N/A Objective : Volume expansion has been known to be the major factor in the development of hypertenision in chronic hemodialysis(HD) patients. But some HD patients remain hypertensive even with adequate volume control, which suggests the role of undefined uremic toxin in the pathogenesis of hypertension. So we aimed to evaluate the status of blood pressure (BP) control and the effect of Kt/V (as a marker for removal of uremic toxin) on BP in chronic HD patients. Methods : The status of BP control was obtained from records of 8 HD session in 132 patients in November 1996 and 127 patients in November 1997. Of 132 patients studied in 1996, 70 patients underwent a follow-up evaluation in 1997. All patients were dialyzed 3 times a week, 4 hours a session. Postdialytic cyclic 3',5' guanosine monophosphate (cGMP) level was measured in 48 patients as a marker of volume status. Results : The prevalence of postdialytic hypertension (>140/90mmHg) was 73.5 in 1996 and 65.3% in 1997. Normotensive patients (postdialytic mean BP <114 mmHg) had higher Kt/V value than hypertensive patients in both 1996 and 1997. But there was no difference in the degree of ultrafiltration (UF) and cGMP level between two groups. Postdialytic mean BP was inversely correlated with Kt/V level but had no relationship with degree of UF and cGMP level in both 1996 and 1997. The group in which postdialytic mean BP had been decreased during 1 year study period had higher degree of elevation in Kt/V than the group in which postdialytic mean BP had been increased. The changes of postdialytic weight and degree of UF during study period were similar between two groups. The number of antihypertensives used were also inversely correlated with Kt/V but not correlated with degree of UF and cGMP level in both 1996 and 1997. Conclusion : Our study indicate that increasing HD adequacy is associated with improved control of postdialytic mean BP and less use of antihypertensive drugs. UF and antihypertensive drugs may not be adequate form of hypertension treatment as once thought and increasing HD adequacy can be an alternative method.

Although a wealth of persuasive evidence was provided for the involvement of dual specificity phosphatase 1 (DUSP1) in cancers, depression, and hepatitis C virus replication, small-molecule inhibitors have been reported rarely. Because most competitive phosphatase inhibitors are disadvantageous in terms of pharmacological properties due to the possession of highly polar groups, we aim in this work to identify the allosteric DUSP1 inhibitors through the structure-based virtual screening. Using the modified protein-ligand binding energy function involving an accurate molecular dehydration term, we identify six allosteric DUSP1 inhibitors that impair the enzymatic activity at low-micromolar level. Consistent with the kinetic analysis on the inhibitory mechanism, docking simulation results indicate that the newly discovered inhibitors would bind in the peripheral binding pocket of DUSP1 in preference to the active site. Besides the micromolar-level biochemical potency, the allosteric DUSP1 inhibitors found in this work may be meritorious over the competitive ones because of the lack of the reactive moiety to mimic the substrate phosphate group. Hence, they are anticipated to serve as a good starting point for the development of new therapeutics against cancers and neurological diseases.

Dual specificity protein phosphatases (DUSPs) belong to the protein tyrosine phosphatase (PTP) family. DUSPsdephosphorylate both phospho-serine/threonine and phospho-tyrosine of mitogen activated protein kinases (MAPKs)and play important roles in cell growth, regulation and signaling. DUSP28, a member of the atypical DUSPs, hasdephosphorylation activity towards proteins involved in cellular signaling processes. DUSP28 is also implicated in thedevelopment of pancreatic cancer and liver cancer. The atomic resolution structure of DUSP28 should help the structurebaseddesign of specific and potent therapeutics. However, the structure and detailed function of DUSP28 have not beenelucidated yet. Here, we prepared a large quantity of DUSP28 protein and crystallized the protein. During the proteinpreparation, we encountered an unexpected proteolytic cleavage in the middle of the protein domain and overcamethe problem by identifying and mutating the pseudo-thrombin cleavage site. By using the purified protein, we were ableto grow diffraction quality crystals and collected a 2.1 Å resolution diffraction data. The preliminary diffraction analysisrevealed that the crystal is in the space group P3121 with unit cell parameters of a = 78.85 Å, b = 78.85 Å, c = 90.26 Å, α =90.00°, β = 90.00° and γ = 120.00°

Crystallization and preliminary diffraction analysis of dual specificity phosphatase 13a

Dual specificity phosphatases (DUSPs) include MAP kinase phosphatases and atypical dual specificity phosphatases,and mediate cell growth and differentiation. They are considered as drug targets against cancers, diabetes, immunediseases, and neuronal diseases. Two different DUSPs, DUSP13a and DUSP13b are coded in one gene (DUSP13) whosealternative splicing results in two sequence-related proteins with different target specificities. The crystal structure ofDUSP13b showed a canonical DUSP fold. However, the structure of DUSP13a has not been determined yet. To understandstructural mechanism of distinctive target specificities of sequence-related DUSPs, we prepared diffraction-quality crystalsof DUSP13a. Cysteine residues in DUSP13a gene were mutated to serine or alanine to prevent cysteine oxidation. Fromthe stabilized protein, we were able to grow good crystals that diffracted to 1.7 Å resolution. The preliminary diffractionanalysis revealed that the crystal is in the space group P21 with unit cell parameters of a = 40.04 Å, b = 89.34 Å, c = 45.90 Å,α = 90.00°, β = 89.87° and γ = 90.00°.

The common space-occupying lesions of the liver associated witb cirrhosis are hepatocellular carcinoma, regenerating nodule, simple liver cyst, hepatic necrosis, and abscess. The differential diagnosis of hepatocellular carcinoma from other lesions is important for its treatment and prognosis. Anoxic pseudolobular necrosis of the liver associated with cirrhosis resembles hepatocellular carcinoma in view of clinical and radiological findings. Clinical diagnosis may depend on biopsy, although differentiation between this lesion and necrotic hepatocellular carcinoma may be difficult. A 55-year-old man was admitted with bematemsis and melena. Radiologic examination showed small, round space occupying lesions of the liver. These lesions resembled necrotic hepatocellular carcinoma. We diagnosed this lesions as anoxic pseudolobular necrosis of liver by repeated biopsies. We report this case because of its similarity with necrotic hepatocellular carcinoma in view of radiologic findings and its rarity.

More than 17 different terms, including carcinosatcoma and pseudosarcoma, have been applied to the rare polypoid tumors of the esophagus that demonstrate both carcinomatous and sarcomatous components. The multiplicity in terminology seems related to the uncertain histogenesis of these tumors. A demonstration of the ultrastructure of the spindle cells (containing desmosomes and tonofilaments) is consistent with an epithelial origin. The patient was a 53 year-old man who had suffered from dysphagia and foreign body sensation in larynx. Endoscopic finding was a large polypiod mass with ulceroinfiltrative lesion at the level of 27cm from the incisor. Pathologic findings were that the covering epithelium showed well differentiated squamous carcinoma with invasive pattern and the stroma contained islands of sarcoma and squamousl cell carcinoma. Immunoreactivity to cytokeratin was not observed. Partial esophagectomy and esophagogastrostomy was done. We report a case of rare malignant esophageal carcinosarcoma.
