      • 슬와 낭종을 형성한 슬관절의 미만성 색소 융모 결절성 활액막염 - 1례 보고 -

        색소 융모 결절성 활액막염은 슬관절에 호발하고 슬와부로 팽창되는 경우는 드물며 이 경우 베이커씨 낭종이나 악성 종양으로 오진될 수도 있다. 저자들은 슬와부로 팽창되어 낭종을 형성한 미만성 색소 융모 결절성 활액막염을 관절경적 활액막 전 절제술과 낭종 제거술로 치료하였고 1년 이상의 추시 기간상 재발이 안된 1례를 보고하는 바이다. Pigmented villonodular synovitis most commonly occurs in the knee joint and rarely extends into the popliteal space, which not only has mimicked Baker's cyst but also has been misdiagnosed as malignant tumor. We report a case of a diffuse pigmented villonodular synovitis of knee joint which is extended into the popliteal space as like a popliteal cyst. We treated this case by arthroscopic total synovectomy and excision of cyst. There was no recurrence during more than one year after operation.

      • 관절경하 유관 나사를 이용한 족관절 관절 고정술 (3례 보고)

        There have been numerous methods to obtain a stable arthrodesis of the tibiotalar joint. However, a controvercy has arisen over the most successful and reproducible method to obtain an arthrodesis. The objective of this report is to show that an ankle arthrodesis can be performed successfully and advantageously by using cannulated screws under arthroscopic control and that our csaes will be a part of the multicentric study of arthroscopic ankle arthrodesis in korea. Between May 1996 and May 1997, we perfomed 3 ankle arthrodesis under arthroscopy using cannulated screws. Two patients had post-traumatic osteoarthritis and one had rheumatoid arthritis. Clinical ankylosis in one case and radiological union in two cases were obtained in 12 weeks postoperatively. We propose that arthroscopic ankle arthrodesis is effective procedure in patient who has minimal deformity and bone loss, advantages like small incision, minimal morbidity, low infection rate and rapid return to normal daily activity, but more cases and comparision with open tecnique will be needed.

      • 석회화 건염의 보존적 치료와 조기 관절경적 치료 결과

        목적: 석회화 건염에 대하여 보존적 치료 및 조기 관절경을 이용한 수술적 치료 후 통증과 임상경과를 비교 관찰하였다. 대상 및 방법: 2002년 2월부터 2008년 5월까지 석회화 건염으로 진단받은 30예 중 6개월 이상 추시관찰이 가능하였던 스테로이드 주사치료 15예 및 수술 치료 15예를 대상으로 하여 석회질의 통증과 운동 범위의 호전을 2주, 12주, 24주째 각각 비교 관찰하여, Constant-Murley score를 이용하여 평가하였다. 결과: 통증 정도는 Constant-Murley score상 보존적 치료군의 경우 수술 전 평균 3.2에서 치료 이후 2주, 12주, 24주째 각각 13.6점, 14.5점, 14.7점이었으며, 수술적 치료군은 수술 전 평균 3.3에서 수술후 2주, 12주, 24주째 각각 10.2점, 13.0점, 14.3점이었다. 운동 범위는 보존적 치료군의 경우 수술 전 평균 14.3에서 치료 이후 2주, 12주, 24주째 각각 21.7점, 31.3점, 35.7점이었으며, 수술적 치료군은 수술 전 평균 14.4에서 수술후 2주, 12주, 24주째 각각 33.1점, 35.8점, 36.4점이었다. 치료 2주째 및 12주째 두 간의 비교에서는 수술적 치료 군이 유의하게 우수하였으나, 최종 추시시는 통계학적인 차이가 없었다. 결론: 석회화 건염의 조기 관절경적 치료는 환자의 만족도를 높이고 조기에 일상 생활로 복귀할 수 있는 효과적인 치료법으로 사료된다. Purpose: We evaluated the level of pain and clinical presentation of calcific tendinitis after treatment with conservative and early arthroscopic operation. Materials and Methods: We reviewed 30 patients with calcific tendinitis with the minimum of 6 months of follow up period, treated from February 2002 to May 2008. We implemented Constant-Murley score to evaluate the pain of calcification and improvement of range of motion at the 2nd week, 12th week and 24th week with the patients who had treatment with steroid injection in 15 cases and operation in the other 15 cases. Results: Evaluating the level of pain using Constant-Murley score, we found that conservative treatment group had the scale of 3.2 before treatment and 13.6, 14.5 and 14.7 at 2nd, 12th, and 24th week, respectively since started treatment. The operation group, on the other hand, had the scale of 3.3 preoperatively and 10.2, 13.0, and 14.3 at 2nd, 12th and 24th week postoperatively. The range of motion in the conservative treatment group showed 14.3 on average before the treatment and 21.7, 31.3 and 35.7 at 2nd, 12th and 24th week after treatment. The operation group had 14.4 on average preoperatively, and 33.1, 35.8 and 36.4 at 2nd, 12th and 24th week postoperatively. The operation group had statistically significant improvement compared to the conservative group at the 2nd and 24th week, but showed no difference between the two groups at the final follow up. Conclusion: Early arthroscopic treatment of calcific tendinitis seems to be an excellent option that can increase the level of satisfaction of the patients and an efficient way of putting the patients back to their normal life promptly.

