen synergistically. Our data suggest that GST has an immunostimulatory effect on synovial cells which is related to enhanced HLA-DR antigen expression.

적기에 베체트병의 심혈관침범을 진단하여 면역억제치료를 시작함으로써 방실전도장애가 호전되어 영구형 심장박동기 삽입을 예방할 수 있었던 환자 1예를 보고하는 바이다. Atrioventricular conduction disturbance is a rare clinical manifestation of Behcet disease. We describe a case in which a 48-year-old man complaining of dyspnea on exertion presented with complete atrioventricular block with junctional escape rhythm caused by Behcet disease. He also had moderate aortic regurgitation and mitral regurgitation. MRI and PET scan revealed inflammatory changes involving the ventricular septum and the ascending aorta. Recovery of atrioventricular conduction and improvement of valvular regurgitation were achieved with immunotherapy. Timely diagnosis and treatment of cardiac Behcet disease can prevent unnecessary pacemaker implantation. (Korean J Med 2014;86:65-69)

IoT 기술의 발달과 사용자들의 요구사항이 높아짐에 따라 IoT 디바이스와 서버와 같은 중계 시스템 간 저지연 통신에 대한 필요성이 증가하고 있다. 이에 본 논문에서는 IoT 디바이스와 엣지 컴퓨팅 기술을 통해 실시간에 가까운 저지연 통신 지원을 위한 gRPC 기반 MQTT 디바이스 연동 인터페이스를 설계하였다. As the development of IoT technology and increasing user requirements, the need for low-latency communication between IoT devices and relay systems such as servers is increasing. Therefore, in this paper, a gRPC-based MQTT device interworking interface was designed to support near real-time, low-latency communication through IoT devices and edge computing technology.

N/A Rheumatoid synovium is characterized by the proliferation of synovial membrane and subsynoivial infiltration of lymphocytes clustered around Ia+ synovial cell. We performed this study to investigate the effect of gamma-interferon (gamma-IFN) on lymphocyte adhesion to rheumatoid synovial cells in vitro. Adherent synovial cells (SC) obtained from rheumatoid synovium by enzyme digestion were cultured at a low density with various concentration of gamma-IFN and then were incubated with freshly isolated peripheral blood T-lymphocytes. Gamma-IFN induced HLA-DR antigen and significant lymphocyte adhesion to SC. The lymphocyte adhesion to SC were enhanced in the dose-response pattern of gamma-IFN, whereas the intensity of DR antigen or the number of DR+ SC showed plateau at higher concentration of gamma-IFN, and were not correlated with adhesion index. Additionally, there was remarkable changes of intracellular fibronectin morphology. These findings suggest that gamma-IFN induced DR antigen on SC plays a central role in adhesion of lymphocyte to SC, however changes of fibronectin may have additive effect on binding enhancement.
