        Primary Ureteral Transitional Cell Carcinoma in a Dog

        14살령의 중성화 수컷 개가 혈뇨와 배뇨통으로 내원하였다. 복부초음파 검사에서 편측성 수신증과 요관 벽이 국소적으로 두꺼워진 수뇨관증이 진단되었다. 수술적 탐색으로 좌측 근위 요관에서 요관 유래의 관내 종괴가 확인되었으며, 종괴 절제술이 실시되었다. 요관 종괴는 조직학적 검사를 통해 유두상 이행세포암종으로 진단되었다. 환자는 안정적으로 회복되었으나, 수술 3개월 후 또 다른 종괴가 우측 신장에서 발견되었다. 신장 종괴에 대한 세포학적 검사 결과 간엽세포종양으로 진단되었다. A 14-year-old, castrated male mixed-breed dog weighing 9 kg was presented for hematuria and dysuria. Abdominal ultrasound showed unilateral hydronephrosis and hydroureter with focal thickening of the ureteral wall. Surgical exploration revealed an intraluminal mass arising from the proximal left ureter. Mass resection was performed. Histopathology of the ureteral mass was consistent with a papillary transitional cell carcinoma. The patient recovered well post-operatively, but was diagnosed with another tumor three months later, this time in the right kidney. Fine-needle aspiration cytology of the renal mass revealed an epithelial cell tumor with mesenchymal features.

        <객주> 의 구조 분석

        The purpose of this thesis is to analyse the plot structure and to define the canon of <Gaek Ju>, a historical novel by Ju-Young Kim. The authoritive text of <Gaek Ju> should be the revised version published in 1992 rather than the original version published in 1984 when it ended the serial on Seoul Shinmun from June 2, 1979 to fed. 29, 1984, In this serial, the wrong chronological background should be modified from 1876 to 1883, and in accordance the age of the title-role from 25 to 32. The plot-structure of this novel is a chain plot-structure borrowing its sources from folk tales. The author enhanced its originality with mixing such literary traditions as Ya-Dahm (an official version-up of the historical tales). P'ansori, folk songs in his own way. Besides, he succeeded to make the people's lives visible by depicting in detail the manners and humours of the merchants in The late Chosen dynasty. In conclusion, this novel should be considered as a masterpiece which included historical truth and the lower class's common belief of the historical novel.

      • $^{18}F$-FDG PET/CT 검사에서 신장 방사능의 효과적인 배설 방법에 관한 연구

        FDG PET/CT 검사에서 FDG는 신장 및 요료계를 통하여 배설된다. 이 방사능의 배설을 촉진 시키는 방법을 비교 및 분석하여 각 의료기관의 실정에 맞는 방법을 선택하는데 도움을 주고자 한다. 8개월간 본원에서 PET/CT 검사를 시행한 환자 중 신장에 병적 병변이 없고 기능이 정상인 환자 총 30명을 대상으로 하였다. FDG-PET/CT 기본검사를 마친 환자들을 무작위로 10명씩 다음 세 집단으로 나눠 지연검사를 촬영하였다. (1) 1시간 대기한 집단(Delay group), (2) 수분을 약 700 mL 섭취한 후 1시간 대기한 집단(Hydration group), (3) 이뇨제를 정맥주사하고 30분 대기한 집단(Lasix group). 평가는 육안평가와 정량분석을 하였다. 육안평가는 기본검사와 비교하여 각 집단별 지연검사 영상에서 신장의 방사능 배설을 네 단계로 나누어 비교 및 분석하였다. 정량분석은 기본검사의 표준화섭취계수에서 지연검사의 표준화섭취계수의 감소율을 측정하여 신장 방사능의 배설률로 활용하였다. 육안평가상 Lasix group이 가장 배설이 잘 되었고 그 다음은 Hydration group이었다. Delay group은 효과적인 배설이 이루어지지 않았다. 정량분석 결과 신장방사능 배설률은 Delay group $9.2{\pm}20.7%$, Hydration group $28.1{\pm}22.8%$, Lasix group $29.5{\pm}23.1%$이었다. Delay group과 Hydration group간, Delay group과 Lasix group간의 배설률은 통계적으로 유의한 차이를 보였다. Hydration group과 Lasix group간은 유의한 차이가 없었다. 본 연구에서 이뇨제를 사용하는 것이 신장의 방사능 배설을 촉진시키는데 가장 효과적임을 확인할 수 있었다. 또한 수분을 섭취하는 방법도 비슷한 효과를 보였으며 수분 섭취 없이 지연영상을 얻는 것은 효과가 없음을 확인 하였다. 이상의 방법들을 각 의료기관 실정에 맞게 적용함으로써 영상의 질 향상에 도움을 줄 수 있을 것이다. Purpose: Renal excretion is the main route of FDG clearance in FDG PET/CT scan. Applying optimal method of renal excretion is very important for enhancing image quality and diagnostic accuracy. We evaluated several methods of renal excretion in FDG PET/CT scan. Materials and Methods: Thirty patients with normal renal function were prospectively included. Patients were divided into three group and undergone early and delayed FDG PET/CT scans. (1) Delay group; at 1 hour later of early scan, delayed scan was performed without additional hydration, (2) Hydration group; at 1 hour later of early scan, delayed scan was performed with additional oral hydration (700 mL of water), (3) Lasix group; lasix was administered at the end of early scan and dealyed scan was performed 30 min later. Early and delayed scans were compared to evaluate efficiency of renal excretion. Visual and quantitative analyses were performed by experienced physician and technologist of nuclear medicine. Results: On the visual analysis, renal excretion was the most evident in Lasix group followed by Hydration group. Delay group showed poor renal excretion. On the quantitative analysis, washout rates were $9.2{\pm}20.7%$, $28.1{\pm}22.8%$ and $29.5{\pm}23.1%$ for Delay, Hydration and Lasix groups, respectively. Conclusion: Administration of lasix was the best method for enhancing renal excretion. Delayed scan with hydration was also efficient method, but delayed scan without hydration was not adequate method.