        「典雅化」的敘事趨向: 明話本小說「入話故事」之「簡敘」、「略敘」、「鋪敘」敘事藝術

        중국화본소설에서 입화고사는 본고사(正話)에 들어가기 전, 작품전체의 내용과 주제 및 다양한 서사기법을 미리 살펴볼 수 있는 중요한 의미를 지니고 있다. 입화고사는 내용과 서사형식에서 본고사와 긴밀히 연결되어 있을 뿐만 아니라, 작품 전체의 주제와 표현방식에 있어서도 유기적인 관계를 유지하고 있다. 특히 명대화본소설속의 입화고사는 서사기교와 형식, 내용에 있어서 ‘정형화’ㆍ‘유기화’ㆍ‘연계화’로 나타나는 특징을 구체적으로 보여주고 있다. 이러한 특징은 송원화본소설의 ‘대중화’와 차별되며, 청대화본소설에서 나타나는 ‘문인화’ 경향과도 다르다고 할 수 있다. 명대화본소설속의 입화고사 서사형식은 크게 세부분으로 나누어진다. 서사형식의 구조와 진행방식, 내용묘사, 그리고 편폭의 길이에 의해서 ‘簡敍’, ‘略敍’, ‘鋪敍’로 구분된다. 구체적인 서술방식에는 인물간의 대화로 이야기를 진행하는 ‘對話敍述’, 전체적으로 개략적인 내용설명만을 위주로 하는 ‘槪略敍述’, 이 두 가지 서술방식이 같이 운용되는 ‘綜合敍述’이다. ‘簡敍’에는 편폭의 제한으로 인하여 ‘對話敍述’이나 ‘槪略敍述’의 단독적인 운용이 많고, ‘略敍’와 ‘鋪敍’에서는 ‘綜合敍述’방식이 많이 등장하며, ‘略敍’보다는 ‘鋪敍’에서 ‘綜合敍述’의 비중이 높다. ‘略敍’에서는 ‘綜合敍述’이 단일구조로 나타나지만, ‘鋪敍’에는 여러 차례 중첩되는 중복구조를 보인다. 이러한 서술방식은 독자의 지적수준과 작가의 수사기교, 그리고 본고사의 서술방식과 밀접한 관련이 있다. 작가는 입화고사와 본고사 서술방식을 유기적으로 조절하여, 비교적 적절한 서사형식과 서술방식을 입화고사에 적용한다. 명대화본소설 입화고사 서사형식의 변화와 운용은 비록 청대화본소설에서 나타나는 정연하고 세밀한 방식에는 아직 미치지 못하지만, 송원화본소설에 나타나는 대중화의 서술방식보다는 훨씬 발전된 양상이다. 명대화본소설 입화고사의 서사적 기교와 예술적 성과는 송원화본소설의 대중화에서 청대화본소설의 문인화로 나아가는 과도기적 특징을 상세히 보여주고 있다.

        변화와 지속: ≪료재지이(聊齋志異)≫에 대한 ≪성몽변언(醒夢駢言)≫의 개편(改編) 연구 ― 서사내용의 ‘첨가’와 ‘삭제’를 중심으로

        In the process of remaking the works of Liaozhaizhiyi (聊齋誌異), Liaozhaizhiyi (聊齋誌異) and Xingmengpianyan (醒夢駢言) show distinct differences with regard to not only their narrative forms and structures but also the contents of the narratives and the themes. What is the most fundamental and important while studying the ‘samenesses’ and ‘differences’ between those two works is ‘the changes in contents of the narratives’. Over the course of the change, there appear ‘addition’ and ‘deletion’ in the ‘completion stage of the change’, and they are operational techniques with which we can look into the narrative contents change processes from multiple perspectives, because the addition and deletion of contents are easy to recognize and the degree of modifying the contents of the original and reproduced work is very precise and direct. The ‘addition’ of narrative contents consist of ‘insertion’ and ‘supplemetation’, and even though they somewhat differ in the process of adding new stories and in the range and the connection of the contents, but it is true that both of them include addition of some contents. In the process of reproducing Liaozhaizhiyi (聊齋誌異) into Xingmengpianyan (醒夢駢言), ‘addition’ is the characteristic that appears the most frequently, and it shows ‘polarization’. The reason that ‘polarization’ appears so remarkably is because the ‘manner of writing’ with which ‘the writer wants to succeed the ‘original’ as it is’ and the ‘intention of reproduction’ with which he wants toreproduce the work into the other that satisfy the tastes of the public. The ‘addition’ of narrative contents is more than mere expansion of a simple story, but rather it is insertion of another independent story, and therefore it is making the original plot richer and more complex. ‘Deletion’ also is an important characteristics that appears in the process of reproduction; it is profoundly related to ‘whether the major contents is included or not’ and ‘whether the narrative organicity is maintained or not’. Even though ‘deletion’ takes place less frequently compared to ‘addition’, it is widespread over the ‘introduction’, ‘development’, and ‘conclusion’ part of Xingmengpianyan (醒夢駢言), and it shows the change process more clearly. Looking into these phenomena of ‘addition’ and ‘deletion’ comprehensively, it is easy to figure out that the writer tries to achieve ‘organicity’ and ‘consistency’ of the contents by reproducing the work. Such ‘empirical arrangement’ and ‘narrative strategy’ is closely related to the narrative contents, rhetorical techniques, narrative forms, and narrative structure, and so on, and also has direct relationship with the reaction and responses from readers, the completeness and artistic result of the work, emphasis of the theme, and implication of meaning. This research is to comprehensively look into what differences and distinctions are made between two works by ‘addition’ and ‘deletion’ that appear in the narrative contents change process, and what narrative perspective is projected in the process. In addition, this research offers opportunity to study the correlation between the original work and reproduced work in fine detail, and to look into the organic relation among the writer, readers, and the work comprehensively and closely.

        `接受`與`排斥` ― 以`中等不一致效應`探究宋元話本小說`夢敍事`

        Sung Yuan`s Huaben novels(宋元話本小說) contain a diversity of narrative structures and contents that describe the world inside dreams, and it is able to look into their artistic characteristics from various perspectives. While traditional researches on Chinese novels have focused on describing characters` images and realizing subjects of works, only little attention was paid on the background and environment. The backgrounds of novels and the arrangements of the environment can be considered as important factors when it comes to expressing the main idea and characters` images of a work. Up to now, researches on Chinese Huaben novels have mostly leaned toward some dimensions such as characters` images, significance of the contents, and the main ideas. Even though such researches have hugely contributed to understanding of story composition and contents of a work and have made great accomplishments, it can be said the contemplation from various perspectives is insufficient. There have rarely been examples of contemplation on artistic characteristics of `dream depictions` from perspectives of `narration` and `communication` or `narration` and `delivery`. Especially, there was no case in which the artistic characteristics of fantastic depiction of dreams, which always appear in Sung Yuan`s Huaben novels, were analyzed from a new perspective. An author`s artistic performance inherent in his/her work can be shown throughout the way he/she controls the application of space, time, and objects when describing `dream depiction` and the way he/she combines characters, subjects, and environments organically. Even though the art of `depiction of dreams` did not receive as much attention as description of characters` image and main ideas did, and it has been always isolated from researches on Chinese novels, without professionally analyzing research, applied narrative techniques are considerably exquisite and complex enough to be studied. Hence, this paper will look into the inherent narrative intention and recognition throughout contemplation on the narrative structure and contents of dreams, based on `moderately incongruity effect`. Thereby, it will be able to compositively consider major narrative functions and narrative characteristics applied among Sung Yuan`s Huaben novels.