        멀티레이트 수치적분법을 이용한 유연다물체 동역학해석

        A Nordsick form opf the multirate integration scheme has been proposed for flexible multibody dynamic systems. It is assumed that vibrational modal coordinates in the equations of motion are treated as fast variables, whereas the relative joint coordinates are treated as slow variables. In the multirate integration, the fast variables are integrated with small step-size, and the slow variables are integrated with larger step-size. The proposed multirate integration method is based on the Adams-Bashforth-Moulton predictor-corrector method and implemented in the Nordsieck vector form. The Nordsieck form of multrate integration method provides effective step-size control and at the same time, inherits the efficiency from the Adams integration method. Simulations of a flexible gun and turret system of the military tank have been carried out to show the effectiveness and efficiency of the proposed method.

        대동맥동 동맥류 파열 - 1례 보고 -

        Aneurysm of the sinus of Valsalva is an uncommon cardiac anomaly, usually congenital in origin, which may occur as an isolated defect or in conjunction with other cardiac malformation. This report is a case of a ruptured sinus of Valsalva aneurysm with ventricular septal defect in a 18-year-old female patient who complained progressive exertional dyspnea. She underwent operative management using total cardiopulmonary bypass. The fistula originated from the right coronary sinus and ruptured into the right ventricle and coexistent lesion was supracristal ventricular septal defect. The repair was done through aortic and right ventricular approach. The ruptured sinus of Valsalva was closed with pledget suture and the ventricular septal defect was closed with patch. The postoperative result was good.

        Authors experienced a unique case of craniopharyngioma which had evolved rapidly after 4 years of total resection for supratentorial meningioma. A 58-year-old woman presented with headache and visual deterioration. Previously, she had undertaken surgical removal of frontal convexity meningioma 4 years ago and had been well without any postoperative sequelae thereafter. Brain magnetic resonance imaging demonstrated a newly developed suprasellar mass. Pertinent operative procedure was performed and histological verification was made as an adamantinomatous craniopharyngioma. She has been showing unremarkable clinical course up to date. Possible pathogenic mechanisms of de novo development of craniopharyngioma are disscussed with review of case.

        순환 다물체동역학에서의 비순환적인 동하중해석 공식

        An efficient non-recursive formulation of dynamic force analysis has been developed for serially connected multibody systems. Although derivation of equations of motion is based on a recursive dynamic formulation with joint relative coordinates, in the proposed formulation, dynamic forces such as joint reaction forces and driving force are computed non-recursively for specified joints. The efficiency of the proposed formulation has been proved by the operational count and the CPU time measure, comparing with that of the conventional recursive Newton-Euler formulation. A simulation of 7-DOF RRC robot arm has been carried out to validate solutions of reaction forces by comparing with those from a commercial dynamic analysis program DADS.
